DVA skin cancer management  There are many types of skin cancers including BCCs or basal cell cancers, SCC or squamous cell cancers as well as melanomas. Todays review is on Bowen disease or Intraepidermal Squamous Cell Carcinoma. This is a common non-invasive cancer of the skin affecting the epidermal layers of skin. IECs most frequently occur on sun-exposed areas and present as red, scaly and persistent patches. IECs may arise de-novo, or from lesions of solar keratosis. If left untreated, Bowen disease may progress to an invasive Squamous Cell Carcinoma. This is one of the most common skin cancers I see in DVA or Department of Defence patients in Australia, and is due to the high UV radiation we have in cities like Brisbane.  Clinical Features Bowen disease most frequently presents as a persistent red scaly patch that slow grows over time. The most commonly affect areas include the legs, arms, and face- areas of sun exposure. Bowen disease or IECs can also affect the lips, oral mucosa, or even genital areas. As Bowen Disease is asymptomatic there is often a delay in the diagnosis, as it may mimic conditions such as dermatitis, psoriasis, tinea, and sun-spots.  Associations Most cases of Bowen disease arise from sun-damaged skin, including areas of actinic keratosis. The most common associations include- UV radiation. This plays a major contributing factor in the development of IECs. Immunosuppression. Patients on certain immunosuppressive medications are more likely to develop IECs.  Arsenic exposure. This was a common cause of Bowen Disease in the past, as arsenic was frequently used in medicines during the 60s and 70s including Bells Asthma Mixture, sheep dip, and as pesticides. Arsenic induced IECs-Bowen disease is a rare event today, as occupational safety has increased. A time lag of 10-20 years is seen between exposure and cancer.  Human Papilloma virus infections. Other associations include genetic diseases, radiotherapy and skin lesions such as porokeratosis, and solar keratosis. Investigations Bowen’s Disease can be suspected clinically, however in the majority of cases, a biopsy should be performed to confirm the diagnosis.  Treatment  There are multiple ways to manage Bowens/IECs. Treatment modalities will depend on the location and size of the cancer, the number of lesions, the degree of functional impairment.  Each modality has both advantages and disadvantages.  Cryotherapy or liquid nitrogen  Liquid nitrogen can be an excellent method of treating IECs. Most cases will require a double –freeze cycle of cryotherapy. This treatment is not recommended for IECs below the knee, as poor healing may occur.  Creams  Imiquimod, or 5-fluorouracil are effective in the treatment of Bowen disease. Clinical studies have shown cure rates approaching 80-90%. Treatment appears to be most successful when these creams are applied for 6-16 weeks.   Curettage and Electrocautery When small patches of IECs are present they may be treated via curettage and cautery. A local anaesthetic is given prior to the procedure to prevent pain and discomfort.  Photodynamic therapy  Photodynamic therapy or PDT can be an excellent non-surgical method of treating skin cancers such as Bowen’s/ IEC’s. PDT uses  a photosensitive cream called 5-aminolevulinic acid or methyl aminolevulinate, that is applied to the skin cancer. The cancerous cells of Bowens/ IEC take up this chemical over a period of 3 hours. A low-level laser light then activates the chemical, and destroys the skin cancer cells. Patients will require two treatments spaced a week or two apart. DVA or the Department of Veteran Affairs in Brisbane, covers PDT for both sunspots on the face and also IECs on all areas. PDT can also be used for multiple or large IECs. The benefit of PDT is that it selectively targets neoplastic cells without affecting the surrounding epidermis. This results in improved cosmetic outcomes, and faster healing times.  Surgery  Surgery is the ideal method of treating IECs in certain areas including recurrent IEC or ones that dwell in hair follicles. Surgery has the highest cure rate for Bowens/ IECs. In the few weeks following complete healing, apply the mixture once a day by placing a cotton pad soaked in apple cider vinegar over the paste. Cancer can originate from the inside of the skin, so it is important to continue the treatment to ensure that the mixture penetrates the affected layers.

The researchers were prompted in their endeavor by the fact that HCTZ has been linked with an increased risk of lip cancer in the past. In fact, a study led by Pottegård attributed 11 percent of lip cancer cases to the drug. Moreover, HCTZ, the authors explain, makes the skin more sensitive to the damage of ultraviolet (UV) rays, due to its photosensitizing effects. This was a further reason for the researchers to examine the drug. HCTZ raises skin cancer risk Using national databases, Pottegård and colleagues examined the use of HCTZ in over 80,000 patients who had been diagnosed with non-melanoma skin cancer, and compared it with its use in a group of 1.5 million healthy controls. Additionally, the researchers considered the use of other antihypertensive medications. The research revealed that those who took HCTZ were up to seven times more likely to develop skin cancer. More specifically, the antihypertensive drug raised the risk of both squamous cell carcinoma and basal cell carcinoma – a less serious form of skin cancer. By contrast, none of the other antihypertensive drugs examined seemed to raise skin cancer risk. "We knew that hydrochlorothiazide made the skin more vulnerable to damage from the sun's UV rays, but what is new and also surprising is that long-term use of this blood pressure medicine leads to such a significant increase in the risk of skin cancer," comments Pottegård. If you have been diagnosed with a melanoma, the first step will be to cut out the actual tumor and the surrounding normal tissue. The amount of normal tissue needed for the examination will be determined by the depth of invasion by the melanoma. The next step will be to determine if your lymph nodes have been involved. If they have not been involved, likely the only thing you will need is a simple excision. If there are melanocytes, and melanoma cells in the lymph node, it is very likely that you will need some sort of chemotherapy. The type of chemotherapy to be used will be determined by our oncologist. Rarely, radiation therapy is also added to this regimen, if the size of the initial tumor was too big or if the surrounding tissue has been involved. The treatment for non-melanoma skin cancers tend to be easier. They rarely metastasize to the rest of the body, so often times a simple excision at the site of the tumor is all that's needed. However, if the tumor is too large or has grown and spread into adjacent structures, radiation therapy can be utilized to shrink the tumor, either before or after the primary excision.

But at the end of the treatment and the short healing period, you're going to have much better skin. Take this treatment seriously. It is a topical form of chemotherapy and it works in the cell's DNA and RNA. Read up on the drug before starting. As you'll see in the video, and hopefully your own experience, your skin can heal very fast. I am very happy with the results and hope I never have to do this again. All of my scaly areas or places that were always flaking are gone. The results were amazing. Looking back on it now - I should have done my whole face at once and gotten it all over with sooner. Like I said, though the results are great, this is not a walk in the park. When I said it made me physically ill, I mean i felt fatigued, tired and brain dead. Like I said, this is serious medicine. On shaving - yes, you can shave, but just take it easy. No one will be touching that skin anyway, so don't worry about that close shave for a week or so. I used a GoPro 3 for this video. Photodynamic Therapy – PDT Photodynamic therapy is a treatment for acne and skin conditions such as sun spots, actinic keratosis, solar keratosis and some forms of skin cancer called IEC or Bowen’s disease. This treatment can also treat superficial BCCs or basal cell cancers. How is Photodynamic Therapy / PDT performed? Photodynamic therapy is a non-surgical and well tolerated treatment that combines the use of a special photosensitizing agent (ALA). This chemical is activated with a specific wavelength and intensity of light. It is a two-part treatment. First, the drug is administered to the skin via cream. An incubation period (wait time) of 90-180 minutes is need for absorption of the cream. This is followed by exposure under a specific light source for several minutes. In the United States the most popular is blue light combined with Levulan. In Australia, we use Metvix and Aktalite or Healite using the red light spectrum. What areas can be covered with photodynamic therapy? PDT is known as a ‘field treatment’, and is applied to the affected area and surrounding tissue. Virtually any area can be treated with PDT, including the face, scalp, hands and chest. PDT in the blue and red light spectrum can also treat acne, zits, pimples and blackheads. Is PDT painful? Patients often experience slight burning and stinging during the procedure. Patient comfort levels are helped with the administration of panadol one hour before illumination. A cool spray of nitrogen during the procedure will also decrease the stinging. If necessary an anaesthetic cream such as EMLA can be used to alleviate any discomfort during the procedure. As a guideline, 95% of patients do not require an anaesthetic. For large areas such as the face or scalp, we recommend an over the counter pain killer such as Panedine to be taken after the procedure. Levuan ALA is less painful than Metvix PDT as blue light is more superficial. What can I expect after photodynamic therapy treatment? Due to the photosensitivity of the drug used, patients may feel sunburned and occasionally swollen after the treatment. This usually subsides within 1-3 days, depending on the extent of PDT and the severity of the treated condition. Peeling and redness usually occurs for 3-6 days after treatment. Patients will need to avoid sun exposure for 48 hours after the treatment. How long does photodynamic therapy take and how much time off work will I need? Preparation of the area for PDT typically takes 10 to 15 minutes. This may involve degreasing the skin or in severe cases a laser assisted micropeel. The cream is the placed on the target area for 1-3 hours. This ‘wait time’ allows adequate absorption of the ALA cream to the target areas. The second part of the treatment (activation of the cream with a specific light source) takes several minutes to complete.) How much time off work will depends on the site and extent of treatment. As a guide the downtime for full face PDT is approximately 3-5 days, which compares favorably with Efudix treatment (downtime of 3-4 weeks). How is PDT different form other treatments for skin cancer? With PDT there is: Less potential for scarring Less down time and quicker recovery (typically days, not weeks/months) High cure rates. How does PDT compare to other methods such as freezing and Efudex therapy? Sunspots or solar keratosis is a common pre-cancerous lesion seen in the Brisbane. Efudex needs to be used twice a day for up to 4 weeks. PDT in contrast to Efudex, is a one off treatment for Solar keratosis. Treatment is over within hours, and downtime shortened to 6-7 days. DVA or the Department of Veteran Affairs and PDT. IF you hold a White Card for skin or a Gold Card, PDT or photodynamic therapy is covered! Patients are not out of pocket, as this treatment is subsidized for the treatment of solar keratosis –sun damage on the face, as well as pre-cancer and skin cancer anywhere on the body. Dr Davin Lim works closely with DVA for legal reports on sun damage to help you obtain this treatment, if you are a suitable candidate. Though PDT is an excellent treatment, I do feel that Fraxel fractional laser resurfacing has now taken over as the best treatment for sun spots. This is technology for you- lets move with the times. Dermatologist tips on skin cancer and Melanoma Are you at risk? In this video, I get pretty nerdy and go into some serious detail about Melanoma, the different kinds of skin cancers and if you are actually at risk. The good news is it’s 99% curable over a five-year period if caught early enough. “Early enough” means the Melanoma growth is still mostly on the epidermis. If it has spread deep into the dermis of your skin, it is considered “late.” That’s why it’s SO IMPORTANT to know what to look for. Who gets Melanoma? For men under 50, it’s the #1 cancer. Women under 50 still get more Melanoma than men, but it’s the #3 cancer (this is due to the number of breast and thyroid cancers). It’s common for both genders under 50. The most vulnerable group is Caucasian men over 55 years of age. And the fairer your skin, the higher your risk. That doesn’t mean darker complexions are in the clear. In fact, the darker your skin tone, the more likely Melanoma will be caught later and be deeper into your dermis. Melanoma can occur in children too. It’s rare, but it can be very aggressive. What puts me at higher risk of Melanoma? Melanoma is directly related to increased sun exposure, and your chances are doubled if you’ve had five or more blistering sunburns. Blistering means your skin was very red and it hurt to touch it. Tanning beds are another contributor. Some statistics: Use of a tanning bed before age 35 increases your chances by 75%, and using a bed 10 times over your life increases your chances by 30%. Chances are higher you will develop Melanoma if you have a lot of moles. If you have fair-colored skin and light colored eyes, you’re at higher risk. Especially, if you are a redhead or have redheads in your gene-line. A history of any skin cancers can also put you at a higher risk. Lastly, if you have a family history of Melanoma, then you are at a higher risk of getting it, too. What are the warning signs of Melanoma? A - Asymmetry - Is the mole a perfect circle or an oval or does it take a funny shape? B - Borders - Are the borders of the mole crisp and clear? Hopefully, so. C - Color - What is the color in the mole and around it? Ideally, it doesn’t have a halo. D - Diameter - Is it larger than a pencil eraser? Then you should have it looked at! E - Evolution - Does it change over time? A benign mole won’t change quickly. Advocate for Yourself If you are concerned you may have Melanoma, and you see your dermatologist who says, “Well, let’s just see what happens over the next month,” I’d encourage you to persist. If you have a strong or funny feeling that it’s Melanoma, then you should consider having it tested. Better safe than sorry! In order to reduce your chances of Melanoma, it’s crucial that you understand proper sun protection. 86% of Melanoma is the result of UV-radiation exposure, either sun or tanning-bed exposure. There are many treatments for skin cancer. A dermatologist selects treatment after considering the following: Type of skin cancer. Where the skin cancer appears on the body. Whether the skin cancer is aggressive. Stage of the cancer (how deeply the skin cancer has grown and whether it has spread). Patient’s health. After considering the above, your dermatologist will choose 1 or more of the following treatments for skin cancer. Surgical treatment: When treating skin cancer, the goal is to remove all of the cancer. When the cancer has not spread, this is often possible. To remove skin cancer, the following surgical treatment may be used: Excision: To perform this, the dermatologist numbs the skin and then surgically cuts out the skin cancer and a small amount of normal-looking skin. This normal-looking skin is called a margin. There are different types of excision. Most excisions can be performed in a dermatologist’s office. Mohs surgery: A dermatologist who has completed additional medical training in Mohs surgery performs this procedure. Once a dermatologist completes this training, the dermatologist is called a Mohs surgeon. Mohs surgery begins with the surgeon removing the visible part of the skin cancer. Because cancer cells are not visible to the naked eye, the surgeon also removes some skin that looks normal but may contain cancer cells. This part of the surgery is performed one layer at a time. After removing a layer of skin, it is prepared so that the surgeon can examine it under a microscope and look for cancer cells. If the surgeon sees cancer cells, the surgeon removes another layer of skin. This layer-by-layer approach continues until the surgeon no longer finds cancer cells. In most cases, Mohs surgery can be completed within a day or less. The cure rate for skin cancer is high when Mohs surgery is used. Curettage and electrodesiccation: This surgical procedure may be used to treat small basal cell and squamous cell skin cancers. It involves scraping the tumor with a curette (a surgical instrument shaped like a long spoon) and then using an electric needle to gently cauterize (burn) the remaining cancer cells and some normal-looking tissue. This scraping and cauterizing process is typically repeated 3 times. The wound tends to heal without stitches. Sometimes, curettage is used alone. Other treatments: Surgical treatment is not right for every case of skin cancer. Some patients cannot undergo surgery. Sometimes, surgery cannot remove all of the cancer, and more treatment is used to help get rid of the cancer. If the skin cancer is caught very early, surgery may not be necessary. Other treatments for skin cancer are: Immunotherapy: This treatment uses the patient’s own immune system to fight the cancer. The patient applies a cream (generic name is imiquimod) to the skin as directed by the dermatologist. Cryosurgery: The dermatologist freezes the skin cancer. Freezing destroys the treated area, causing the skin and cancer cells to slough off. Chemotherapy applied to the skin: The generic name for the medicine used in this treatment is 5-fluorouracil or 5-FU. The patient applies 5-FU to the skin cancer. It destroys the damaged skin cells. When the skin heals, new skin appears. Chemotherapy: If the cancer spreads beyond the skin, chemotherapy may kill the cancer cells. When a patient gets chemotherapy, the patient takes medicine. This medicine may be swallowed, injected (shots), or infused (given with an IV). The medicine travels throughout the body and kills the cancer cells. The medicine also destroys some normal cells. This can cause side effects, such as vomiting and hair loss. When chemotherapy stops, the side effects usually disappear. Photodynamic therapy: This treatment consists of 2 phases. First, a chemical is applied to the skin cancer. This chemical sits on the skin cancer for several hours. During the second phase, the skin cancer is exposed to a special light. This light destroys the cancer cells. Radiation therapy: Radiation may be used to treat older adults who have a large skin cancer, skin cancers that cover a large area, or a skin cancer that is difficult to surgically remove. Radiation therapy gradually destroys the cancer cells through repeat exposure to radiation. A patient may receive 15 to 30 treatments. This treatment is often only recommended for older adults. Many years after a person is exposed to radiation, new skin cancer can develop. What outcome can someone with skin cancer expect? If it is caught early and properly treated, skin cancer can be cured. Even melanoma, which can be deadly, has a cure rate of almost 100 percent when treated early. Even if you get a clean bill of health, you need to continue to see your dermatologist. Once a person gets skin cancer, the risk of getting another skin cancer is higher. Sometimes skin cancer returns. Your dermatologist will tell you how often you should return for checkups.

Those given the supplement had less swelling and demonstrated fewer indications of pain. However, more research is needed to determine the safety and efficacy of this use in humans. Dosing The appropriate dose of graviola depends on several factors such as the user’s age, health, and several other conditions. At this time there is not enough scientific information to determine an appropriate range of doses for graviola. Be sure to follow relevant directions on product labels and consult your pharmacist or physician or other healthcare professional before using. Possible Side Effects DO NOT EAT THE FRUIT: Eating the fruit could lead to movement disorders similar to Parkinson’s disease, according to a case-control study in the French West Indies. No established human dosage for graviola leaf exists yet. Long-term or high-dose supplementation may cause damage to the myelin sheath, disrupting normal nerve function. Movement disorders may also occur because of compounds called alkaloids present in the plant. Symptoms of these conditions are similar to those of Parkinson’s disease. Talk with a doctor prior to taking a graviola leaf supplement or making/drinking soursop tea. How to Make Soursop Tea Directions: Boil 1 liter of water, then take 15 soursop dry leaves (fresh or dried ones both have same value) and 1 small stem, cut into small pieces and put into the boiling water. Don’t close the vessel and Continue to boil the water on small flame for 30 mins,until the water evaporates to 500 ml . Now the soursop tea is ready to drink. Drink 1 cup (165ml) filtered soursop tea hot or warm or cool , 3 times every day, in the morning, afternoon and in the night. For better taste you can add 1/2 spoon of lime juice ,2 pudina leafs and add some honey (not sugar). After drinking, the body may feel the effects of heat, but no side effects. To cool the body drink 3 glasses of fresh watermelon juice, and drink 3 glasses of goats milk every day. In Uganda, many albinos with cancer can not access necessary treatment, with many hopelessly retiring to their homes to wait for death. "I had my own personal tanning bed in my home, and so did a lot of my friends growing up. ... Everyone tanned," Willoughby said. After one of her classmates in nursing school was diagnosed with melanoma, Willoughby made her first dermatology appointment at age 21. Sure enough, she had skin cancer. I now walk around with three scars that remind me every day to wear an SPF. Please do the same. A tan is never worth it. It really isn’t. Institute Esterderm - while highly favourable with bloggers is not a company i recommend and the article i wrote mentioning this product i have asked to be taken down. While it might be the next best thing - any company promoting DNA damage (a tan) is a company i cannot recommend. Especially when using that product i did (as they said) go a lovely shade of brown but did (as they said wouldn't) receive sun damage in the form of sunspots and pigmentation. While it may be tempting to skip the sunscreen step in your morning routine when the letters "SPF" are written on every other bottle you're picking up, don't give in. "You would need to apply 14 times the amount [of powder] people normally use," Baumann says. The same goes for foundation—unless you want to slather on seven times as much as you would actually use to even your skin tone. Unfortunately the SPF12-15 usually found in regular moisturisers doesn't cut the mustard, as we don't re-apply it every two hours and we simply don't use anywhere near as much as the thick layers they slap on when they test sunscreens in labs. You need to apply a broad-spectrum protection with SPF (UVB) 30-50 and high UVA protection, so you'll have some decent protection throughout the day. Do I still need to apply it on a cloudy day? While you may not get sunburnt, your skin can still be exposed to significant amounts of UVA without even noticing. UVA can penetrate window glass and clouds. I always recommend the same sun protection level of broad-spectrum SPF 30-50 whether the sun is out or not. Remember 30-20-2-1: 30: The minimum SPF you should use (other must-haves water resistance and a broad spectrum formula). 20: The number of minutes before you go out in the sun that you should apply sunscreen. That is, unless your lotion has titanium dioxide or zinc oxide, which are effective immediately. 2: The number of hours you can go without reapplying if you're not sweating or in the water. 1: The number of ounces you need to coat your body with enough product (a 0.002 mm layer) to provide the SPF listed on the label. If you're using lotion, that's about the amount that would fill a shot glass; if you're using a clear, continuous spray, that's 30 to 90 seconds of spraying—enough to create a visibly glossy sheen as it goes on. I know too much sun can cause cancer, but aren't the chemicals in sunscreens unhealthy too? Use the sunscreen. Although a few studies have raised questions about the safety of oxybenzone and retinyl palmitate, two ingredients found in some sunscreens, they are entirely avoidable if you read labels. So, what are potential concerns about those two components? Oxybenzone has been shown to cause hormone disruption in studies of cancer cells (however, a study on its effect when applied to skin did not show any statistically significant change in hormone levels).
