Short video of my friend and constant companion for 3-4 days at MD Anderson Cancer Center the internal radiation machine. Treated me for cervical cancer after 5-6 weeks external radiation therapy. Posted to give others battling cervical cancer an idea of what the machine does and sounds like running. Treatments were every 45 minutes for about 10 minute intervals. Side effects lots of gas (do NOT drink carbonated beverages in the hospital), vaginal pain (the sew in the wires), and terrible diarrhea.

Is it still the same chemotherapy used in peritoneal chemotherapy in this case? We have learned so much about endometrial cancer. We used to think it as one disease, but we have learned that even within a stage, and the cancer is confined only to the uterus and has not spread to the lymph nodes, there are differences in prognosis that depend on how the cancer looks underneath the microscope, which is called histology. We are learning to understand in different molecular features that endometrial cancer. We are even learning to type endometrial cancers between those that we feel are more hormonally driven, Type 1 cancers, versus those that are not hormonally driven. They express different molecular markers and look a little different under the microscope. With the help of a good expert pathologist, we try to decide what is the risk for this endometrial cancer.

Endometrial Hyperplasia there is proliferation of the endometrium which becomes very thick. Primarily the hormonal trigger for endometrial proliferation is estrogen. The hormone that blocks the effects of estrogen on endometrium is progesterone. ETIOLOGY Obesity can cause this because there is increased aromatase which converts more testoserone to estrogen. Polycystic Ovarian Syndrome (PCOS) which has a high estrogen and testosterone and there is no ovulation and therefore there is no corpus luteum and therefore no progesterone. Tumors that release estrogen releasing. Hormone replacement therapy in post menopausal women. Two types of hyperplasia, simple and complex. Normally there is small amounts of gland and lots of stromal tissue. In simple there are more glands, but not touching each other. In complex the glands are touching each other. In Atypia there are abnormal nucleus which is really large and it is a very bad sign. Complex atypia has a 30% chance of become endometrial cancer. Sign and symptoms is abnormal heavy bleeding due to lack of ovulation. Diagnosis is done initially by doing a transvaginal ultrasound looking for endometrial thickness. If less than 5mm than 95% can rule out endometrial hyperplasia. Biopsy is done with a pipelle, however it can miss areas of cancer because small sample. Dilation and curretage and hysteroscopy. Treatment depends on the pathology report. If there is no atypia you can give mirena and MPA which causes enodmetrium to go back to normal. Atypia requires hysterectomy. Type 1 endometrial cancer is endometroid adenocarcinoma. Endometrial Carcinoma Most common gyne cancer in women, especially in post menopausal. Type 1: Endometroid Adenocarinoma, due to estrogen secondary to endometrial hyperplasia. Type 2 is clear cell, serous carcinoma and there is no precursor, no precursor, more agressive and occurs much later (70s and 80s). Risk Factors related to unopposed estrogen. Risk factors are Cancers (Breast, Ovarian, Colon Cancer). Obesity, Late Menopause, Diabestes Mellitus, HTN, Nulliparity, Unopposed Estrogen due anovulation, Hormone replacement and tamoxifen. However, COC is protective. Signs and Symptoms in type 1 is postmenopausal bleeding. In type 2 there is bloating, bowel dysfunction, pelvic pressure. Staging is done by direct extension myometrium, serosa, bladder and large bowel. Or down cervix and vagina and finally fallopian tube. Lympahtic spread can go to pelvic para-aortic lymph nodes. Fallopian tube and into peritoneum. Hematogenous spread is liver and lungs. Stage 1 is when only in uterus. Stage 2 goes into cervix Stage 3 is when it goes into adjacent strucutres. Stage 4 is beyond pelvic cavity. Investigations are Transvaginal Ultrasound, Biopsy with pipelle, Dilation and curretage, and hysteroscopy. Treatment depends on staging, surgery of hysterectomy with bilateral salpingo-oopherectomy (BSO), pelvic wash and lymph nodes. Omentectomy and this can be done laparoscopy. If there is metastasis then also do chemotherapy and if it recurs than perform radiation therapy

But her prayers for a compassionate doctor were answered when the family called the Physician Referral Service at Memorial Sloan-Kettering Cancer Center and found Carol Brown, a gynecologic oncologist and surgeon. Edwards underwent a course of treatment that included surgery, chemotherapy, and radiation quite beautifully, Brown says admiringly. {partial transcript} Back in June, I realized that something was happening with my body that wasn’t normal. I went to a doctor and she said ‘you had Endometrial cancer.’ I said ‘What is that?’ And she said cancer of the lining of the uterus. That punch hits you in your stomach that you have cancer. Sister Edwards has been such a part of this church. I shutter to think of what this church would be like without her presence and her contributions. I told my two daughters together. You know being a mother you hate to tell your child something that you know is going to hurt them. Went upstairs to my room and just laid on the floor for a while to get myself together. You start thinking about, oh my god, what happens if she passes away. And I just kind of felt like what the best place she could be at to be treated. I wanted a compassionate doctor. Terry was praying for the same thing. We wanted a doctor that will sit and listen to her cause she has a lot to say. And my sister Peggy called a physician referral service and that’s how she was able to get me into Sloane-Kettering. The day of my consultation about ten of us went. And they took me in there to get my pressure taken and stuff. And all of a sudden I heard all this laughing and clapping out there, they’re making too much noise, this is a hospital. And then Doctor Brown walked in and said “Hi, I’m Doctor Brown.”

Located inside the Regional Cancer Center, it's called Stereotactic Body Radiation. First used in brain cancer, SBRT uses sophisticated guidance that can track subtle tumor movement and deliver high doses of radiation-in a fraction of the time. "We're able to cut down the number of visits and at the same time deliver a much more potent and much more effective dose of radiation than we could the conventional way. Conventional course of treatment may be 20/25, 30/35, 40/45 visits. With stereotactic treatment we can compress all that down to a total number of one, two, three, four, five treatments," says Dr. Mantz. "There is a group of patients who are long term survivors of advanced gynecologic cancer who will continue to enjoy a good quality of life for many years," says Dr. Mantz. This research investigates the use of vaginal fold brachiotherapy as an alternative to radiotherapy in patients with a high-intermediate risk. Brachiotherapy was shown to be as effective as external beam radiotherapy, had a lower toxicity profile and offered the patients a higher quality of life. Dr Creutzberg goes on to discuss the lack of effective biomarkers for this disease and discusses her plans for future research.

A First from SWFL: Stereotactic Body Radiation Therapy for Gynecological Cancers

Five years ago, cancer specialists in Southwest Florida began a study, taking a cutting edge radiation delivery system and using it on their patients with advanced gynecologic cancers who were otherwise out of options. "The cancer stopped responding to chemotherapy or the disease was just too advanced otherwise to treat with a meaningful surgery," says Dr. Constantine Mantz, a radiation oncologist on the medical staff of Lee Memorial Health System. This treatment was successful 80% of the time. Years of monitoring found it prevented recurrence or cured the women altogether. These results are putting Southwest Florida on the map. "Where we're so excited is that this is the first report of it ever being used in women with gynecologic cancer. Ovarian, uterine, fallopian tube, cervix or vagina," says Dr. Jimmy Orr, a gynecological oncologist with Lee Memorial Health System's medical staff. The treatment is dependent on a machine that focuses a narrow field of radiation allowing doctors to target the cancer directly without affecting the surrounding tissue and organs. "This treatment technology is something called stereotactic body radiation therapy. We can treat tumors in the body very precisely and accurately to very high doses of radiation that we cant achieve with any other form of therapy. Being able to deliver a much greater dosage of radiation we can substantially cut down the number of visits," says Dr. Mantz. Stereotactic body radiation was previously used for stable tumors including those in the prostate and brain, but was untried for female cancers. "I think this really brings us into a whole new era of treatment by allowing us to use very precise technology and techniques. This relates to reproductive tract cancer and it's the first report ever as it relates to using this technique in those women," says Dr. Orr. The next step is offering it to women with early stage disease. The doctors believe this may one day be the go-to procedure for gynecological cancers.
