Estrogen is produced by your ovaries all month long, but can also be produced by your fat cells. You can enter early perimenopause in your 30’s. In this stage, it’s not unusual to skip ovulation a few times a year. The missed ovulation means there is less progesterone produced, which leads to a condition called estrogen dominance. A symptom of estrogen dominance is weight gain in the hips and thighs. Menopause and Belly Fat As you age, your period becomes increasingly irregular, and then stops when you reach menopause. At and beyond menopause, both estrogen and progesterone are low. Your body tries to get more estrogen from your fat cells by encouraging fat production. Metabolic shifts caused by the low estrogen levels, shift your fat storage from your lower body to your belly, resulting in the menopausal belly fat that is a problem for so many post-menopausal women. Steroids are a group of hormones that regulate body functions – and they are all derived from cholesterol. Or fat. That’s right, fat. Estrogen, which is usually the focus of HRT, is actually a generic term for three different forms of hormone called Estriol, Estrodiol and Estrone - Estrodiol being the most potent. Estrogen is called the female sex hormone because it regulates the menstrual cycle and causes thing to grow during puberty – Dr Raj likes estrogen because it creates curves and it also stimulates cell growth in preparation for pregnancy. Estrogen is a growth hormone. It has received a lot of bad press because people have been taking too much estrogen in the form of synthetic hormone for a long period of time. And any hormone you take in excess is going to cause problems. And a growth hormone like estrogen if you take too much of it. You are going to get excessive cell growth which is another term for what we call cancer. So its understandable, estrogen is not good or bad, but if you take too much estrogen its going to cause problems. Progesterone is the pro gestational hormone. It’s the hormone that keeps the uterine lining intact if you get pregnant. And its depleted when you are under stress. So the typical reaction is high stress, high cortisol levels when you are under stress and the depletion of the progesterone. Its pretty amazing what progestronine helps with. It helps protect from many of the negative effects of extra estrogen. It helps women sleep. It gives them better sex drive, better energy levels, makes your skin look better. It’s a wonderful hormone. If we compare them side-by-side, you’ll see that they counterbalance each other in many ways. There’s an ideal ratio between these two hormones. Estrogen given in too large of a dose can cause depression, headaches, it increases the risk of blood clotting and stroke. It causes the loss of many nutrients such as zink and copper. It interferes with oxygen levels. Again that in a person that is getting excessive estrogen which has basically been the history of menopausal treatment. Dr Raj wants to leave you with an important point to free yourself from chronic fatigue and pain and depression and overweight. Ovarian hormones do not spontaneously malfunction by themselves. The female hormones do not all of a sudden malfunction by themselves. They are responding to stress as a stress hormone called cortisol. Every molecule of cortisol that you produce comes from progesterone. When you are under stress guess what happens? Your cortisol goes up and your progesterone drops. Dr Raj wants to test your cortisol and fix it. And he wants to test your female hormone and fix your whole hormonal system at the same time and make all your symptoms go away. PCOS makes it more difficult for the body to use the hormone insulin, which normally helps convert sugars and starches from foods into energy. This condition is called insulin resistance which can cause insulin and sugar to convert into glucose and to build up in the bloodstream. High insulin levels increase the production of male hormones called androgens. High androgen levels lead to symptoms such as body hair growth, acne, irregular periods and weight gain. Because the weight gain is triggered by male hormones, it is typically in the abdomen. That is where men tend to carry weight. Maybe you’ve gained 5 pounds in one day, your waist is much less defined, your rings no longer fit, and your shoes feel tighter. If you’re struggling with fluid retention you should know you’re not alone. Hi, I'm Robin Nielsen, the Chief Wellness Officer here at Insulite Health. I’m so glad you’re here. We’ve found that water retention, also known as edema or fluid retention is a very hot topic for women with PCOS and hormone imbalance, so we want you to understand what is happening and show you how you have all the power to reduce fluid retention in your body. And it can happen at any age. I can tell when I’m retaining water. Rings that were loose I can’t even put on my fingers. My face looks a little puffier and my shoes are tight. That’s because the extremities are the first to retain fluids due to less circulation in those areas…. When I was younger this happened for me all the time, and it’s a main reason I gained weight so easily. But as I learned why this happens and how to avoid it, I rarely retain water any more. You might have “cankels” – swollen ankles - where it’s hard to see the bones in your ankles. My mother and great grandmother had this symptom – and it’s so important to address because it’s a serious indication that something’s not right. Some other symptoms of fluid retention are: aching in the affected areas, feeling stiff in the joints, rapid weight gain over days or weeks, unexplained weight fluctuations, when you press the skin it may hold an indent for a few seconds. Fluid retention, bloating, and water retention are three terms for the same condition – excess fluids accumulating in your body’s cavities. Women with PCOS often report this troubling symptom and as we age it can become more and more of a problem. Imbalanced hormones, especially estrogen, might be affecting your body’s normalizing mechanisms, or it could be a side effect of birth control pills, which is one of the more common PCOS drugs. There can be many causes of fluid retention: insulin resistance, hot weather, standing for long periods of time, hormone fluctuations – particularly estrogen, the birth control pill, nutrient deficiencies, elevate cortisol (stress), chronic venous insufficiency where the weakened valves in the veins of the legs fail to efficiently return blood to the heart and certain medications. Insulin tells your kidneys to hang on to sodium and water which in turn causes fluid retention. In fact, it’s thought to affect almost 7% of pre-menopausal women in the US. But there is surprisingly limited information on how to treat it naturally. That's the topic of this video. What Is The Best Diet for PCOS? PCOS (Polycystic ovary syndrome) is a condition characterised by hormonal imbalances in women. Specifically, an imbalance in male hormones (or androgens) produced by the ovaries. Unfortunately there is no known cure yet, and the cause is unknown. However, genetic predisposition coupled with inadequate diet is thought to be a major driver. STUDY: http://www.ncbi.nlm.nih.gov/pubmed/23420000 Research shows the most effective eating pattern for PCOS is one that promotes weight loss and reduces levels of the hormone insulin. This is because PCOS coupled with weight gain drives insulin resistance. That’s why PCOS dramatically increases risk of type 2 diabetes and other metabolic health conditions. Additionally, studies show high insulin levels appear to upset regulation of the sex hormones. This worsens PCOS in a viscous cycle. Whichever diet or eating pattern helps you to successfully lose weight and lower insulin will have the best possible outcomes. Assuming you have been unsuccessful following a low-fat diet (which can work), the best alternative is a low carb diet. A low carb diet is an eating pattern where carbs make up roughly 30% or less of your energy intake. By comparison, the average American diet is around 60% carbs. Reducing carbs tends to equal a higher protein intake, shown to keep you feeling full for longer and reduce total calories consumed through the day. That’s why low carb diets can work well for weight loss when not counting calories. A ketogenic diet, which is an extremely low carb diet, has also shown to be promising, but it's incredibly hard to stick to long-term, so mch so that I don't typically recommend it. Aside from overall eating patterns, there are some common food concerns of PCOS patients that should be clarified. One of the main issues is dairy consumption. Some observational studies have linked low-fat dairy intake with increased risk of PCOS, while others suggest full-fat dairy has a protective effect. It’s really unclear at this stage, but we cannot draw any firm conclusions from observational studies. If low-fat dairy is indeed bad for PCOS, and full fat dairy is not, it’s likely because low-fat dairy and other “diet” products tend to have significantly more sugar. In any case, dairy-free is certainly worth a try if other changes have not helped. With a wholesome diet, dairy is not essential. There have also been some concerns with soy and PCOS, but again there isn’t much evidence to go by. It seems moderate soy consumption is likely safe, but at the same time it seems an unnecessary risk to eat a lot or increase your intake. STUDY: http://www.ncbi.nlm.nih.gov/pubmed/21793705 Several studies indicate those with PCOS tend to have high levels of a compound in the blood called homocysteine. High levels are thought to be an independent risk factor for heart disease and stroke The most efficient way to reduce homocysteine is by increasing folate or folic acid intake, which are forms of vitamin b9. Folate comes from whole foods, while folic acid is the synthetic version. The top folate sources per 100-gram serve are beans and lentils, raw spinach, asparagus, and romaine (cos) lettuce. Lastly, you must Limit Junk Food and Added Sugars, which are a nightmare for treating PCOS. They’re typically high in calories and added sugars, which can raise insulin levels and disrupt other hormones. Know that treating PCOS also involves reducing chronic stress, increasing physical activity levels and getting adequate sleep. Functions include: Puberty changes in women Fertility Plays a role in controlling cholesterol levels Helps bone health Affects the brain, including mood Also affects the heart, skin and other body tissues Affects heart and blood vessels In males, the maturation of sperm and labido maintenance Estrogen is produced by the ovaries, fat tissue, testis and adrenal glands. There are three kinds of estrogen: Estradiol - the primary kind of estrogen found in women during their fertile years Primarily produced in the ovaries, but also the placenta and adrenal glands Excess estradiol is implicated in many diseases, such as cancers and fibroids Found in both women and men Estriol - the estrogen associated with pregnancy A waste product of estrogen metabolism Estrone - the estrogen produced postmenopause Produced in tissues throughout the body, mainly fat and muscle tissues Known as the weakest form of estrogen Found in both women and men Estrogen levels change: Throughout the month when fertile, with peak estrogen in the middle of a woman’s menstrual cycle, signalling ovulation, and the lowest levels while bleeding. Estrogen levels drop after menopause. Problems can arise when your body produces too much or too little estrogen Too little - symptoms: Women Irregular periods Mood swings Loss of sexual desire Trouble sleeping Hot flashes Headaches Men Excess adipose tissue around the midsection Low sexual desire Too little causes: Anorexia Extreme exercise Medications Hypogonadism Post-childbirth Too much - symptoms: Women Irregular periods Weight gain Fatigue Low sex drive Depression and anxiety More intense premenstrual pain Cysts Men Erectile dysfunction Female characteristics, such as breasts Prostate enlargement and cancer Low sex drive Depression Fatigue Weight gain Too much causes: Obesity Puberty Diabetes Pregnancy High blood pressure Ovarian or adrenal gland tumors Medications When estrogen is broken down through aerobic metabolism, it can be broken into “good” or “bad” estrogen metabolites. The good metabolites, 2-hydroxyestrone (2-OHE), are estrogen antagonists. They have high antioxidant activity, providing protection for the heart and brain. The bad metabolites, 16-hydroxyestrone (16-OHE), are estrogen agonists. They have been implicated in causing cancer and weight gain. Therapies that increase the 2-OHE:16-OHE ratio are thought to decrease the risk for a number of estrogen-responsive cancers. Natural compounds found in food have been founds to be antiestrogenic and thus are being further explored to discover how to capture these benefits for possible cancer prevention and treatment. DIM (Diindolylmethane) DIM is a stable compound derived from the plant indole I3C (indole-3-carbinol) found in cruciferous vegetables. Dietary indoles found in cruciferous veggies such as broccoli and cabbage, have been found to exhibit chemoprotective and chemotherapeutic properties against many cancers. Its benefit has to do with hormone balancing. I3C has been found to decrease estrogen signaling and alter estrogen metabolism, leading to a reduced production of “bad” estrogen metabolites. DIM consumption has been linked to an increase in the production of the “good” estrogen metabolites and thus a resulting decrease in the production of the “bad” estrogen metabolites. -Increases the 2-OHE:16-OHE ratio One study on women with thyroid proliferative disease (TPD) found that supplemental DIM enhanced estrogen metabolism in such a way as to act as a dietary antiestrogenic supplement and in this way may decrease the likelihood of women to develop TPD. DIM has other hormone balancing properties and can contribute to more balanced hormones. -This more beneficial hormone balance can lead to a healthier metabolism The hormonal balance influence of DIM also positively impacts testosterone. Weight loss for women over 40 is quite a bit different than in your 20s or 30s. You might notice that as you get older, your hormones start to change, and what used to work before has just stopped working. In this video, you will learn: - Why it's actually desirable to gain 4-10 pounds of body fat during menopause - How hormones affect weight loss - How does body fat change with age? - How to lose weight, stay toned, and have great energy levels during menopause and beyond. In my seminars, I always say "hormones affect your body fat far more than calories". We know this intuitively, because for example, someone who is a type 1 diabetic can eat 5000, 10,000 or more calories per day, without putting on body fat. For that matter, they don't put on muscle either. In fact, without their insulin, they will just wither away and die. At the other end of the spectrum, you have a person who has a slow thyroid. This person can eat 800 calories per day, and still put on body fat. Those are of course extremes, but during menopause, the intimate balance of estrogen, progesterone and testosterone will affect your body fat. And frequently, cortisol, insulin and thyroid go along for the ride, and give you all those symptoms you don't want, like weight gain, hot flashes, low energy, poor sleep, etc. You'll notice that whereas in your 20s and 30s, you carried your body fat in your buns and thighs, and maybe your triceps, after 40, the fat seems to shift to your belly area. This is usually without any changes to your exercise or nutrition, which can be very frustrating. But to tell you the truth, if you were not overweight to begin with, it's actually desirable to gain 4-10 pounds of fat during perimenopause and menopause. Why? Because there are 3 organs that make estrogen: ovaries, adrenal glands, and body fat. Yes, body fat makes estrogen. But since the ovaries are no longer doing their job, body fat increases slightly to improve your estrogen status. So although you may not like the extra 4-10 pounds, it does result in better skin, better sleep, better mood, and fewer hot flashes. Again, I want to emphasize that you should gain those 4-10 pounds on condition that you were not overweight to begin with. So now you know how your body works, and why it does what it does. What do we do about it? When we work with our clients, we take them through a very thorough assessment to be able to customize and individualize the approach we take, because every woman is different. But here are the basics. First, we really need to tighten up your nutrition. We need to identify your food sensitivities, and remove them. Second, we need to eat more broccoli, cauliflower, Brussels Sprouts and cabbage. Those 4 vegetables have a natural chemical in them called "Indole-3-carbinol", which helps with estrogen balance. We also need to exercise properly. Strength training becomes particularly important after 40 because we want to maintain muscle mass, and bone mass. With lower estrogen levels, bone mass declines as well. We can prevent that through proper strength training (and let me tell you, a body pump class is not considered strength training. It's just cardio with weights. Not that it's bad for you, but it's not strength training). Also, strength training is important for looking good. You can stretch, and you can do cardio until the cows come home, but the only way to tone is through strength training. And lastly, we use certain supplementation strategies with our clients to support their body during these years. What we recommend is a high-quality multi-vitamin, a B complex, vitamin C, fish oil, and magnesium. The combination of these tends to be very helpful for energy levels, mental clarity, and of course, belly fat. Some tips to lose weight with an ovarian cyst. Ovarian cysts can form on or over the ovaries, and may be the result of a hormonal imbalance. Chronic problems with ovarian cysts or numerous cysts in the ovaries may indicate a condition called polycystic ovarian syndrome, or PCOS. The PCOS Foundation reports that between 5 and 10 percent of women of childbearing age suffer from PCOS. Approximately half of these women experience weight gain to the point of obesity. The reason for this is the hormonal imbalance often associated with this condition. One of the hormones affected is insulin, which changes the way sugars, starches and other foods are used and stored by the body. To lose weight with ovarian cysts, use some of the same techniques used by people who do not suffer from the condition. Step 1: Reduce your caloric intake. To lose weight, you need to create a calorie deficit. The National Institutes of Health maintain that a calorie deficit of 3,500 calories causes a weight loss of 1 pound. A calorie deficit of 500 calories each day adds up to 1 pound of weight loss each week. Step 2: Limit processed foods. The National Information Center for Women's Health recommends limiting processed foods to promote weight loss in women suffering from polycystic ovarian syndrome. Processed foods tend to contain added sugars. And as insulin levels change the way sugar is stored, you should minimize the amount of sugar you consume. Step 3: Minimize your intake of saturated fats. There is some evidence that minimizing saturated fat intake can help promote weight loss in women with PCOS, according to the McKinley Health Center at the University of Illinois. More studies are needed, but consider avoiding high-fat meats, poultry skin, and any foods high in saturated fats to help you lose weight. Step 4: Eat more fruits, vegetables and lean meats, advises the National Information Center on Women's Health. These foods are generally lower in calories and sodium than convenient foods and prepackaged foods. By increasing your intake of these foods, you will naturally reduce your caloric intake. Step 5: Do more exercise. The Cleveland Clinic suggests 20 to 30 minutes of exercise at least four to five times a week to encourage weight loss for women living with PCOS. Any type of exercise is beneficial, including swimming, walking, biking, jogging or playing tennis, basketball, soccer or other competitive sports. You want to be focusing on the below proportions of macronutrients on your plate to keep your blood sugar stable between meals: - 50% dark leafy greens or green vegetables - 25% protein (vegan or non-vegan) - 15% grains (quinoa, amaranth, buckwheat, brown rice are the gluten free grains you want to be focusing on) - 5% fats (coconut oil, avocado, olive oil, flaxseed oil, walnuts, almonds etc) - 5% sweet (front load your palette with something sweet to avoid cravings, add some dates, raisins, fruit, caramelised onions, honey or something sweet cooked into your meals) I will do a video on different meal suggestions soon... the idea is to eat as MUCH variety as possible.
