Pernicious anemia (types of anemia) nursing, pathophysiology, symptoms, and treatment for the NCLEX exam. Pernicious anemia is a form of vitamin b 12 anemia that is an autoimmune condition where the body does not produce intrinsic factor which plays a role in absorbing vitamin b 12.The body needs vitamin b 12 to make healthy red blood cells. A protein called intrinsic factor plays a role in the body’s ability to absorb vitamin b 12. Low vitamin B12 levels lead the body to produce unhealthy red blood cells. Pernicious anemia is a type of macrocytic anemia where the red blood cell will appear very large and oval-shaped (rather than normal sized and round-shaped). Signs and symptoms of pernicious anemia include feeling very fatigued, tingling or burning in the hand or feet (also called paresthesia), red, smooth tongue, depression, loss of appetite etc. Treatment for pernicious anemia is supplementation of vitamin B 12 through intramuscular injections of vitamin B 12. Nursing care of a patient with pernicious anemia include administering vitamin B 12 injections, educating the patient about the condition and medication regime, good oral hygiene due to tongue changes, and safety precautions due to paresthesia in the hands and feet. http://www.stomponstep1.com/folate-b12-deficiency-megaloblastic-anemia-hypersegmented/ Vitamin B9 (Folate/Folic Acid) is very important for 1 carbon chemical reactions (AKA methylation) during the creation of DNA. These functions are particularly important in tissues that undergo frequent cellular division (like hematopoietic cells) and during periods of rapid cellular division (such as infancy and pregnancy). Folate is found naturally in leafy vegetables and Folium is the Latin word for leaf. Today many grains are also fortified with Folate. With so many items having Folate added a deficiency from decreased intake of Folate is not very common. Usually a deficiency is caused by an increased demand for folate through pregnancy or hemolytic anemias such as Sickle Cell Disease. Folate deficiencies in pregnant women are linked with the occurrence of Neural Tube Defects in the fetus. A deficiency can also be caused by drugs that inhibit the pathways that Folate is involved in such as Trimethoprim (antibiotic) and Methotrexate (Chemotherapy). A Folate deficiency primarily causes anemia (decreased red blood cells). Anemias can present with a wide range of symptoms including weakness, fatigue, pale skin and shortness of breath. Folate Deficiency causes a specific type of Anemia called Macrocytic/Megaloblstic Anemia where the size (Mean Corpuscular Volume) of the red blood cells increased. This is because as the creation of DNA is inhibited the cell cycle in the hematopoietic cells is stalled. Cellular division is stopped but the cell continues to grow resulting in a smaller number of cells which are larger than normal cells. Patients with this type of anemia also often present with Hypersegmented Neutrophils on a blood smear, which is a neutrophil with 5 or more "segments" in the nucleus. You are also going to see an elevation of Homocysteine levels in Folate Deficiency. This is because B12 needs to receive a methyl group from Folate so it can pass it on to Homocysteine to create Methionine. Less Folate means B12 doesn't have a methyl group to pass on and Homocysteine builds up. Unlike B12 deficiencies, Folate deficiency does not result in a buildup of Methylmalonic Acid or neurological symptoms. Vitamin B12 (Cobalamin) is a vitamin, like Folate, that is important for 1 carbon chemical reactions. B12 is found in many animal products and Vegans are at the highest risk of deficiency. B12 taken in through the diet is bound to proteins. Stomach acid and digestive enzymes must first separate B12 from the proteins. If there is a deficiency of stomach acid due to disease or medication (like proton pump inhibitors or antacids) it can cause decreased absorption and B12 deficiency. Once it is no longer protein bound, B12 binds Intrinsic Factor which is released by the Parietal Cells in the stomach. Intrinsic Factor then chaperones the B12 to the terminal ileum where it is absorbed. Pernicious Anemia is the autoimmune destruction of parietal cells which leads to less Intrinsic Factor production. Less intrinsic factor means less B12 absorption and B12 deficiency. Damage to the terminal ileum, such as in Crohn's Disease, can also inhibit B12 absorption. Folate and B12 work closely together handing off methyl groups to each other. It is sort of like a game of hot potato. Folate doesn't "want" the methyl group so it passes it on to B12. B12 doesn't "want" it either so it passes it off to methionine. In effect, B12 helps recycle methyltetrahydrofolate back into tetrohydrofolate which can be used to create DNA. This means that a deficiency of B12 can cause a deficiency of Folate as less Folate is being recycled into its "active" form. Therefore, B12 deficiency present very similarly to Folate deficiency. You get Macrocytic/Megaloblastic Anemia with Hypersegmented Neutrophils and increased Homocysteine levels. If you mistakenly diagnose Folate Deficiency when it is really B12 Deficiency the patient will get better with Folate supplementation, but permanent neurological damage will be done overtime. Odd chain fatty acids are broken down to eventually give Methylmalonyl CoA. B12 is a cofactor in the process that converts methylmalonyl CoA into Succinyl CoA which can then be used in the TCA cycle to generate energy. If there is not enough B12 this reaction is slowed and Methylmalonic Acid builds up. This Methylmalonic Acid build up is toxic to neurons and leads to demyelination in the posterior and lateral columns of the spinal cord. This is called Subacute Combined Degeneration and presents with peripheral numbness/tingling, spasticity, and loss of vibration and proprioception.

Vitamin B12 Deficiency Signs, Symptoms, And Risk Factors The symptoms of vitamin B12 deficiency are diverse, including both physical and mental symptoms. The physical symptoms include palpitations, incurable fatigue, loss of balance, numbness, tingling, pale skin, and sore and red tongue. The mental symptoms include memory loss, depression, or obsessive compulsive disorder. Vitamin B12 Is Essential For Your Nerves And Genes Vitamin B12 (cobalamin) is one of the 8 essential B vitamins. As it is water soluble, your body cannot store it; so, it needs to be constantly sourced through food. All B vitamins help produce energy in the body by converting carbohydrates from the food into fuel (glucose) and use fat and protein optimally Signs And Symptoms Of Vitamin B12 Deficiency As an adult, you need only 2.4 mcg vitamin B12 daily through your diet. For infants below 1 year, the daily requirement ranges from 0.4 to 0.5 mcg. Pregnant and lactating mothers require 2.6 and 2.8 mcg, respectively. The only source of vitamin B12 are foods, mostly from animal sources, and supplements. While vegetarians can get it from dairy products, they still risk not getting enough. However, vegans have a high risk of vitamin B12 deficiency if they do not eat B12-fortified foods and supplements. Deficiency occurs either when you don’t get enough of it through your diet or when your body cannot absorb it effectively. Poor absorption is caused by aging, low stomach acid, chronic digestive disorders like Crohn’s disease or celiac disease, stomach surgeries, and pernicious anemia. 1. Memory Loss, Depression, OCD, And Psychosis The first signs of vitamin B12 deficiency, which could take years to develop, have either to do with your nerves and mental health. The list includes depression, memory loss, psychosis, obsessive-compulsive disorder, and dementia. The reason behind this is that vitamin B12 helps build the protective myelin sheath around nerve cells. 2. Anemia, Fatigue, Shortness Of Breath There are certain blood-related signs that the deficiency could manifest in. But it is highly unlikely that you have hematological symptoms if you already have nervous and mental symptoms. The likelihood of it diminishes further with the severity of the neuropsychiatric symptoms. Anemia is a classic vitamin B12 deficiency symptom. Severe anemia could lead to breathing difficulty on exertion, fatigue, as well as symptoms related to congestive heart failure, such as ankle edema, breathing difficulty, frequent urination in the night, etc. 3. Nausea, Vomiting, Heartburn, And Bloating Some symptoms of the deficiency involve the digestive tract like nausea, vomiting, heartburn, abdominal bloating and gas, constipation or diarrhea, loss of appetite, and weight loss. An enlarged liver is another symptom. 4. Skin Lesions, Vitiligo, And Sore Tongue Any change in the cobalamin content in the body can show on the skin as vitiligo, dermatitis, hyperpigmentation, or acne. Skin lesions, especially the ones not responding to any other treatment, are most likely from cobalamin deficiency. Your hair and nail could change, too. Vitamin B12 Is Found Mostly In Animal Sources Since vitamin B12 cannot be produced in the body, it is essential to include food that supplies it in our diet. Plants don’t make it and the only living things that make vitamin B12 are certain bacteria present in the digestive tract of animals. However, each individual may experience symptoms differently. Symptoms may include: Weak, Numbness or tingling in hands and feet, Difficulty walking, Nausea, Decreased appetite, Weight loss, Irritability, Lack of energy or tiring easily (fatigue), Diarrhea, smooth and tender tongue, Increased heart rate (tachycardia), The symptoms of megaloblastic anemia may resemble other blood conditions or health problems. Other types of megaloblastic anemia may be associated with type 1 diabetes, thyroid disease, and a family history of the disease. I'm also going to talk about how to overcome Vitamin B12 deficiency and the health benefits of Vitamin B12. Surprisingly, about 40% of the population is deficient. The biggest reason most people have Vitamin B12 deficiency is because they don’t have a healthy digestive system to digest this vitamin properly, also known as leaky gut. If you can’t absorb nutrients and vitamins properly, then you won’t be able to absorb the vitamins you need for optimal health. People who struggle with leaky gut or digestive issues, vegans or vegetarians, and people who don’t’ have enough probiotics in their diet all tend to have a Vitamin B12 deficiency. Some of the benefits of Vitamin B12 include red blood cell formation, which is vital to heart health, cellular function, and supporting the brain and nerve system. If you want to overcome Vitamin B12 deficiency, then you must identify the cause and then follow these specific steps: 1.
