TFTs may be requested if a patient is thought to suffer from hyperthyroidism (overactive thyroid) or hypothyroidism (underactive thyroid), or to monitor the effectiveness of either thyroid-suppression or hormone replacement therapy. It is also requested routinely in conditions linked to thyroid disease, such as atrial fibrillation and anxiety disorder. A TFT panel typically includes thyroid hormones such as thyroid-stimulating hormone (TSH, thyrotropin) and thyroxine (T4), and triiodothyronine (T3) depending on local laboratory policy. Thyroid-stimulating hormone (TSH, thyrotropin) is generally increased in hypothyroidism and decreased in hyperthyroidism, making it the most important test for early detection of both of these conditions. The result of this assay is suggestive of the presence and cause of thyroid disease, since a measurement of elevated TSH generally indicates hypothyroidism, while a measurement of low TSH generally indicates hyperthyroidism. However, when TSH is measured by itself, it can yield misleading results, so additional thyroid function tests must be compared with the result of this test for accurate diagnosis. TSH is produced in the pituitary gland. The production of TSH is controlled by thyrotropin-releasing hormone (TRH), which is produced in the hypothalamus. TSH levels may be suppressed by excess free T3 (fT3) or free T4 (fT4) in the blood. Third generation TSH assay is the current requirement for modern standards of care. At present, TSH testing in the United States is typically carried out with automated platforms using advanced forms of immunometric assay. Nonetheless, there is currently no international standard for measurement of thyroid-stimulating hormone. Accurate interpretation takes a variety of factors into account, such as the thyroid hormones i.e. thyroxine (T4) and triiodothyronine (T3), current medical status (such as pregnancy), certain medications like propylthiouracil, temporal effects including circadian rhythm and hysteresis, and other past medical history. Free thyroxine (fT4) and Free triiodothyronine (fT3) are generally elevated in hyperthyroidism and decreased in hypothyroidism. An increased thyroxine-binding globulin results in an increased total thyroxine and total triiodothyronine without an actual increase in hormonal activity of thyroid hormones. Thyroid hormone uptake (Tuptake or T3 uptake) is a measure of the unbound thyroxine binding globulins in the blood, that is, the TBG that is unsaturated with thyroid hormone. Unsaturated TBG increases with decreased levels of thyroid hormones. The Free Thyroxine Index (FTI or T7) is obtained by multiplying the total T4 with T3 uptake. FTI is considered to be a more reliable indicator of thyroid status in the presence of abnormalities in plasma protein binding. This test is rarely used now that reliable free thyroxine and free triiodothyronine assays are routinely available. The Thyrotroph Thyroid Hormone Sensitivity Index (TTSI, also referred to as Thyrotroph T4 Resistance Index or TT4RI) was developed to enable fast screening for resistance to thyroid hormone. Somewhat similar to the TSH Index it is calculated from equilibrium values for TSH and FT4, however with a different equation. ► 8 Signs That You Have Too Much Estrogen In many cases, having too much of something in your body is bound to cause some sort of issue. Take estrogen for example: according to WebMD, too much estrogen can cause weight gain. In fact, they also add that many overweight women tend to have high estrogen levels. For those who may not be familiar with what estrogen is, it’s an entire class of related hormones which includes estradiol, estriol and estrone, according to WebMD. Estrogen levels tend to rise at certain times. The main time is during puberty, which is when a girl’s body is beginning to change. Another reason estrogen may rise is because of weight gain, which coincidently, is one of the symptoms of high estrogen levels. Below you will find the signs that you have too much estrogen. Abdominal Weight Gain: According to a study published in the Journal of Clinical Endocrinology and Metabolism, the fat tissue produces excess estrogen, mainly in the abdominal area, which can cause abdominal weight gain. As the fatty tissue produces estrogen, estrogen produces more fatty tissue, and if you have too much estrogen, this will cause weight gain in the area that has too much of it. Swollen or Tender Breasts: When you are pregnant, estrogen causes the breast ducts to enlarge. Also, progesterone, which is produced mainly in the ovaries following ovulation each month, can cause the breasts to feel sore, according to Healthline. They add that if you notice that your breasts are sore or swollen and you are not ovulating or pregnant, it could be a cause of too much estrogen. Fibrocystic Breasts: A woman’s breast changes occur in response to the hormones that are made by the ovaries. If a woman has fibrocystic breasts, it could be caused by high estrogen and low progesterone levels, according to Healthline. Women will experience breast tissue changes; they will become lumpy and rope-like. They will also be painful and tender to touch. Mood Swings: A hormonal imbalance can cause mood swings; which in this case would be too much estrogen. This occurs because it affects the chemistry of the brain, according to Healthline. Women who are pregnant or going through menopause can experience mood swings. Memory Problems: As I mentioned earlier, one of the signs of high estrogen is thyroid issues, which can result in hyperthyroidism. According to Healthline, hyperthyroidism can cause memory issues. This occurs because the body is unable to produce the hormone that is essential for energy metabolism. Headache: It is very common for women to experience headaches that are linked to hormonal fluctuations, according to Healthline. In this video will have a look at the Thyroid Gland and the process of thyroid hormone production. The Thyroid secretes hormones that govern many of the metabolic and growth functions in your body, the hormones are called thyroxine and triiodothyronine, commonly called T4 and T3, respectively. Under the influence of a hormone secreted by the Anterior Pituitary gland, which is called thyroid stimulating hormone, the thyroid will manufacture and secrete T3 and T4 thereby raising their blood levels. Thyroid hormone production: About one fifth of ingested iodides are selectively removed from the circulating blood by the cells of the thyroid gland and used for synthesis of the thyroid hormones. The iodides first have to leave the circulation and enter the thyroid cells. This is achieved by the action of a sodium-iodide symporter, which co-transports one iodide ion along with two sodium ions across the basolateral membrane into the thyroid cell. This symporter uses the sodium gradient that is created by another pump: the sodium-potassium ATPase pump. Once inside the follicular epithelial cells, Iodide is transported into the follicle: across the apical membrane: by pendrin. Thyroglobulin contain tyrosine amino acids to which the iodide ions will bind. T3 and T4 are produced within the thyroglobulin molecule. Conversion of the iodide ions to an oxidized form of iodine. This process is promoted by the enzyme peroxidase. The second steps is organification of thyroglobulin. Which means the Iodination of Tyrosine and Formation of the Thyroid Hormones. This process results in both monoiodotyrosine (MIT) and diiodotyrosine (DIT). DIT and MIT will combine to form T3 and T4. Another important thing to remember is the fact that not all DIT and MIT will combine, therefore the current thyroglobulin will contain all before mentioned forms: MIT, DIT, T3 and T4. Also the proportions in T3 and T4 are different: each thyroglobulin molecule contains up to 30 thyroxine (or T4) molecules and just a few triiodothyronine (or T3) molecules. Interestingly, in this form, the thyroid hormones are stored in the follicles in an amount sufficient to supply the body with its normal requirements of thyroid hormones for 2 to 3 months. Therefore, when synthesis of thyroid hormone is decreased, the physiologic effects of deficiency are not observed for several months. Now we will have a look at the process of releasing the hormones into the circulation. The apical surface of the thyroid cells close around the thryroglobulins. This creates pinocytic vesicles in the cells. The vesicles will fuse with lysosomes to digest and cleave the thyroglublin molecules and release thyroxine and triiodothyronine in free form. These T3 and T4 molecules will diffuse into the surrounding capillaries, so they enter the blood circulation. In the circulation, they will combine with plasma proteins known as thyroxine-binding globulin. Because about 75 percent of the iodinated tyrosine in the thyroglobulin will remain monoiodotyrosine and diiodotyrosine, this is not functional and therefore should not enter the circulation. Instead, the iodine is cleaved from them by a deiodinase enzyme, making the iodine available for recycling in new hormones. About 93 percent of the thyroid hormone released from the thyroid gland is normally thyroxine and only 7 percent is triiodothyronine. However, in the peripheral tissues, T4 will be turning into T3 for a great part: its deiodinated. So, simply said, the function of the thyroid gland is to take iodide, found in many foods, and convert it into thyroid hormones: thyroxine (T4) and triiodothyronine (T3). So what is the role of these thyroid hormones actually? Stress triggers adrenal hormones (cortisol, etc). This triggers the “flight or flight” response, which lowers thyroid hormone. BOOM Cortisol decreases TSH, lowering thyroid hormone. Chronically, this is what is seen in Hashimoto’s. Cortisol inhibits thyroid hormone conversion and increase rT3. The other most significant indirect effect the adrenals have on thyroid function is via their influence on blood sugar. High or low cortisol can cause hypoglycemica, hyperglycemia or both. Blood sugar imbalances cause hypothyroid symptoms. Adrenal stress has a lot to do with thyroid function. In order for thyroid hormone circulating in blood to have a physiological effect, it must first activate receptors on cells. Inflammatory cytokines (“fight or flight”) have been shown to effect these receptor cites. "If you’re familiar with insulin resistance, where the cells gradually lose their sensitivity to insulin, this is a similar pattern. It’s as if the thyroid hormone is knocking on the cell’s door, but the cells don’t answer. A perfect example of this in practice is the Hashimoto’s patient who is taking replacement hormones but still suffers from hypothyroid symptoms – often in spite of repeated changes in the dose and type of medication. So what we want is we want to see your numbers right in here right under the good spot of this curve and we call that good health. So let's take an example. And her doctor kept telling her oh you're looking fine you're looking fine everything's doing just OK. And all that was ever measured was are T.S.A.. And if you look at that T.S.A. number right there you'd say that looks great to me. Well here was the problem when we ran the rest of her thyroid panel look every single marker except for TSA which was out of range. This poor woman she wasn't. [00:01:28] Not only was she not making enough tea for but she was also not converting it into T-3. Remember that liver and the intestine they convert T4 into T-3 so that you can use it. And this person here she had elevated thyroid based antibodies and elevated thyroid globulin antibodies. This woman had Hashimoto's. So she has an auto immune disease against her own thyroid and you cannot treat that by taking Synthroid. Here are three different patients. This one has a low TSA TSA right there and this one's low. This one has a normal TSA H and this one has an elevated TSA H three completely different TSA H values. All three of them have low T-3. You can not use TSA as a reliable marker for measuring thyroid function. All of these people have low conversion and there are TSA agents all over the map. So here's my question to you. You got a little helicopter model here. There's ten pieces I just chose this because there are ten different markers in the thyroid panel that I run for patients. Now you've got to get into this helicopter and go someplace. I want to know which pieces are you going to leave out if you've got to get into the helicopter and fly which pieces are you going to leave on the runway. Yeah I didn't think so. Resistance to Hypothyroidism Medication and Adrenal Fatigue

A lot of people ask me while they’re on thyroid medication is why the dosage need to be adjusted after a little while. In another words, many people do experience the necessity to especially increase their thyroid medication for their hypothyroidism after they’ve been starting on the medication for some time. This can be three months, can be six months, can be a year, but over time they find that they need the ever-larger dose of thyroid medication in order to achieve the similar effect that they had when they first started. This is a very interesting question and it brings up the real underlying problem is that what is really going on in the thyroid gland? Now let’s take a look at physiology here. If you have a primary hypothyroidism, something’s wrong with the thyroid gland, it’s not making thyroid medication, then you give the body thyroid medication, you will feel brand new and you will feel significantly better. This is indeed the case for many reasons, whether you have primary or even secondary hypothyroidism, the fact that you put more thyroid medications into the body means that the body get increased metabolism and that leads to increase in energy, reduce in fatigue, reduce in constipation. And it’s a sense of well-being because it’s almost like you’re stepping on the gasoline pedal of the car, and the car is running a little faster. So you’re able to do more things during the day and less problematic. Now as this goes on, most patients who are on thyroid after initial adjustment will be quite satisfied. But they will find as time goes on their fatigue tend to return, the constipation tend to get worse. And they go see the doctor, laboratory tests are usually taken, and it could be normal but they are feeling not as good. So the doctor will invariably raise the thyroid medication a little bit. And then a few months later another little bit. Of course if the thyroid test is showing that it continue to be low then the doctor has more reasons to increase the dosage faster and stronger, and even including adding other types of hormones, including testosterone and other types of growth hormone, et cetera. But just on the thyroid alone, if the thyroid testing does not show any significant change but you still complain of being fatigued, then increasing thyroid dosage for you is quite common. The question is, why would that happen? And we have many reasons why to consider. First, there could be a resistance issue, meaning that the body is either getting used to it or it resists for the thyroid to actually get into the cell. There’s also a problem of conversion, for example, when thyroid medications need to be converted into different parts in order to work in the cells, and if there is a defect in this conversion then the body will not be able to get the thyroid that it needs. Number three, there could be also binding issues in the sense that thyroid medication could be bounded to certain proteins such as thyroid-binding globulins and then when it is binded it is not available to the cells. On top of that the body could be getting used to it. There could be receptor site issues, or simply adopt the body deteriorate with time. And so there are many, many factors that could cause this type of issue. The question is, how do you know which one is which? The laboratory test is usually not very good, is not sensitive enough to really be able to tell us in great detail which part of the mechanism may not be working well. So it really all comes down to detailed history, checking with your doctor on an ongoing basis, and asking the right questions. It is obvious that if you have to keep upping the dose eventually some people will get into trouble. Symptoms such as wired and tired are quite frequent, and that is an indication that thyroid medication may have reached a maximum, and you have to question whether this is even the right thing to do at that point in time. There are many, many possibilities, and we cannot cover them all here. But the important thing to understand is that if it is not converting, or your body needs more, there is a reason. The reasons are many including one which is often overlooked, which is adrenal fatigue. And so, see your doctor. Make sure you discuss these issues. And if there are no answers for you then look outside the box and look into talking to a adrenal fatigue literate doctor for more explanation, and to explore. The body reacts logically. If it is not working well there is a reason. So I encourage you to pursue that as the way to understand the root problem. And the only by solving the root problem will you get to recover from your illness on a long-term basis.
