Rheumatoid arthritis is a chronic disease of small joints of hands and feet which cause synovitis and deformities and is a very painful condition. This disease is classically characterized by periods of remissions where the patient is absolutely pain free and comfortable with deformity of hands and feet he has. Periods of exuberation, where the disease gets progressed leading to inflammation of the joints, swelling, low fever, severe pain which again leads to further deformities of the joints. Typical features of Rheumatoid arthritis are involvement and deformities of small joints of hands and toes, large joints are very rarely involved, when they are involved disease is of severe category. Patient normally has morning stiffness. Few patients have some modules in their hands then they have loss of appetite, their bones are weak, they break at minimum injury and are painful.

8 Signs and Symptoms of Rheumatoid Arthritis Rheumatoid arthritis is a serious autoimmune disease that attacks the joints and other body parts. But RA can be tough to diagnose. Symptoms can mimic other illnesses, or they may flare, then fade, only to flare again somewhere else. Lab tests aren’t perfect—you can test negative for RA factors and still have it. And X-rays don’t show signs until later on. Here are some tricky rheumatoid arthritis symptoms and hints that they’re due to RA and not some other condition. Hard to heal injuries It’s possible to think you have an injury—such as a sprained ankle that doesn’t seem to heal—when the symptoms are actually due to RA. This is more common in younger people, says Lisa A. Mandl, MD, MPH, assistant attending rheumatologist at Hospital for Special Surgery in New York City. Numbness or tingling in the hands What happens is that the swelling in the arm compresses the nerves going into the hands. The sensation is often worse at night. If you go to a doctor with these symptoms and don’t have (or tell him or her about) other RA symptoms, you may be diagnosed only with carpal tunnel syndrome. Foot trouble One area in which people often have RA-related pain or inflammation is the forefoot. Women often stop wearing heels and head to a podiatrist due to the pain. Some people with RA may also develop pain in the heel because of plantar fasciitis, a common foot disorder caused by swelling of the tissue at the bottom of the foot, near the heel. Eye problems People with RA are also at risk for Sjogrens syndrome, an autoimmune disorder that can cause dryness of the eyes, mouth, nose, throat, or skin due to inflammation that stops glands from releasing moisture, This can happen even in the early stages of RA, but it’s unlikely to be the only symptom. Most people with dry eyes head to an eye doctor to find out the cause, but Dr. Mandl recommends telling your doctor—even an eye doctor or other specialist—about additional symptoms you’re having in any part of the body. Pairs of achy joints One of the most predominant symptoms of rheumatoid arthritis is aching in the joints. People often think their pain is due to overexertion or osteoarthritis, the type of arthritis common in old age. This achiness can also be misdiagnosed as fibromyalgia or chronic fatigue syndrome (fatigue is another symptom of RA). RA joint pain is not fleeting; it usually lasts longer than a week. It can also be symmetrical, meaning both hands, feet, knees, or ankles will be affected at the same time. Morning stiffness Another characteristic of rheumatoid arthritis is stiffness in the joints in the morning. Again, this is also a common problem in osteoarthritis, which can cause pain after long periods of inactivity, like sleeping. The difference between the two is that osteoarthritis pain usually subsides in about a half hour. Stiffness from rheumatoid arthritis will last much longer, possibly for a good chunk of the day. The right kind of exercise can help alleviate stiffness for people with RA and osteoarthritis pain. Locked joints People with RA can sometimes experience locked joints, particularly in the knees and elbows. This happens because there’s so much swelling of the tendons around the joint, the joint cannot bend. It can lead to cysts behind the knee that can puff out and inhibit motion. The symptom can be mistaken for a meniscus tear, a knee joint injury that's common in sports, and which can also lead to cysts. Nodules These are firm lumps that grow under the skin near the affected joints. They often appear at the back of the elbows, and sometimes people get them in the eyes. They're more common in people who have advanced rheumatoid arthritis, but occasionally show up earlier, The nodules can at times mimic gout, another form of arthritis.

Plummer-Vinson Syndrome associated with Mutilans Rheumatoid Arthritis The gastric cardia seen from an retrogade endoscopy from the hole of surgical gastrostomy performed 16 years previous, The gastrostomy tube is removed. A standard endoscope is passed retrograde to the upper esophageal sphinter. This is the case of a 65 year-old female who had suffered from a mutilans rheumatoid arthritis since 19. She had difficulty to swallow, and therefore she had not been tasting flavors well during 16 years. She arrived for a second opinion, and on two different clinics they didn´t do an endoscopy becasue of Temporomandibular joint involvement. The only treatment they did was to place a gastrostomy tube to feed her. Deformity of both hands is observed, the patient had suffered from 19 years of this rheumatoid arthritis, There destruction of finger bones, due to arthritis of the temporomandibular joint, do not open her jaw, and impossible to do an upper endoscopy, the patient could not wander, comes in a wheelchair, there are deformation on feet, and two previous surgeries were placed prosthetic knees. For the therapeutic management, we took a conservative approach because of her status and oxygen saturation. However we had intended to try an aggressive plan, like using a hydrostatic dilator and combine it with argon plasma. Also, because of her arthritis, her temporo-mandibular joint is compromised so the endoscopy was not able to be done properly, so we were planning on doing it through the nasal airways. The endoscopy was finally done via a gastrostomy fistula. When getting to the superior esophageal sphincter, the inferior part was surrounded with a membrane. Rheumatoid arthritis symptoms, treatment, nursing interventions, and medications NCLEX review lecture. Rheumatoid arthritis (RA) is an autoimmune condition that causes inflammation of the joints. It specifically affects the membrane lining of the joint called the synovium. Rheumatoid arthritis can eventually lead to complete bone fusion and severe immobility. Stages of rheumatoid arthritis include: synovitis, formation of a pannus, and bone ankylosis. Rheumaotid arthritis symptoms include: soft, tender, warm, and swollen joints. The patient will feel very tired and experience a fever. RA affects the same joints bilaterally, therefore it is symmetrical. In addition, to the stiffness and pain being worst in the mornings (greater than 30 minutes or more) or after long periods of inactivity. Rheumatoid arthritis affects all types of joints. In contrast, osteoarthritis tends to affect the weight bearing joints. Most commonly the fingers and wrist are affected in rheumatoid arthritis, but it can also affect the neck, shoulders, elbows, ankles, knee, and feet. RA doesn't just affect the joints but it can extend to the heart, skin, eyes, mouth, lungs, and cause anemia. In osteoarthritis the joints are only affected, not the system. Rheumatoid arthritis treatment includes: heat and cold therapy, using assistive devices, physical exercise (ROM and low impact aerobics) along with plans for periods of rest, always resting an inflamed joint, medications: NSAIDs, corticosteriods, and DMARDs, and surgery. There is no cure for RA. Osteoarthritis vs. rheumatoid arthritis (OA vs. RA) nursing lecture about the symptoms, pathophysiology, treatment, and nursing interventions for the NCLEX exam. Rheumatoid arthritis and osteoarthritis are both forms of arthritis that affect the joints but in different ways. Osteoarthritis is the most common type of arthritis that develops due to the deterioration of the articular cartilage. Rheumatoid arthritis is an autoimmune condition that causes inflammation of the synovium within the joint space. In severe cases of RA, the patient can experience bone fusion (ankylosis). Osteoarthritis vs rheumatoid arthritis symptoms include: OA: crepitus, bony outgrowths (Heberden and Bouchard's Nodes which are found on the hands..see pictures in video), joints will feel hard, bony, morning stiffness LESS than 30 minutes, only affects joints (not other systems of the body), and is unsymmetrical. RA: morning stiffness GREATER than 30 minutes, soft, warm, swollen joints, affects joints symmetrically and other systems of the body (fever, anemia, heart, lungs etc.) Treatment for osteoarthritis and rheumatoid arthritis include: NSAIDs, tylenol, physical exercise (low-impact exercises, range of motion, wearing-bearing), assistive devices, heat and cold therapy, corticosteriods (oral for RA and intra-arthritic injections in OA). In severe cases of rheumatoid arthritis DMARDs (disease-modifying antirheumatic drugs) may be used, and these medications suppress the immune system from attacking the joint along with helping slow down the destruction of the disease on the joints and bones. Dr. Ebraheim educational animated video illustrates spine concepts associated the cervical spine - rheumatoid arthritis. Cervical spine involvement occurs in about 90% of the patients with rheumatoid arthritis. All rheumatoid arthritis patients should have cervical spine examination. Start with getting cervical spine x-rays, because this helps to diagnose atlantoaxial instability. Early aggressive medical treatment can decrease this risk. C1-C2 instability is common and can occur in up to 80%. It occurs due to transverse ligament pathology. So you will need to get flexion extension views in patients with rheumatoid arthritis, especially preoperative x-rays, and if it looks bad, you have to stabilize the spine before doing elective total hip or total knee procedures. Discover the C1-C2 instability and fix it first before doing elective total hip procedure. You see in the x-rays the Atlanto Dental Interval: A.D.I., if it was more than 3.5 mm that means instability of the upper cervical spine may be present. If it is more than 7 mm it means disruption of the alar ligament, these patients can have cervical spine myelopathy. The A.D.I. is an unreliable predictor of paralysis. The posterior atlanto dental interval is a better predicting test, it can predict the spinal cord injury better. If the posterior A.D.I. is less than 14 mm it can predict spinal cord injury, get an MRI. The surgery is done if the A.D.I. is more than 10mm or if the P.A.D.I. is less than 14mm, the operation is C1-C2 fusion. Clinically: the C1-C2 instability could give neck pain, headache, and myelopathy with abnormal gait, paresthesia and difficulty in fine motor control. Basilar Invagination: Occur in about 40% of the patients with rheumatoid arthritis, basilar invagination is superior migration of the odontoid so the tip of the odontoid is above the foramen magnum; in this case you do occiput to C2 fusion, plus or minus odontoid resection. The Subaxial Subluxation: Occur in about 20% of the patients. Indication of surgery is neurological compromise. The space available for the cord is less than 14 mm then do posterior fusion surgery; surgery is usually not successful in severe types of neurological impairment. When do you do surgery in rheumatoid arthritis? You do it if there is: • Severe pain • Neurological deficit • X-ray showing that the P.A.D.I. is less than 14mm • Superior odontoid migration • Subaxial subluxation and the sagittal canal diameter is less than 14 mm. If the posterior atlanto- dental interval (P.A.D.I.) is more than 14 mm, the patient will demonstrate significant motor recovery after surgery. This video focuses only on rheumatoid arthritis as it affects the hands and fingers. Rheumatoid arthritis is different from osteoarthritis, which results from wear-and-tear on bones and tissues of a joint. Rheumatoid arthritis affects the lining of your joints, causing a painful swelling that can eventually result in bone erosion and joint deformity. Symptoms present themselves in a variety of ways, including the following. Skin coloration and sensitivity The skin around the wrists and knuckles may start to look red, feel warm, and become sensitive to the touch. Swelling Swelling occurs as RA causes an accumulation of fluid in the hand and finger joints. Smaller joints are usually affected first, especially joints that connect your fingers to your hands. Pain and stiffness As RA progresses, joint discomfort from rheumatoid arthritis becomes more noticeable. It's typically worst in the morning or after periods of inactivity. Simple tasks involving the hands and wrists become more challenging. Symmetrical inflammation Rheumatoid arthritis often affects both sides of the body equally, which doesn't normally happen with osteoarthritis. Carpal tunnel syndrome Individuals with rheumatoid arthritis may develop carpal tunnel syndrome, which causes tingling and numbness in their hands and fingers. The carpal tunnel is a narrow opening between bones and ligaments within the wrist. The median nerve, which passes through this opening, can become compressed, triggering numbness and/or tingling in the thumb and several of the fingers fingers. Fatigue and flu-like symptoms As mentioned, rheumatoid arthritis can cause symptoms elsewhere in the body. These symptoms are similar to having the flu, including a lack of energy, nausea, fever and weight loss. Nodules near the joints Nodules are hard, rounded bumps that grow around joints as tissue becomes inflamed. Besides appearing on the wrists and fingers, nodules can also develop on forearms and elbows. Malalignment of finger joints As time passes, swelling and inflammation caused by RA may may result in joint deformity. However, this does not happen in all cases. Unfortunately, no cure for rheumatoid arthritis is currently available. But there are treatments that can reduce pain and symptoms, especially if they begin in the early stages of the disease. Sjögren's syndrome A chronic systemic inflammatory autoimmune disease that can cause death if : pulmonary infection, a lymphoma, or renal failure. T & B lymphocytes of the immune system invades the exocrine glands in your body. Eventually this invasion leads to ductal epithelial cell hyperplasia, which cause a duct obstruction. Later it leads to atrophy, fibrosis and hyalinization of acini. Still later there is atrophy and replacement with fat of parenchyma. The B lymphocytes can cause lymphoid follicles with germinal centers in the salivary glands.40 times more likely to develop B cell non-Hodgkin lymphomas. Females are affected more than males Symptoms: Alopecia (hairloss), Dry: skin, eyes, nose, mouth, respiratory tract, vagina. The dry skin can be treated with lubricants. The dry vagina can cause dyspareunia, which is a painful sexual intercourse. This can also be helped with lubricants. The dry eyes, also called keratoconjunctivits sicca for more than 3 month is an important sign for Sjögrens syndrome. The patient is usually complaining about bad vision, photosensitivity and irritation of the cornea. Tests: Schirmers test, Slit-lamp (tear film breakup time) The treatment for dry eyes: Avoiding drugs that cause dryness for example anticholinergics, diuretics, antihistamines, antidepressants, use artificial tears, or lubricant drops like, methylcellulose or hypromellose. Stimulate lubrication of the eyes locally with cAMP or cyclosporine 2% olive solution, or we can stimulate systemically with pilocarpine or cevimeline. tiny plugs placed in the tear drainage ducts, corneal transplantation. The dry mouth, or also called xerostomia of more than 3 months is also an important sign of Sjögrens syndrome. This patient is saying that she have a daily dry mouth with difficulty chewing, swallowing and even the taste of food is not as good as before. Treatment for dryness of mouth is to drink water daily to help swallowing for example. We can stimulate the salivary glands locally by sugarfree gum, lozenges, or mouthwash containing carboxymethylcellulose. We can also stimulate salivary glands systemically with drugs, like, Pilocarpine or cevimeline. We can avoid drugs that decrease salivary gland secretion, like anticholinergics, antidepressants or antihistamines. This patient also have tooth decay, which can be prevented by regular dental visits with topical fluoride application, or simply by keeping a good oral hygiene after each meal. She is complaining of secondary Candida infection. In this case we give topical nystatin, lozenges, or clotrimazole. Stones can appear in the salivary ducts, which have to be removed to preserve the salivary tissue. We can measure the saliva production, which will be low in Sjögrens syndrome, meaning less than 1,5 ml in 15 min. We can also use techniques like, salivary scintiscanning or sialography. Except dryness, the patient may present with enlarged parotid glands There are also extraglandular symptoms. So the most common is Arthralgias and arthritis. We treat it with hydroxychloroquine or methotrexate and prednisolone. Raynauds phenomenon can be seen, and its treated with nifedipine and gloves which protect from cold. Lymphadenopathy in the cervical or axillary region can be seen. Vasculitis can present itself as rashes on the skin called purpuras, which can be treated with glucocorticoids or immunosuppressive agents like Cyclophosphamide. Kidney diseases like interstitial nephritis, kidney stones, impaired concentrating ability or renal tubular acidosis. Bicarbonate replacement can be given for renal tubular acidosis. Chronic hepatobiliary diseases Risks of non-Hodgkin lymphomas can be 40 times higher in Sjögrens syndrome patients. This can be treated with antiCD20 therapy together with a CHOP regimen, which is a chemotherapy regimen consisting of Cyclophosphamide, Hydroxydaunorubicin, Oncovin (Vincristine), Prednisolone. Rarely splenomegaly or myositis may be seen. If we make blood tests, we can see leukopenia, anemia, an increased Erythrocyte sedimentation rate, and some autoantibodies. The autoantibodies are Rheumatoid factors and antinuclear antibodies called SS-A (Ro) and SS-B (La). Except blood tests, we can confirm that this is Sjögrens syndrome by taking a biopsy from the lip, more specifically from the minor salivary glands in the buccal mucosa. This will show in the microscope that lymphocytes have attacked the salivary glands There will be many large areas of lymphocytes with atrophy of the tissue. Genetic tests can show an association of Sjögrens syndrome with HLA DR3. Primary Secondary Rheumatoid arthritis, Systemic lupus erythematosus, Systemic sclerosis, mixed connective tissue disease, vasculitis, Hashimoto's thyroiditis, Polymyositis, primary biliary cirrhosis and Chronic autoimmune hepatitis.

Find out why… next RA Puts Kidneys at Risk Marianne Wait writing for Arthritis Today reported people with rheumatoid arthritis (RA) are almost 5 percent more likely than other people to develop reduced kidney function, a sign of chronic kidney disease, according to a study published recently in the American Journal of Kidney Diseases. Although that figure may not sound high, senior study author Eric Matteson, MD, chair of the rheumatology department at Mayo Clinic in Rochester, Minn., says it’s significant. “It’s a big deal when you think there’s 1.5 million patients with RA, and how frequent kidney disease is becoming in the population.” This study looked at the health records of 1,623 people – 813 people who were diagnosed with rheumatoid arthritis between 1980 and 2007 and 813 people without RA who were similar in terms of age, gender, weight and other factors. Among those who had normal kidney function when they entered the study, 25 percent of those with RA and 20 percent of those without RA developed mildly or moderately reduced kidney function over 20 years. There was no difference in the rate of severe kidney dysfunction. RA patients who had high levels of inflammation in the first year after RA diagnosis (as measured by the blood test known as erythrocyte sedimentation rate, or “sed rate”) were more likely than others to develop reduced kidney function. “The general conclusion from that is that inflammation is a risk factor for renal dysfunction,” says Dr. Matteson.
