Reasons For A Medical Bill There are a several common reason you get a medical bill after you see the doctor. Here are just a just a few: Your plan has a copay, coinsurance and/or deductible. The insurance billed denied because the plan coverage termed. There is missing information that the insurance is requesting either from the patient/subscriber or provider. Your plan denied coverage for the medical treatment rendered. Or the insurance was never billed because there was no insurance given at the time you saw the doctor. ~Please comment below other reasons you can think of or have had to deal with of why you received a medical bill. Typically, the doctors office attempts to collect updated demographic/name, address, phone number, along with the most current insurance information before a patient is seen. At times, this doesn’t happen, but it should be collected at the time the patient is seen. One problem is that when the patient is asked at the time they are seen they do not pay attention in updating their current contact information. The patient kind of go through the motions because we are not well or anxious because doctor’s office environment is intimidating and we just want to get it over with and feel better or get good news that we are ok and healthy. So, the patient checks in takes a seat and when calls goes in and is examined by the doctor.

How to Get Your Body to Eat Its Own Fat

When you cut carbs, you force the body to use body fat as fuel. Starting this (ketosis), you want to increase dietary fat to help you go from one meal to the next without being too hungry. Then once you are fat-adapted, you can then reduce the amount of dietary fat you consume. For example, don't add extra fat (MCT oil, coconut oil, keto bombs, etc.) and only eat the fat that is normally in protein. This action forces your body to use up it's own body fat and not as much as the dietary fat. Also make sure your dietary protein intake in moderate and not too much or too little because if too little, your body will also use up it's own muscle protein as fuel; too much and it could be converted to sugar as fuel. Hey guys, in this video we are gonna show you how to get your body to burn its own fat. No. 1 you have to lower your carbs and sugar, like this (carbs) right here because in the presence of this your body will not burn this (fat), so we have to lower the carbs down to about between 20 to 50grams if you have a stubborn metabolism, bring it down below 20grams per day, not per meal. What happens is you are gonna put yourself into ketosis where your body is running on fat fuel, you have to adapt to fat burning. When you start out, you have to increase the dietary fat in your diet because it is gonna be hard to go into one meal to the next when you do intermittent fasting. I put a link down below for those of you who don’t know what I am talking about. So we want to combine healthy ketosis with intermittent fasting, lower the carbs and add more fat in the diet. Get comfortable with this to the point that you are not craving anything anymore, you are not hungry anymore because your body is finally starting to tap into your own your own fat. Why? Because you lowered your sugar and carbs. So initially, you want to increase your fat but as you adapt, you may plateau. In which case, at that point you may need to reduce your fat so your body consumes more of your own fat and not the dietary fat. If we want to get rid of this (fat) in the body we have to adjust the dietary fat. We start lowering the dietary fat a little bit, until your body starts utilizing your own fat. So we have to lower the dietary fat to force our body to use up our own fat for energy. That’s kind of the adjustment that you gonna have to go through to get your body to eats its own fat. Now make sure that you always going to have a moderate amount of protein in the diet between 3 to 6 ounces per meal. Why? Because if you cut out protein, your body start breaking down your own muscle protein and other proteins for fuel and we want to avoid that. We want to make sure that there’s some protein to use to replace that parts or else you are gonna lose your hair and the muscles are gonna get weak. If you have too much protein that is also gonna stop the fat burning too so have a moderate amount. So I hope that helped, the next things is to just get started. I am really excited to talk to you today so welcome from South Africa. What time is it over there right now? Professor Noakes: Yes, Passed 2 o’clock in the afternoon. Dr. Berg: Oh wow! And what day is it? Professor Noakes: I have to think it’s Thursday. Dr. Berg: Okay got it, great! So, I want to first talk about you at one point promoted the high carb diets for awhile right? Tell me a little bit about that and then, I think what change your mind? Professor Noakes: So I started running in the late 1960’s early 70’s and I started my physiology training in mid-1970 and that was one of the first years that the new theory arrives that muscle glycogen, carbohydrates in the muscle was the single effect to determining the athletic performance and that is how it was projected to all of us. So I am a young student studying physiology in medicine and I am running marathon and I discover this, this is the most exciting piece of information that I heard in the whole medical training. So I was started promoting this high carbohydrate diet in early 1970’s and I have been coursed adapted to that diet and I went to complete my medical training, I was surrounded by cardiologist. It was them telling us if you eat fat, you are gonna die. So I was gonna eat lots of carbohydrate and It was gonna make me run better well in fact, it made me run worse and I got fatter and progressively sicker. And then in 1981, my father was diagnosed with Type 2 Diabetes and he died 10 years later. And eventually I discovered that I also had type 2 diabetes and I realized that I got 10 years to solve this problem out unless I am gonna die like he did. I kicked off the series with the very popular, intermittent fasting or time-restricted eating. It's claim to fame is that helps burn belly fat/ lose your belly and it does so through fast weight loss. But is fast weight loss even healthy? When I consider purchasing an individual health insurance plan for myself or my family, do I have any financial obligations beyond the monthly premium and annual deductible? Answers: It depends on the plan, but some plans have the following cost-sharing elements that you should be aware of. Co-Payments: Some plans include a co-payment, which is typically a specific flat fee you pay for each medical service, such as $30 for an office visit. After the co-payment is made, the insurance company typically pays the remainder of the covered medical charges. Deductibles: Some plans include a deductible, which typically refers to the amount of money you must pay each year before your health insurance plan starts to pay for covered medical expenses. Coinsurance: Some plans include coinsurance. Coinsurance is a cost sharing requirement that makes you responsible for paying a certain percentage of any costs. The insurance company pays the remaining percentage of the covered medical expenses after your insurance deductible is met. Out-of-pocket limit: Some plans include an out-of-pocket limit. Typically, the out-of-pocket limit is the maximum amount you will pay out of your own pocket for covered medical expenses in a given year. The out-of-pocket limit typically includes deductibles and coinsurance. But, out-of-pocket limits don't typically apply to co-payments. Lifetime maximum: Most plans include a lifetime maximum. Typically the lifetime maximum is the amount your insurance plan will pay for covered medical expenses in the course of your lifetime. Exclusions & Limitations: Most health insurance carriers disclose exclusions & limitations of their plans. It is always a good idea to know what benefits are limited and which services are excluded on your plan. You will be obligated to pay for 100% of services that are excluded on your policy. Beginning September 23, 2010, the Patient Protection and Affordable Care Act (health care reform) begins to phase out annual dollar limits. Starting on September 23, 2012, annual limits on health insurance plans must be at least $2 million. By 2014 no new health insurance plan will be permitted to have an annual dollar limit on most covered benefits. Some health insurance plans purchased before March 23, 2010 have what is called "grandfathered status." Health Insurance Plans with Grandfathered status are exempt from several changes required by health care reform including this phase out of annual limits on health coverage. If you purchased your health insurance policy after March 23, 2010 and you're due for a routine preventive care screening like a mammogram or colonoscopy, you may be able to receive that preventive care screening without making a co-payment. You can talk to your insurer or your licensed eHealthInsurance agent if you need help determining whether or not you qualify for a screening without a co-payment. There are five important changes that occurred with individual and family health insurance policies on September 23, 2010. Those changes are: 1. Added protection from rate increases: Insurance companies will need to publically disclose any rate increases and provide justification before raising your monthly premiums. 2. Added protection from having insurance canceled: An insurance company cannot cancel your policy except in cases of intentional misrepresentations or fraud. 3. Coverage for preventive care: Certain recommended preventive services, immunizations, and screenings will be covered with no cost sharing requirement. 4. No lifetime maximums on health coverage: No lifetime limits on the dollar value of those health benefits deemed to be essential by the Department of Health and Human Services. 5. No pre-existing condition exclusions for children: If you have children under the age of 19 with pre-existing medical conditions, their application for health insurance cannot be declined due to a pre-existing medical condition. In some states a child may need to wait for the state's open-enrollment period before their application will be approved.

Panelists in this Forum event examined aspects of telehealth in the U.S., including doctor and patient buy-in and the impact of the Affordable Care Act, and in other high-income countries. They also took a look at the role of telehealth in low-income countries, where technologies such as mobile phones can serve as vital tools for healthcare workers to ensure and evaluate standards of care in the diagnosis and treatment of common illnesses. TV Anchor: A lot of health insurance plans make you designate a primary care physician. Finding the right doctor, well, it's can be a little difficult a little stressful. Joining us is Dr. Michael Brown. He's a family medicine physician with Meritas Health Park Plaza in Kansas City (near Parkville). Good to have you with us. You have some interesting information about why a primary care doctor is so important. Dr. Michael Brown: You know, Mark, a primary care doctor is one of the most important people on your healthcare team. We all know that healthcare is just extremely complex, and you need somebody to be your chief educator on that team. I think a lot of patients are actually surprised by how much a primary care doctor can handle before having to refer you to a specialist. My job is just to make that journey an easy experience for my patient. TV Anchor: So how does somebody go about picking the right primary care physician? Dr. Michael Brown: Well you know the first thing you have to do is work with your insurance and see who's available to you and covered by your insurance. Then I still think word-of-mouth is one of the best ways to find the right primary care physician. Now the great thing is in today's age is you can use social media to find like-minded people and see who they would recommend. I know my own wife is kind of one of my chief advertisers on Facebook when her friends are looking for physicians, and I'm proud to say that occasionally she still recommends me to them. TV Anchor: Really? Well, that's good. Good to have your wife on your side, absolutely. All right a lot of people these days with the internet they get on there they have a symptom or something they google and then they go “Oh my god, I'm gonna die. I've got this, I’ve got that. What do you think about people getting on the Internet to find out their medical conditions? Dr. Michael Brown: You know I kind of think my role’s evolved over the last several years to become kind of a second opinion for Dr. Google. You know, people do tend to come in with concerns that they found on the Internet. I actually think it's great when a patient comes into me having looked up information online. They're an engaged patient, and I just try to be that guy through that journey and convince them that no, it's not always cancer. No, it's not always your heart, but let's find out what we need to do with the condition that they have. TV Anchor: A lot of big pharma companies are also pushing their products on social media or in the advertising on TV you see. They say to ask your doctor about it. What do you think about that? Dr. Michael Brown: I think it starts the conversation, and I think it brings patients into my office. But again, that tells you why you need a good primary care physician because you need someone to help you navigate through and understand what is best for you. TV Anchor: If someone comes in and they don't like you, which not everyone likes everyone, how do you break up with your doctor? Dr. Michael Brown: I tell patients that there are as many physicians as there are personalities so, yes, there's going to be some times where maybe you don't see eye to eye with your physician. I think that's okay. I think that if you have a hard time or you're not understanding your physician, start that conversation. Let them know why. See if they can make some compromises with you or guide your care in the way that you want. If not, ask them for a referral. We have lots of friends and lots of colleagues. If one may be a better fit for you, I'd be happy to recommend you to one of those people. Knowing your personality as I do, I can send you to peers. TV Anchor: You like constructive feedback, right? Dr. Michael Brown: Absolutely. You know, in today's age, the healthcare journey is your journey. I'm just trying to help you get the care that you're looking for in the way that you want it because there are lots of decisions that have to be made along the way.
