Dermatologist tips on skin cancer and Melanoma Are you at risk? In this video, I get pretty nerdy and go into some serious detail about Melanoma, the different kinds of skin cancers and if you are actually at risk. Think that’s Melanoma on your skin or a harmless bump? Watch the video, and you'll have a better idea. The good news is it’s 99% curable over a five-year period if caught early enough. “Early enough” means the Melanoma growth is still mostly on the epidermis. If it has spread deep into the dermis of your skin, it is considered “late.” That’s why it’s SO IMPORTANT to know what to look for. Who gets Melanoma? For men under 50, it’s the #1 cancer. Women under 50 still get more Melanoma than men, but it’s the #3 cancer (this is due to the number of breast and thyroid cancers). It’s common for both genders under 50. The most vulnerable group is Caucasian men over 55 years of age. And the fairer your skin, the higher your risk. That doesn’t mean darker complexions are in the clear. In fact, the darker your skin tone, the more likely Melanoma will be caught later and be deeper into your dermis. Melanoma can occur in children too. It’s rare, but it can be very aggressive. What puts me at higher risk of Melanoma? Melanoma is directly related to increased sun exposure, and your chances are doubled if you’ve had five or more blistering sunburns. Blistering means your skin was very red and it hurt to touch it. Tanning beds are another contributor. Some statistics: Use of a tanning bed before age 35 increases your chances by 75%, and using a bed 10 times over your life increases your chances by 30%. Chances are higher you will develop Melanoma if you have a lot of moles. If you have fair-colored skin and light colored eyes, you’re at higher risk. Especially, if you are a redhead or have redheads in your gene-line. A history of any skin cancers can also put you at a higher risk. Lastly, if you have a family history of Melanoma, then you are at a higher risk of getting it, too. What are the warning signs of Melanoma? A - Asymmetry - Is the mole a perfect circle or an oval or does it take a funny shape? B - Borders - Are the borders of the mole crisp and clear? Hopefully, so. C - Color - What is the color in the mole and around it? Ideally, it doesn’t have a halo. D - Diameter - Is it larger than a pencil eraser? Then you should have it looked at! E - Evolution - Does it change over time? A benign mole won’t change quickly. Advocate for Yourself If you are concerned you may have Melanoma, and you see your dermatologist who says, “Well, let’s just see what happens over the next month,” I’d encourage you to persist. If you have a strong or funny feeling that it’s Melanoma, then you should consider having it tested. Better safe than sorry! In order to reduce your chances of Melanoma, it’s crucial that you understand proper sun protection. 86% of Melanoma is the result of UV-radiation exposure, either sun or tanning-bed exposure. Melanoma is one of the few preventable cancers, and with good sun protection, you shouldn’t have to worry about ever having it. Hopefully, you found this information helpful. Mohs Micrographic Surgery Mohs surgery, named for Fredrick Mohs, was first conceived sixty years ago using chemical fixatives to allow tissue from the skin to be cut and examined for malignancy, but has evolved rapidly in recent years due to the ability to freeze tissue to allow a much faster turnaround. This breakthrough is what allows tumors that have recurred, are located close to vital structures, or which exhibit particularly aggressive features under the microscope, to be removed with 99% accuracy, and the defect to be surgically repaired, all in the same office visit. Utilizing Mohs surgical techniques, the surgeon performs precise excision of the skin cancer in stages, one tissue layer at a time. The tissue is then processed onto microscope slides by a Mohs histotechnician. These slides are carefully examined under the microscope by the Mohs surgeon so that any microscopic roots of the cancer can be precisely identified and mapped. Unlike conventional techniques which only sample a small portion of the edges, around 1%, Dr. Ewanowski examines 100% of the edge and bottom of the tissue. When cancer cells are seen, an additional tissue layer is removed only in areas where the cancer is still present, saving as much normal, healthy skin as possible. This tissue sparing technique results in a smaller post-surgical defect, and once the cancer has been removed entirely, Dr. Ewanowski will explain options for repair of the wound. Florida has the unenviable distinction of being the skin cancer capital of the United States. While the Sunshine State offers good weather and a desirable lifestyle, excessive sun exposure often leads to skin cancer. The good news is that there are a variety of treatments for skin cancer and pre-cancerous lesions. Your provider will determine which modality is best. Surgical removal of the cancerous tissue is often the best treatment. Mohs Micrographic Surgery is a more sophisticated surgical technique with the highest cure rate of all skin cancer treatment methods, about 99% for the most common types of skin cancer. Mohs Micrographic surgeons are Dermatologists who are specially trained in the technique, acting as both surgeon and pathologist. Mohs (pronounced mose, rhymes with nose) is named after Dr. Frederic Mohs, who pioneered the technique in the 1940s. Mohs Micrographic Surgery offers the highest cure rate because the technique ensures that all of the cancerous tissue is removed, while sparing as much healthy skin as possible. Skin cancers can be deceptively small on the surface of the skin, much like an iceberg. To remove it completely requires microscopic examination at a cellular level. The Mohs surgeon maps out the cancerous area, excises small portions of tissue and immediately examines each one under a microscope. This process is repeated until no cancerous tissue is left. Typically, the surgeon then reconstructs the surgical wound by using a variety of repair techniques. The benefits are two-fold; high cure rate and better cosmetic results for the patient. Mohs Surgery is conveniently performed under local anesthesia in an office setting. Both Dr. Andrew T. Jaffe and Dr. Andrew Kontos are Board Certified and Fellowship-Trained by the American College of Mohs Surgeons. Dr. W.G. Eshbaugh is a Board Certified Plastic Surgeon who has extensive experience in Mohs closures, which means that he performs closures and reconstructions of surgical wounds after the Mohs procedure is complete.

Michelle: Their back, their rear end, their feet, behind their ears. These are places that are overlooked, even by interns on physical exams. Sheila: It was definitely cosmetic, it might have been an acid peel or something. It was going to make me look beautiful. Max: Sheila Chervin came in for a cosmetic procedure, Dr. Green said, "Not before I do a full body skin exam." Sheila: I was surprised, I was surprised. I would say I was surprised. Speaker 4: Because you were coming in for something cosmetic. Sheila: Yeah. I wanted to be beautiful, what are you looking at my body for? Max: Because Sheila had a skin cancer, fortunately an early stage curable squamous cell cancer. The point here is that cancer could have grown for years and maybe even spread before it was found. A recent study found that only a little over half of family docs and GP's perform full skin exams. They or the patients may be in a rush, or they're embarrassed to get completely undressed, but it pays off. Michelle: I would say 80 percent of patients who come in for something cosmetic, not related to their skin cancer, not related to a skin exam, 80 percent of the time I find a skin cancer or pre-cancer at the very least. Max: Here's something else to remember, skin cancer can be tricky to identify. If you are going to go to the trouble of a full skin exam, you're better off going to a dermatologist. They're trained to recognize skin cancer, don't end up with advanced cancer because you were too busy or too embarrassed, Dr. Max Gomez, CBS 2 news. Dr. Butterwick of Cosmetic Laser Dermatology explains that the intensity of these UVA rays does not go down during the winter. She highlights the need for sunscreen during this time of year. Visit us online at http://bit.ly/2pdZguTskincancer Video transcript: Hi, I'm Dr. Laurie Jacobson and May is Skin Cancer Awareness Month. Skin cancer is the most common type of cancer diagnosed in the United States and there are over three million new cases diagnosed each year. It's very important to increase awareness about this common condition and it's very easy to treat and to prevent and that's what I'd like to go over today. So you definitely want to see a board certified dermatologist for your annual skin screening and why this is so crucial is you can prevent and treat this disease quite easily if you catch it early. Adults starting at age 18 are advised to come in annually for a skin check, we'll give you a screening and then we can give you guidance about how frequently you should be seen after that. I think some of the myths that exist out there is that you have to be old to get skin cancer and you have to be caucasian. I've diagnosed skin cancer in someone as young as 20 and that hasn't been just on one occasion, unfortunately. Another myth is that only Caucasians get skin cancer. And I've diagnosed skin cancer in African Americans patients and Asian patients as well as Hispanic patients. It exists in every race, every gender, and every age. So it's important to get your skin screened regardless of those factors. I feel terribly for Hugh Jackman that he's developed these skin cancers but I'm proud of him for coming out and telling the world about his experience and Hugh if you need a dermatologist, I'm here for you.

Sun exposure and tanning salons are the most common causes of skin cancer today. There are other factors, such as environmental pollution that affects skin cells, atmospheric radiation. Early detection of skin cancer is the key. Have someone check your skin often and look for irregularities and changes. If there are any changes, be sure to visit your dermatologist immediately. Also, Dr. Goldman recommends using sunscreen with at least SPF 30. Even on a cloudy day you can damage the skin, which is why sunscreen must be worn every single day in the summer regardless of the weather. Water resistant or waterproof sunscreen does not actually stay on all day long. This is why it is so important to reapply every time you get in the water and then climb back out. Another thing many people do not realize is that more sun is actually absorbed into the skin when you are near water or in the snow in winter. The reflection off the snow or water hits the skin from top and bottom for a double dose of danger. Knowledge about sunlight and cancer is just now becoming mainstream. Every year people are diagnosed with skin cancer because of the unsafe practices years in the past, such as lying in the sun for hours to obtain a tan. Doctors are now giving free skin cancer screenings, so there is no excuse for anyone to go undiagnosed today. Once diagnosed, photodynamic therapy can be used to treat skin cancer. This completely eliminates the need to cut the cancerous cells out of the skin. This laser treatment can eliminate early stages of skin cancer while preventing it from developing again.
