Educational video describing the condition of gout and pseudogout. The most common joint affected by gout is the 1st metatarsophalangeal joint. The most common joint affected by pseudogout is the knee joint. Gout and pseudogout are similar problems with different causes. Gout Gout is caused by the build-up of uric acid crystals inside a joint. The best test to diagnose gout is with a joint fluid analysis. Elevated uric acid is not a good criterion. 90% of patients suffering from gout are men between the ages of 40-60 years. Gout crystals are needle-shaped and negatively birefringent. When placed under polarized light they will be yellow. Uric acid builds up in the body by two main mechanisms: 1-Excessive urate production. 2-Diminished urate clearance. Uric acid is produced from the breakdown of proteins inside the body and from the proteins of food that is eaten. Precipitating factors The sudden attack of gout can be brought on by anything that increases the level of uric acid in the blood such as: •Dehydration •Increased consumption of alcohol •Eating large amount of meat or seafood •Trauma/surgery Diagnostic testing •Aspiration and analysis of the joint fluid is the best method for diagnosis. •There are blood tests such as white blood cell count, C-reactive protein, erythrocyte sedimentation rate and uric acid level that are helpful in supporting the diagnosis if elevated, but if normal cannot definitively rule out gout or pseudogout. Pseudogout Pseudogout is a metabolic disease where calcium pyrophosphate dihydrate crystals (CPPD) are formed within the joint space. Most often affects the knee, occurs more in older patients. Calcification of fibrocartilage (chondrocalcinosis). Pseudogout crystals are rhomboid shaped and positively birefringent. Crystals will be blue when placed under polarized light. Associated conditions: •Hyperparathyroidism •Rheumatoid arthritis •Gout Gout and pseudogout both show a sudden onset of pain, redness and swelling typically affecting a single joint in 80% of the cases. Gout symptoms and signs include pain, swelling and arthritis. Patients with gout have periarticular erosions along with the formation of uric acid soft tissue masses in and around the joint which can be seen on x-ray. Soft tissue tophus deposition with periarticular erosions call “punch-out” lesions. X-rays in pseudogout will show thin calcification in the articular cartilage or menisci, with involvement of the patellofemoral joint. Calcifications of the synovium, tendon and ligaments can also be seen. Treatment of Gout & pseudogout •Acute gout: indocine, colchicine (be aware of peptic ulcer) •Chronic gout: allopurinol (xanthine oxidase inhibitor), colchicine •Uricosuric drugs such as probenecid which increase uric acid excretion by the kidneys may be helpful. •Treatment of pseudogout: NSAIDs, intraarticular injections. "Osteoarthritis (OA) with CPPD" reflects a situation where osteoarthritis features are the most apparent. Pyrophosphate arthropathy is a term that can refer to several of these situations. The exact cause of CPPD is unknown, although increased breakdown of adenosine triphosphate (ATP; the molecule used as energy currency in all living things), which results in increased pyrophosphate levels in joints, is thought to be one reason why crystals may develop. Familial forms are rare. One genetic study found an association between chondrocalcinosis and a region of chromosome 8q. The gene ANKH is involved in crystal-related inflammatory reactions and inorganic phosphate transport. Excessive calcium (due to hypomagnesemia) has a potential relationship with chondrocalcinosis, and magnesium supplementation may reduce or alleviate symptoms. Hyperparathyroidism, hypothyroidism, hemochromatosis, hypophosphatemia, hypomagnesemia and renal osteodystrophy are often also associated with chondrocalcinosis. Other diseases associated with chondrocalcinosis are Wilson's disease and osteoarthritis. In some cases, traumatic arthritis can cause chondrocalcinosis. The disorder is more common in older adults. The disease is often occult until crystal deposits are coincidentally detected and diagnosed by a pathologist in various orthopedic specimens. It may be asymptomatic, or it can be associated with osteoarthritis, or it can present as an acute or chronic inflammatory arthritis that causes pain in one or more joints. The white blood cell count is often raised. The arthritis is usually polyarticular (i.e., it leads to an inflammation of several joints in the body), although it may begin as monoarticular (i.e., confined to just one joint). CPPD crystals tend to form within articular tissues. In theory, any joint may be affected, but statistics show that the knees are the most commonly affected joints, as well as wrists and hips. In rare cases, pseudogout may affect the spinal canal and cause damage to the spinal cord. In many instances, patients may also have signs of carpal tunnel syndrome. Bone spurs are bony projections also known as osteophytes. Osteophytes cause irritation of synovial membrane, capsule and ligament resulting in severe pain. Shoulder Joint Osteoarthritis Is Associated With Irritation Or Inflammation Of Ligaments And Rim Of Glenoid Cavity. Shoulder Joint Osteoarthritis Is Associated With Tendon Inflammation And Causes Rotator Cuff Pain. Shoulder Joint Is Very Painful To Touch And Movements. Pain Does Respond To Anti-Inflammatory Medications. Pain Is Continuous In Advanced Disease and May Need Advanced Surgical Treatment. Joint Movements Are Restricted During Initial Phase And Later In Advanced Stage. Patient Is Unable To Carry Weights On Diseased Arm And Do Movements Beyond 20 To 25 Degree Rotation. b. Rheumatoid Arthritis of Shoulder Joint Rheumatoid arthritis is an inflammatory disease of shoulder joint. Soft tissue inflammation of capsule and synovial membrane causes thickening of capsule and synovial membrane as well as severe pain. Inflamed synovial membrane secretes excessive amount of synovial fluid resulting in joint effusion and swelling. Rheumatoid Arthritis causes severe pain and often pain is associated with mild fever. Pain is secondary to damaged cartilage and irritation of ligaments and tendon. Rheumatoid Arthritis also causes Inflammation of ligaments and tendon. Severe case may result in partial tear of ligament and tendon. In advanced stage of rheumatoid arthritis, shoulder joint often partially dislocate during rapid movement or rotation of shoulder joint. Subluxation is partial dislocation. Subluxation is either anterior or posterior. Subluxation causes severe sudden pain, which last for several hours. Pain does not respond to NSAIDs and opioids. c. Gout Normal shoulder joint movement is painless. Shoulder joint of patient suffering with gout is extremely painful. Pain may last on and often for few days. Gout is caused by inflammation resulting from uric acid deposits over synovial membrane, cartilage and rim of glenoid cavity. Uric acid causes inflammation of synovium, articulating cartilage and rim of glenoid socket. Pain is spread over front and back of the shoulder joint. Palpation, pressure and movement of shoulder joint result in severe pain. Tender Painful Swelling is observed over Shoulder Joint. Joint movements such as flexion, extension and rotation are extremely painful during the active period of gout. d. Pseudogout Pseudogout is rare and is a painful condition of the shoulder joint. Symptoms are often similar to gout. Calcium Pyro Phosphate crystals are widely spread in shoulder joint. Crystals are deposited over articulating cartilage, synovial membrane, joint capsule and glenoid cavity. Calcium Pyro Phosphate crystals causes inflammation of cartilage and synovial membrane. Inflammation follows swelling of cartilage and synovial membrane as well as increased secretion of synovial fluid. Pseudogout is diagnosed following lab study of synovial fluid. Crystals of Calcium Pyrophosphate are lineal long. Crystals are embedded in the surface of cartilage and synovial membrane. Crystals are also observed in Shoulder joint capsule and the edge of the humerus within the shoulder joint. Pain caused by pseudogout is often spread over front and back of shoulder joint. Pain is severe in the morning and gets less painful later in the day. Shoulder joint movement is restricted in the morning. Pain becomes less severe as the joint movements are improved during the day. Swelling is observed over front and back of the shoulder joint. Swelling is soft mostly caused by increased secretion of synovial fluid within the joint. e. Septic Arthritis Septic Arthritis is caused by infection of shoulder joint resulting in collection of abscess (pus) within the joint. Septic Arthritis can also be caused by penetrating injury or infection from other source spreads to joint through blood. Joint movements are painful. Joint capsule is inflamed. Movement of shoulder joint irritates.Inflame joint capsule causes severe intractable pain. Skin over the joint is red and hot indicates underlying infection and inflammation. Patient guards and protect the shoulder joint. Patients avoid any movement of shoulder joint. Shoulder joint muscles are contracted or in spasm to prevent shoulder joint movement, which result in severe pain.

Osteoarthritis is the most frequent cause of disability in the United States. A recent report claims that 50 million adults suffer from this disease. Osteoarthritis manifests as a gradual and progressive degeneration of various joints. The prevalence of Osteoarthritis increases with age, typically worsening after age 60; women are more susceptible than men. Though less prevalent than Osteoarthritis of the knee and the hip, osteoarthritis of the shoulder can be just as debilitating. Glenohumeral osteoarthrosis (shoulder) is classically characterized by pain, weakness, restricted motion, cartilage loss (which covers the joint surface of the upper arm bone and shoulder cavity), and can be associated with bony deformities called osteophytes. Treatment of shoulder osteoarthritis should be based on the patient's age, severity of symptoms, level of activity, radiographic findings and other medical conditions present. The non-operative treatment options currently include modification of activities, physical therapy, pharmacotherapy and injections with corticosteroids and/or visco-supplementation. Nonoperative treatment does not eliminate the actual pain cause but helps decreasing the symptoms. If this type of treatment fails, surgical treatment is a possible next step. The most definite treatment modality is a total joint arthroplasty (replacement of the joint with a prosthesis) with satisfactory reports on pain reduction and functional outcome. This treatment method works well in, in general, older patients. Due to various reasons, such as the lifetime of the prosthesis, young, highly active patients are often poor candidates for shoulder arthroplasty. The ideal surgical treatment for young active patients with symptomatic glenohumeral osteoarthritis is controversial. Dr. Millett has developed an aggressive arthroscopic (surgery through key holes) approach called Comprehensive Arthroscopic Management (CAM) for patients with advanced osteoarthritis as an alternative to arthroplasty. The condition occurs more in older age groups and 1/3 of the patients are over 75 years of age. x-ray changes of chondrocalcinosis is seen in the knees on radiograph. Gout affects more distal joints. Pseudogout tends to affect more proximal joints. an x-ray will show calcification of the meniscus in the knee or triangular fibrocartilage in the wrist. Rule out endocrine or metabolic diseases: •Hyperparathyroidism •Hypophosphatasia •Hemochromatosis •Rheumatoid arthritis •Hypothyroidism Differential diagnosis: gout Clinical presentation •Asymptomatic radiographic findings alone •Acute crystal induced synovitis •Chronic arthropathy The condition is commonly seen in the knee and wrist but may also occur in the acetabulum labrum, symphysis pubis and annulus fibrosus of the disc. It is a disease of articular cartilage, the gristle that caps the ends of long bones .Cartilage consists of a mixture of proteins, sugars, and collagen. Interspersed in this matrix of substances are chondrocytes, cartilage cells. The purpose of the chondrocytes is to manufacture new healthy matrix and keep it healthy. In the picture to your right you can see that osteoarthritic cartilage has cracks in it. These are called "fissures." Osteoarthritis starts when chondrocytes begin to manufacture destructive enzymes. In addition, there is a complex interplay of events that leads to bone spur formation and inflammation of the synovium (the lining of the joint), which causes further joint destruction. There are multiple factors that predispose to OA of the shoulder. These include: rheumatoid arthritis,previous surgery, instability, rotator cuff disease, and trauma. Trauma can come from recreational activities such as weightlifting... or from occupation. There are two endpoints that effective arthritis treatment must meet. The first is adequate control of symptoms. The second is possible slowing down or reversal of cartilage loss. So... what can be done? Unfortunately, many treatments have been tried but they all seem to fall short. The usual approaches to OA consist of non-steroidal anti-inflammatory drugs, analgesics, physical therapy, glucocorticoid injections, and exercises. Results are generally disappointing Most patients end up with a shoulder arthroplasty (shoulder replacement). While this is OK for many people, it is not a good option for the younger, healthy active person who wishes to continue to be active. Recent advances from standard to reverse procedures may be better. However, the most exciting news may be the use of mesenchymal stem cells combined with growth factors, administered after fenestration and scarification of the exposed bony. Rotator cuff arthropathy or cuff tear arthropathy is shoulder arthritis AS A RESULT and IN A SETTING of rotator cuff tears. Rotator cuff muscles hold the ball and the socket together and prevent the humeral head from moving out of the socket when the person raises an arm. In people with torn rotator cuff, the head of the humerus moves upwards and rub onto the acromion. This causes damages to the covering cartilage and eventually arthritis. Combination of cuff tear and arthritis is a devastating condition that seriously impairs functions of the shoulder joint. Post-traumatic arthritis – is arthritis that develops after physical injury to the joint. Chondrolysis - sudden loss of cartilage that happens occasionally after a shoulder surgery, commonly seen in association with infusion of local anesthetics into the joint for pain management.

Do not remove the omega 6 fully, it is still needed but in moderation. Eat more omega 3 acids of olive oil, flaxseed, nuts and pumpkin seeds. GRAINS AND REFINED FLOUR. Opt for the healthy options like lean proteins (beans, seeds, nuts) and less whey. REFINED SALT. This is the table salt we use daily. Limit this salt use and you will keep all the calcium needed for healthy bones, against fractures or osteoporosis even. Eat it in minimal doses or buy Himalayan pink salt or sea salt. Also, avoid junk food – it is too salty. MONOSODIUM GLUTAMATE FOODS. The MSG additive makes better flavor in store bought foods like frozen snacks, chips, frozen dinner, Chinese food, salad dressing and soups. It is especially bad for those with rheumatoid arthritis. Eat fresh and natural, homemade foods. Avoid the aspartame foods too, like artificial sweeteners. BEER. Too much alcohol impacts the joints first. Those having beer often and too much, have worse joint health. the purines of it turn to uric acid and this inflames the joints. Beer also has gluten in it and those with fragile joints must avoid it all the time.

Shoulder Arthritis Stretches & Exercises - Ask Doctor Jo

These shoulder arthritis stretches and exercises should help relieve the pain caused by shoulder arthritis. See Doctor Jo’s blog post about this at: http://www.askdoctorjo.com/shoulder-arthritis The first stretches are called pendulums. Use a chair or counter top for balance, and lean over so your arm hangs down towards the ground. Move your body, not your arm in circles so your arm swings around like a pendulum. You can also rock front to back and side to side. Start off with 10 of each and work your way up to a minute of each. Then you are going to be in standing using a weight or resistive band. Try to keep your arm straight the whole time and don't bend your elbows. Your thumb is going to be pointed upwards. The first two are front and back which are flexion and extension. You don't need to raise the band way above your head. You can bring it to a 90 degree angle which is where your arm is parallel to the floor for flexion. Now you are going do put your arm out at a 45 degree angle which we call scaption. Now you will do shoulder shrugs. Lift your shoulders up towards the ceiling, but try to keep your head and neck in one spot. Don’t bring your ears down towards your shoulders, lift your shoulders up. Do 10 of these. Now for shoulder circles. You can roll them clockwise, and then counter clockwise. This will stretch out the shoulder and neck muscles. Start with10 each way, and work your way up from there. The next exercise is going to be rows or scapular retraction. You can use your feet as an anchor. You want to keep your elbows in close to your body and make a rowing motion. Try to squeeze your shoulder blades together when you pull back. Make sure you are using a slow controlled motion. For the final stretches you can use a Swiss/therapy ball, or if you don’t have one, you can use a table or counter top. This is an active assisted stretch, which means you are moving the arm now, but the ball will support the weight. You will slide your arms forward with your thumbs facing upward towards the ceiling and lean your body forward until you feel a stretch. You can stabilize the ball with your other hand, or stretch one shoulder at a time. You can also go out at an angle like the scaption or completely out to the side to stretch your pec muscles as well.
