Cleveland Clinic News Service - May 1, 2017 CLEVELAND - Skin cancer is the most common type of cancer and melanoma is the deadliest form. Only about 1 percent of all skin cancers are diagnosed as melanoma, but it’s responsible for most skin cancer deaths, according to the American Cancer Society. Brian Gastman, M.D. treats melanoma at Cleveland Clinic and said people are surviving longer these days thanks to newer treatments that boost the immune system to attack the cancer. “These immunotherapies, these therapies that increase the immune system’s strength if you will, they basically un-blind the immune system and they go after the tumor, anywhere the tumor may be,” said Dr. Gastman. While recent melanoma drugs have certainly helped move the needle when it comes to survival, Dr. Gastman says better biopsy techniques and improved surgical accuracy have contributed as well. When melanoma is found and treated early - before it has spread to the lymph nodes or farther - people live longer. Most melanomas are caused by sun exposure, so Dr. Gastman said it’s best to reduce the risk of developing melanoma by protecting skin from the sun. He recommends the following sun safety measures: - Seek shade when outdoors - Wear sun protective clothing - Wear a hat and sun glasses - Apply sunscreen; products labeled “broad spectrum” with an SPF of 30 or higher are best Dr. Gastman suggested keeping an eye out for situations that may expose skin to ultraviolet light in less obvious ways as well. “Let’s say you work in an office or a storefront and you’re near a window; you’re driving a lot and you’re near a window; you’re outside but you’re covered up, but the problem is your clothes don’t have UV protection. Those are times where you may be letting yourself get a risk, and you could prevent that,” said Dr. Gastman. Experts also recommend avoiding tanning beds. Tanning beds emit harmful ultraviolet rays and have been linked to an increased risk of melanoma. A bump that looks a little pearly, a bump that has blood vessels growing on it, or a bump that easily bleeds are all signs and symptoms of basal cell carcinoma. Only a biopsy can give you a definitive answer as to whether you have basal cell carcinoma or not, so if you have a suspicious lesion, see a dermatologist for a biopsy.

According to Melissa Piliang, M.D., of Cleveland Clinic, there are parts of our bodies that are regularly exposed to the sun during our everyday tasks and not protecting them can put our health at risk. “I think it’s important to put sunscreen on every day as part of your routine,” said Dr. Piliang. “The face, chest and back of the hands are places that you can get a lot of sun just running in and out of stores, to and from your car, to your mailbox; those kinds of situations.” Dr. Piliang said even if a person gets burned in just one small spot, it will still increase their risk of developing skin cancer in that spot, no matter how small. Spray-on sunscreens are a quick and easy way to get the job done. There are even a variety of lotions and makeups that contain SPF to protect the skin. Dr. Piliang also said it’s easy to overlook the fact that the top of our heads get a lot of sun exposure even just doing everyday things like walking from our cars to our destinations. She said it’s good to have a hat handy on those especially sunny summer days. “For people who are worried about sun exposure on their head, a hat is very helpful,” said Dr. Piliang. “Actually a hat is very helpful for everyone because it protects the face, the head, and the top of the ears.” According to the U.S. Centers for Disease Control and Prevention (CDC), skin cancer is the most common form of cancer in the U.S. and men, especially those with lighter skin, are more likely than anyone else to get skin cancer. Fortunately, most skin cancers can be prevented by protecting the skin from the sun. Melanoma: Causes and symptoms Symptoms of melanoma Causes of Melanoma Melanoma skin cancer Any changes to moles should be checked by a doctor As with other forms of cancer, the early stages of melanoma may not produce any symptoms. This is why actively checking skin to spot early signs of the disease is important. Changes in the appearance of the skin are key indicators of melanoma and are also used in the diagnostic process. The Melanoma Research Foundation has produced a web page that compares pictures of melanoma with those of normal moles. This American non-profit organization also lists the following symptoms and signs that should prompt a visit to the doctor: Skin change - a new spot or mole or a change in color, shape, or size of a current spot or mole. Skin sore that fails to heal. Spot or sore that becomes painful, itchy, tender, or bleeds. Spot or lump that looks shiny, waxy, smooth, or pale. A firm red lump that bleeds or appears ulcerated or crusty. A flat, red spot that is rough, dry, or scaly. The ABCDE examination of skin moles is also a key way to reveal suspect lesions (see below). Causes of melanoma As with all cancers, research is ongoing into the causes of melanoma. People with certain types of skin are more prone to developing melanoma, with the following factors associated with an increased incidence of skin cancer: High freckle density or tendency to develop freckles after sun exposure. High number of moles. Five or more atypical moles. Presence of actinic lentigines (small gray-brown spots also known as liver spots, sun spots, or age spots). Giant congenital melanocytic nevus (brown skin marks present at birth - birth marks). Pale skin that does not tan easily and burns; light-colored eyes. Red or light-colored hair. High sun exposure, particularly if it produces blistering sunburn, and especially if sun exposure is intermittent rather than regular. Age - risk increases with age. Family or personal history of melanoma. Being female. Having an organ transplant.

This will even out the skin colour, tone and texture and reduce the appearance of blood vessels, freckles, lentigos and pigmentation. Results can be seen after two to three treatments. Occasionally more are required. After treatment, the skin will look irritated, reddened and will peel. This usually resolves in 5 days, however may last 7-10 days. It is extremely important to stay out of the sun for forty-eight hours after treatment. Exposure can result in burning and redness. Even a sunscreen will not protect against the photosensitivity of the skin at this stage. Most people stay home and watch movies and relax for 48 hours. Photodynamic therapy is safe for all skin colours and types and is effective at all ages. It is safe to be treated while on other medications or treatments. Other methods of treating these types of lesions include dry ice or surgical removal, both of which can leave nasty reddened scars. Severe acne can also leave disfiguring scars all over the face and body. The amazing thing about Photodynamic therapy is that there is no scarring once the skin heals, leaving it looking smooth and healthy.
