Dec 9th 2010.... I got the news you have malig tumor and a vary rare tumor. I have Adenoid cysytic carcinoma of the small salivary glands. It had taken over most of my tongue, the floor of mouth skin and my entire jaw bone. I had surgery to remove the bulk of tumor including 60% of my tongue ans we chose to leave the entire jaw bone in Gods hands as It would severly disform me and I leave me without a face and on a trach for life and feeding tube for life and I did not want that for my sons nor I so I have Faith and trust God for a miracle for me and all other cancer patients out there...God Bless us all

A cyst is a lining that is an epithelial lining inside the bone which has got a fluid or a semi fluid inside the lining. It is either attached to the bone causing bone loss around the area. After the surgery, there is recurrence of cyst. There are lot of reason for that. We have to understand if it is really a cyst or if it is something else, if it is a benign tumour or a malignant tumour. We have to understand what kind of surgery has been done, if it is partially excised or it is completely removed from the area. So what happens in that area is little bit of healthy bone is also removed. These are all in microscopic level. Sometimes, there is some residual epithelial tissue or infected tissue which is left over. However assessment, regular follow up and understanding of what has happened inside the mouth will give the patient as well as the clinician a better idea especially in a surgical perspective. The surgical perspective has got many other parameters like the maintenance by the patient, the medication taken by the patient, the duration of the cyst, and other things are very important to assess the situation like this.

"It's really an asymptomatic infection. Most women who have HPV have absolutely no symptoms. Whereas other STDs, like herpes, certainly has symptoms if there's an outbreak," says Dr. Kevin Fleishman, gynecologist on Lee Memorial Health System's medical staff. Many of them clear without ever being detected. Some are more likely to turn bad. The human papilloma virus has more than 100 strains. "Some are low risk, some are high risk. The high risk types are known to cause almost all cervical cancer," says Dr. Fleishman. This new study revealed a rising number of people with oral HPV. It's estimated 16-million Americans between 14 and 69 are infected. This raises their risk for cancers of the mouth. "Many, many of these cancers are caused by HPV and these are largely cancers of the tonsils or the base of the tongue. Way back in the back of the throat where you can't see with a tongue blade," says Dr. Phillip Andrews, otolaryngologist on Lee Memorial Health System's medical staff. The FDA approved an HPV vaccine for boys and girls. First aimed at preventing cervical cancer in girls, doctors now think it could also prevent 10,000 new cases of oral cancer each year. But fewer than half the people who could get the vaccine take even the first of 3 doses. "The best time to do a vaccine is before somebody's been exposed to what you're vaccinating against. And therefore, when it comes to sexually transmitted diseases, the best time to vaccinate is before there's been any sexual activity," says Dr. Fleishman. The findings concluded the rise in HPV cancer rates combined with low vaccination rates require an intervention of public education. Endoscopy Cancer of The Tongue Tobacco and alcohol use: Tobacco use (cigarettes, pipes, cigars, and smokeless tobacco) is responsible for most cases of oral cancer. Alcohol, particularly beer and hard liquor, are associated with an increased risk of developing oral cancer. Tongue cancers are malignant tumors, usually "squamous cell cancers", that can originate on any part of the tongue surface. These cancers are divided into those that arise in the oral tongue; the part of the tongue that resides within the oral cavity, and the 'base of the tongue', the part of the tongue that cannot be easily be seen during a typical oral exam. Tongue cancer is malignant tumor that develops when an unrestrained growth of cancer cells occurs in the tongue. It is a common form of oral cancer, and statistics state that the frequency of incidence in greater in males than in females. Tongue cancer is treatable if diagnosed during the early stages of the disease. Undetected and advanced cases spread rapidly to the mouth, throat, jaws and neck. Causes and risk factors for Tongue Cancer A host of risk factors are associated with the development of tongue cancer: • Smoking • Chewing tobacco • Excessive intake of alcohol • Ill fitting dentures • Acid reflux is also an important causative factor • Poor diet with a deficiency of vital nutrients • A weak immune system • Exposure to certain chemicals • HPV infection Tongue cancer is often grouped with other mouth cancers, such as cancer of the lips, hard palate, cheek lining, floor of the mouth (portion of the mouth underneath the front of the tongue) or gums. These cancers are collectively known as oral cavity cancer. Risk Factors The exact cause of tongue cancer is unknown. However, the following lifestyle factors may be related: Smoking cigarettes, cigars or a pipe Poor oral and dental hygiene Irritation of the mucous membranes in the mouth due to smoking and drinking Use of chewing tobacco, snuff or other tobacco products Heavy alcohol consumption Recently, human papilloma viruses (HPV), the same types that are associated with cervical cancer, have been linked to the development of tongue and tonsil squamous cell cancers. The link is very strong in patients with little or no history of smoking or alcohol abuse. The tumor biopsy can be sent for analysis to determine the presence of HPV; this is important because the presence of HPV may be associated with a much better prognosis. Symptoms If you have any of these symptoms do not assume it is due to tongue cancer. These symptoms may be caused by other conditions. Tell your doctor if you have any of these: Persistent tongue pain or ulcers Difficulty swallowing for a period of time Numbness or difficulty moving the tongue Change in speech (due to inability to move the tongue over the teeth when speaking) Pain when chewing and speaking Bleeding from the mouth or tongue Diagnosis Tongue cancer may be detected by your dentist during a routine dental cleaning, or by your doctor during a routine physical exam. To confirm diagnosis, your doctor will ask about your symptoms and medical history, and perform a physical exam. This may include: Examination of your tongue for lumps or masses Use of a fiberoptic scope-a thin tube with a tiny camera to examine the base of the tongue A tongue biopsy-removal of a sample of tongue tissue to test for cancer cells CT scan-a type of X-ray that uses a computer to make pictures of the mouth Chest X-ray to determine if the cancer has spread to the lungs Other imaging such as MRI or PET scanning Treatment Treatment depends on the stage of the cancer, as well as the size and location of the tumor. Surgery This is surgical removal of the cancerous tumor and nearby tissue, and possibly nearby lymph nodes. This is often the preferred treatment when the tumor is on the visible side of the tongue, when it is quite small (less than 2 cm), and when it is lateralized to one side and does not involve the base of the tongue. Radiation Therapy (or Radiotherapy) This is the use of radiation to kill cancer cells and shrink tumors. This method is used when the cancer is at the back of the tongue. Chemotherapy Chemotherapy is sometimes used with radiation to destroy the cancerous growth, especially if surgery is not planned. Rehabilitation and Follow-Up After treatment, your doctor may recommend: Therapy to improve tongue movement, chewing, and swallowing Speech therapy, if use of the tongue is affected Close monitoring of your mouth, throat, esophagus and lungs to see if the cancer has come back or spread Prevention To help reduce your chance of getting tongue cancer, take the following steps: Don't smoke or use tobacco products. Hey everyone! So a lot has happened since I've last made a video. On July 16, I had a 7 hour bone graft surgery. A piece of bone from the back of my hip was taken and put into my upper and lower jaw. You may be thinking, wasn't that painful? Well, yes it was. But that's okay because I'm currently on the road to recovery and I'm getting better! Some of these pictures may be a bit too much to handle for some of you, heads up! This surgery has been the biggest so far of all of my surgeries and there are still more to come. But I'm almost done with it all which is super exciting! I promise I will be back soon. Before my surgery I filmed a video and I edited it. I just forgot to do the voiceovers so I will try to find a way to work around it and post it. Upon examination of the throat, the uvula is swollen, pale, and somewhat translucent (uvular hydrops). If greatly enlarged, the uvula might rest on the tongue and move in and out with respiration. There might be an associated rash or a history of exposure to phsical stimuli, allergens, or a recurrent seasonal indicence. What to do: Because of the known association of uvular with hypopharyngeal edema, watch for signs of airway compromise. If a patient complains of resiratory difficulty or breathes with stridor, commence treatment with intravenous lines and intubation and cricothyrotomy equipment at the bedside, and a crosstable lateral soft tissue neck x ray to rule out epiglottic swelling. If there is no acute respiraory difficulty, ask about precipitating events. Consider foods, drugs, physical agents, inhalants, insect bites and hereditary angioedema. When fever, sore throat and pharyngeal injection are present, culture the throat with a rapid strep screen and give an antibiotic that covers Haemophilus influenzae (e.g., Biaxin, Augmentin, Bactrim). It is reasonable to obtain a complete blood count with a manual differential to demonstrate eosinophilia to support the possibility of an allergic reaction or a high leukocyte count with increased granulocytes and bands to support a bacterial infection. Initially the patient should receive parenteral H1 and H2 antihistamines like hydroxazine 50-100mg im or diphenhydramine 25-50mg iv along with cimetidine 300mg iv or po or ranitidine 50mg iv or 150mg po. More severe cases should receive repeated doses of epinephrine 0.3ml of 1:1000 sq every 20 minutes x3. Nebulized isomeric or racemic epinephrine or albuterol are also effective. Parenteral corticosteroids like SoluMedrol 125mg iv are also typically used, although efficacy remains umproven. If there is a history of recurrent episodes of edema and there is a family history of the same, consider ordering a C4 complement level or C'1 esterase inhibitor levels as a screening test for hereditary angioedema. In this condition, the edema often involves the uvula and soft palate together. Uvular decompression may be useful in patients that are resistant to medical therapy or whose symptoms progress rapidly. This procedure consists of grasping the uvula with forceps and either making several lacerations with a sterile needle or snippint the distal centimeter as a patial uvulectomy. All patient s should be observed for an adequate period of time to insure that there is either improvement or no further worsening of the swelling before being discharged home. Upon discharge, they should receive 4-5 days of H1 and H2 blockers and steroids if required.
