Steroids are a group of hormones that regulate body functions – and they are all derived from cholesterol. Or fat. That’s right, fat. Estrogen, which is usually the focus of HRT, is actually a generic term for three different forms of hormone called Estriol, Estrodiol and Estrone - Estrodiol being the most potent. Estrogen is called the female sex hormone because it regulates the menstrual cycle and causes thing to grow during puberty – Dr Raj likes estrogen because it creates curves and it also stimulates cell growth in preparation for pregnancy. Estrogen is a growth hormone. It has received a lot of bad press because people have been taking too much estrogen in the form of synthetic hormone for a long period of time. And any hormone you take in excess is going to cause problems. And a growth hormone like estrogen if you take too much of it. You are going to get excessive cell growth which is another term for what we call cancer. So its understandable, estrogen is not good or bad, but if you take too much estrogen its going to cause problems. Progesterone is the pro gestational hormone. It’s the hormone that keeps the uterine lining intact if you get pregnant. And its depleted when you are under stress. So the typical reaction is high stress, high cortisol levels when you are under stress and the depletion of the progesterone. Its pretty amazing what progestronine helps with. It helps protect from many of the negative effects of extra estrogen. It helps women sleep. It gives them better sex drive, better energy levels, makes your skin look better. It’s a wonderful hormone. If we compare them side-by-side, you’ll see that they counterbalance each other in many ways. There’s an ideal ratio between these two hormones. Estrogen given in too large of a dose can cause depression, headaches, it increases the risk of blood clotting and stroke. It causes the loss of many nutrients such as zink and copper. It interferes with oxygen levels. Again that in a person that is getting excessive estrogen which has basically been the history of menopausal treatment. Dr Raj wants to leave you with an important point to free yourself from chronic fatigue and pain and depression and overweight. Ovarian hormones do not spontaneously malfunction by themselves. The female hormones do not all of a sudden malfunction by themselves. They are responding to stress as a stress hormone called cortisol. Every molecule of cortisol that you produce comes from progesterone. When you are under stress guess what happens? Your cortisol goes up and your progesterone drops. Dr Raj wants to test your cortisol and fix it. And he wants to test your female hormone and fix your whole hormonal system at the same time and make all your symptoms go away. What are the signs you are pregnant while on birth control? You hit that row of do nothing pills in the birth control pack and your period does not come. It could come late for other reasons like hormones out of whack. And it was concern that my hormones would be messed up that I did not use them. Then I cannot say that you got pregnant on the pill because you used antibiotics. Antibiotics can make you poop a lot, passing the pills through faster. If you were taking thyroid medication, it would metabolize them faster. However, if you get a stomach bug and throw them up, you could get pregnant. If I’m throwing up a lot, that’s a sign I am pregnant on birth control. If you are nauseous or throwing up in the morning but not in the afternoon or evening, then it is likely due to pregnancy. If it is all day, then you may need antibiotics or another medication. I have not been taking antibiotics. If you were on the pill, you’d be able to guess you were pregnant if you had a light period the next cycle and none the one after that. With a birth control implant, though, a woman would not have a period at all. Well, most of the time. You could develop breast tenderness in early pregnancy. I get that with my normal cycle. Peeing a lot is a sign of pregnancy. Or a urinary tract infection. If you used an injected contraceptive, your odds of pregnancy are really low barring medication that interferes with it. I do not use the injectables. What are the other signs I could be pregnant while on birth control? If you used an intrauterine, it causes heavier than normal periods. If you’re pregnant, the period stops. That’s usually normal regardless of the contraception method. You could be certain you did a pregnancy test and it came back positive. Don’t birth control methods interfere with that? If you used a condom, cervical cap or even an intrauterine, you’d have the normal hormonal cycles for ovulation and pregnancy. It is only the hormone based pills and shots that throw off your hormones. And it may be that my contraceptive method failed. If you were taking birth control pills, the estrogen or progesterone won’t throw off the pregnancy test. In fact, no medicine you take for anything would mess it up. So a pregnancy test is the best way to see if I’m pregnant. And it would be a sure sign to talk to your doctor about the best way to stop your birth control, since you clearly won’t need it for a while.

The TRUTH About the DANGERS of Birth Control | Weight Gain, Depression & Appetite Changes

In this episode of Abbey’s Kitchen, Abbey is joined by our fav dieter, Debbie! Today Debbie is concerned that birth control is poisoning us and contributes to a ton of toxins in our body. Debbie claims the pill causes blood clots, stroke, depression, loss of libido and even weight gain. Abbey admits that contraceptives can be a very controversial and complex topic, and in the past the pill used to contain scary high doses of estrogen, but over time, scientists have developed much safer birth control pills that have been developed with fewer side effects. Debbie is still not convinced and blames everything on toxic hormones like estrogen and progesterone. In actuality, Abbey shares with Debbie a 2014 Cochrane review found that the combination of estrogen and progesterone found no significant difference in weight gain, but the largest difference in weight gain was about 4 pounds after 12 months. Debbie still sees this as significant and wants to KILL THE PILL. Abbey also found that those who gained the weight were in the less estrogen group, which counters the theory that estrogen causes the weight gain. So, then we assume progesterone is the culprit? However, another review found that the progesterone-only pill did not cause a significant gain weight. However, the only method of progesterone birth control that did cause weight gain was: depot-medroxyprogesterone acetate which found girls were gaining an average of 10 pounds in one year. Debbie still believes that all birth control is bad, and mentioned that her friend’s appetite increased and they bloated up. Abbey looked at the research and found that the majority of studies on appetite were solely perception based. Granted, high doses of estrogen tend to cause water retention, but most pills have pretty low doses. Recent studies have concluded that birth control doesn’t cause significant bloating. Debbie also claimed that birth control causes depression. Abbey checked out the same Danish study and it found that when they observed one million women taking birth control for 6 years, there was a significant correlation between birth control and clinical depression. This is not a new theory, but sadly women’s health is not always at the forefront in the medical community and sadly there’s a lot of politics that come into play. Debbie is no feminist but she’s sure that if men had to take the pill, there wouldn’t be all these side effects. Finally, Abbey and Debbie agree on something. Abbey has found some research with the male pill, but because of the ton of side effects, a lot of that research has been put on hold. It’s a man’s world. Abbey and Debbie both agree that there’s a lot more that needs to be done, but at least studies like the Danish one are starting to lift the veil and giving women a voice and more control over their choices. At the end of the day, it’s a risk and benefit analysis and it’s up to you to decide for yourself whether you can tolerate the side effects and whether it outweigh the risk of having an unwanted pregnancy. Contraception and Birth Control for USMLE Step 2

Simple handwritten Contraceptive (Birth Control) lecture for medical students taking the USMLE. Will be looking at how contraceptives work and the pharmacology. Also the different contraception methods, such as Oral contraceptive, Intrauterine Devices, Implants. Contraception is a method of females to prevent pregnancies. There are several contraceptive methods, however, there is no 100% full proof method. Tier 1 failure rate is less than 1%, Tier 2 have a contraceptive failure rate 2-3% and finally tier 3 is the barrier method and they are about 80% effective. Tier 1 is made up of Intrauterine devices (mirena, Copper), DMPA, Implanon (subcutaenous) and sterilization (tubal ligation or vesectomy). Tier 2 are the Oral Contraceptive Pills, Patches and Rings, and they are made up of comboniation or just progesterone. Tier 3 is the barrier methods include condoms, diaphragm and the spermicidies which prevents implantation. Can also prevent STDs and HPV. Intrauterine devices are implantedin the uterus. They are made up of mirena and copper. Mirena secretes levonogestrel which is a type of progesterone that has three effects. Thickens the cervical mucosa which prevents sperm form ascending into the uterus. The endometrial wall is also thickened that decreases the chance of implantation. Increase tubal motility so it is not in blastocyst and it can't implant into endometrium. Copper Intrauterine device leads to inflammation response which causes the same effects. Mirena can work up to five years, whereas copper can work for 10 years. In Mirena there will be decrease bleeding and cramping. However, Copper can increase bleeding and cramping. Contra-indications to Intrauterine Devices is pregnancy, history of breast cancer, leiomyomas, any uterine infection such as PID and endometritis, especially chlamydia. Any bleeding where the cause is unknown. Relative complications are anatomical uterine anomalies (size and shape), Heart Disease because sometimes perforates and then bleeding can affect the heart. Any history of ectopic pregnancy. Finally there may be complications such as expulsion of the device within 1 month. Perforation, ectopic pregnancy, infection (PID) which usually happens within first 2 months. Sterilization and generally irreversible. Tubal ligation and vesectomy are the two methods. Vesectomies are more effective. TIER 2 are the hormonal methods there are two types which are known as combined and progesterone only. In combined there is pill patch and ring. In Progesterone only there is pill injection and implant. No LH surge means there is no ovulation and there is atrophy of the endometrium and therefore there is no implantation possible. Cervix becomes thick and edematous and prevents sperm from coming in. Combined method is when there is estrogen and progesterone. The pill can begin any day however effects won't begin for 7 days. 21 days and 7 days off. Off days are for the withdrawal bleed. Also helps regulate and normalize the menstrual cycle. The nuvaring enters the vagina and slowly releases estrogen and progesterone. Every three weeks it needs to be removed for 1 week. Patch also slowly release estrogen and progesterone under the skin which gives 60% greater than pills. Progestin only method in pill form is a daily pill that is continuous which causes irregular breakthrough bleeding. Injection form Depo-Medroxyprogesterone Acetate (DMPA) is injected into the skin and slowly released and it lasts for three months. Implanon is implanted subcutaneously and slowly releases etanogestrol and lasts for 3 years.

To pill or not to pill? That is the question. Hormonal Birth Control Pills Hormonal or chemical birth control pills will be explained in this video, with the pros and cons of each type. But before I demonstrate the different types, I would like to explain to you how hormonal birth control pills work. When a lady is pregnant, ovulation (or egg release) does not occur. That’s how our body’s system normally works. The hormones in a woman’s body are balanced in a certain way that makes the hormones that stimulate the release of the eggs – called the ovulation hormones – to become inactive during pregnancy. So we thought: If we give the woman pregnancy hormones in pills, we will be able to inhibit ovulation, in other words: to stop it from happening, which, in turn, would prevent the pregnancy to occur. So the scientists decided to put the pregnancy hormone and the female hormone in a pill. This gives us an idea about the first type of birth control pills. This type consists of synthet-ic forms of hormones – the pregnancy hormone progesterone (called progestin) and the female hormone oestrogen. On the other hand there’s another type of birth control pills that consists of the pregnancy hormone only, and in a very small amount. It is called the mini Pill or the Progesterone-Only Pill (P.O.P.). The pills I will talk about are not the only hormonal birth control methods. We can take other forms of chemical or birth control at the doctor’s office, which would have the same mechanism of action. So, back to the pills. Let me explain them now. Let’s start with a combined pill. “Combined” because it has a combination of hormones. Let’s know more about it. The combined pill consists of progestin – a synthetic progester-one (the pregnancy hormone) – and oestrogen (the female hormone). You will need to take a pill daily for 21 days, starting from the 7th day of your menstruation. Even if you’re still bleeding, you must start using the pill. Take a pill daily for 21 days and stop the pill for 7 days, taking into consideration that your period cycle is 28 days. The combined pill is a good birth control choice in cases of: • You don’t mind to take hormones for birth control • You usually suffer from pains [I’m not sure about what comes here → 2:40] during menstruating, since it will ease the dysmenorrhea. • If your period is not regular, this will regulate it. • You bleed heavily during the period: it will decrease the amount of bleeding, which in turn might prevent you from becoming anaemic. • If you have acne: this might improve your skin condition. • You’re not a smoker The combined pills are not good if you … don’t wish to take any hormones … are Breastfeeding … are a heart patient … have hypertension or high blood pressure … have risk of thrombosis or atherosclerosis … have had diabetes for more than 20 years … have a liver or gall bladder disease … have a history of breast cancer in the past 5 years … are suffering from recurrent migraines or have the symptoms that proceed mi-graine, called the aura … are overweight … have been taking medications such as antibiotics, epilepsy medications or anti-depressants All these may weaken the effect of the combined pill. Always read the patient in-formation leaflets. This birth control method is 99% effective, on condition that the pill is taken on the same time every day. The other type is the mini Pill or the pill that... It is not a good idea to use estrogen and progesterone birth control when you smoke. If you have a history of blood clot formation, whether during pregnancy, spontaneous blood clot formation, or with the use of oral combined birth control pills that have estrogen and progesterone, this may be a better birth control option for you.

This has been a topic that I've been researching over the past few months, and decided that it was something that needed to be discussed on FemmeHead because hardly anyone talks about it. Why is female libido so taboo? There was a study done in Germany on medical students to access the impact of HBC on libido. http://bit.ly/2m20rx7 They took out women using multiple forms as well as those not sexually active. Out of the remaining 1046 participants + 32.4% were at risk of FSD + 5.8% at high risk of hypoactive sexual desire disorder + 1% for arousal disorder + 1.2% for decreased lubrication + 8.7% for orgasm disorder + 2.6% for satisfaction problems + 1.1% for pain Then they divided them further into four groups: + hormonal oral contraception + non-oral hormonal contraception + non-hormonal contraception + no contraception I couldn’t figure out if they categorized FAM as non-hormonal or no contraception. Lowest to Highest risk of FSD: + non-hormonal contraception + no contraception + hormonal oral contraception + non-oral hormonal contraception For desire and arousal oral contraceptive users and non-oral hormonal contraceptive users were at the highest risk. Can contraceptive injections affect chances of conception in future? - Dr. Shefali Tyagi

Contraceptive injections are another method of contraception in which the progesterone is given as injection and it works for either 8 or 12 week. The side effects of such injections can be that the periods can become irregular. Sometimes you even stop having your periods. There can be weight gain. There can be effect on your bone. That means the bone can become slightly weaker. There can be breast tenderness, mood swings. So these are the common side effects. Now if you can start conceiving after stopping the injections, the answer would be that if you are planning pregnancy, don’t go for those injections as periods can become very irregular. Sometimes the cycles get back to their normal regular rhythmic flow upto a year after stopping the injections. So if you are planning another baby, then probably the injectable contraceptives will not be the best choice for you. You can either go for Copper T or contraceptive pills because in that case, the fertility is not affected so much. So it might take a little while to become pregnant after stopping these injectable contraceptives.

I felt exactly the same as when I started taking the birth control pills many years ago. it's all due to too much progesterone. If your progesterone is normal or normal/high, think very carefully before trying Vitex. It's very likely that it will just make things worse for you. I get asked frequently about my thoughts on IUD's. Naturally, I wanted to start a conversation about them. Here's is the first video of two (probably more) where I talk about Mirena and Skyla, two hormonal IUD's. In an upcoming video I'll cover the Paragard. I want an open, honest discussion about birth control because I feel like so often we get a one-sided, back of the box description of them. Share your thoughts and experiences, ask me you questions and let's get this conversation rolling! Additional Mirena/Skyla information: How do they insert it? -You have to go into a health care office. They will insert speculum, clean vagina and cervix with antiseptic, then use a special instrument to align cervical canal and uterine cavity, and another to measure depth of uterus. Then they’ll fold down the arms, place it in an applicator tube, place that tube in cervical canal and place IUD in uterus, then trim string so it doesn’t hang too far down in your vagina. You may experience dizziness, fainting, nausea, low blood pressure, slower heart rate, and often you will be asked to continue to lay down for a few minutes. Upkeep? -After each period you have to check for the string -Sometimes you will have a check up one month after insertion to make sure it is sitting right and you don’t have any infection. Side effects that need to be addressed by doctor: severe cramps, pelvic pain, extreme dizziness, heavy or ongoing bleeding, vaginal sores, foul discharge, severe pain in side or lower stomach, pale skin, weakness, easy bruising or bleeding, fever chills, or other signs of infection, pain during sex, sudden numbness/weakness on one side of body, severe headache, confusion, problems with vision, sensitivity to light, jaundice, allergic reaction Less serious side effects: irregular periods, changes in patter or flow, breakthrough bleeding, or heavier bleeding following insertion, back pain, headache, nervousness, mild dizziness, nausea, vomiting, bloating, breast tenderness or pain, weight game, acne, changes in hair growth, mood changes, loss of libido, mild itching, skin rash, puffiness in face, hands, ankles or feet. Precautions - Ectopic Pregnancy, General Pregnancy (may cause spontaneous abortion or preterm labor), pelvic inflammatory disease, Irregular Bleeding and Amenorrhea, Perforation (total or partial penetration/embedment of Mirena in uterus or cervix), Expulsion (Mirena coming out of uterus), Ovarian Cysts, Breast Cancer (if you have/had or have a potential of getting you should avoid hormonal BC because some breast cancers or hormone-sensitive) Do not get if you have abnormal vaginal bleeding, uncontrolled pelvic infection, a condition affecting shape of uterus, past, present or possible breast cancer, liver problems, cervical or uterine cancer, weak immune system, have had a serious pelvic infection following pregnancy or abortion, history of pelvic inflammatory disease, recent abnormal pap smear. In other words, if you use this birth control option and you decide that you want to get pregnant, it can take up to a year for your periods to return and for you to start ovulating in order for you to get pregnant. So if you're thinking in the next year or two that you would like to consider pregnancy, you probably should consider a different, more easily reversible form of birth control. Another concern about this form of birth control is the risk associated with bone loss. Now, the risk of bone loss is relatively low over the first year. But if you continue this form of birth control for two or more years, you may want to consider changing your options in order to reduce this risk from occurring. Again, I highly recommend that you talk to your nurse practitioner or your physician about whether this is a good birth control option for you. Despite the side effects, this may be the birth control option for you. And while you're concerned about the side effect profile, it's probably better to use this than to use nothing at all.

When you take a Combined tablet, both hormones are working on estrogens and progesterone and if you only take progesterone, then only one hormone is worked.   Progesterone: The hormone progesterone works at three different levels. First, it prevents the ovulation, which means that no eggs are released. The result is that fertilization does not happen and therefore, pregnancy is stopped. Second, it condenses the cervical mucus, so that sperm can not go into the uterus. Obviously, if you are fat, sperm can not take your way to the egg.
