Welcome to LY Med, where I go over everything you need to know for the USMLE STEP 1, with new videos every day. In this video, we will discuss electrolyte disturbances. Your kidneys are incredibly important in regulating electrolytes and if you have a disturbance in the levels, you can have symptoms! We'll talk about sodium first: Too little sodium is called hyponatremia and can be caused by loss of salt or changes in fluid. Thus we can break it down to hypovolemic, hypervolemic, and euvolemic. Common causes of hypervolemic hyponatremia is organ failure like in heart failure or kidney failure. Hypovolemic is caused by loss of fluid like in sweating, diarrhea, diuretics, or aldosterone insufficiency. Euvolemic is usually caused by too much ADH, or SIADH. Causes of SIADH include small cell carcinoma of the lung, CNS lesions and medications. Hypernatremia is from too much salt or a loss of fluid. Again, ADH plays a role. If you hae too little ADH, then you'll lose a ton of water. We call this diabetes insipidus. Now what causes this low ADH? Well CNS lesions can lead to a loss of ADH from pituitary damage. Another way you can get this is if your ADH receptors are mutated. We call this nephrogenic diabetes insipidus. How can you tell the two apart? Just give an ADH analogue like desmopressin! In central DI, it'll fix it. However, in nephrogenic DI it won't fix the problem. Instead, pharmacology that target's nephrogenic DI include thiazides and amilorides (K+ sparing diuretics). Done with sodium! Moving on to potassium disturbances! A lot of pumps use potassium, including the Na/K ATPase pump, the H/K+ pump, and also all those potassium pumps in your kidneys! Too little potassium is called hypokalemia. This is caused by increased insulin and sympathetic activation as it activates our Na/K+ pump. Alkalosis can also cause this, as well as increase renal excretion like in hyperaldosteronism, diuretic use, vomiting etc. What are some findings of hypokalemia? Well your cells are hyperpolarized and disrupts the membrane potential of your cardiocytes! This can lead to arrhythmia and you'll see U waves on ECG. Hyperkalemia is caused by low insulin, low sympathetic drive and acidosis. Also hypoaldosteronism (Addison's) and K sparing diuretics can cause this. Also know that cell lysis like in cancer or rhabdomyolysis or leukemia can cause this! When cells die, they release K+! What shows up symptomatically? Well you get arrhythmias here too! You get peaked T waves and prolonged QRS to the point that it looks like a sine wave! How do we treat it? We can give calcium to help stabilize the membrane. To finish off our this talk, we'll go over some last electrolytes. We'll start magnesium. Low magnesium is often concurrent with calcium and potassium loss leading to tetany and torsades de pointes. Too much acts as a depressant, leading to loss of deep tendon reflex and cardiac arrest. Phosphate: low leads to rickets and osteomalacia. High levels lead to renal stone and metastatic calcification. Calcium: they help stabilize the cells. Low levels lead to unstable cells and lead to tetany and seizures. You can see this with Trousseau sign, Chvosteks sign. In high calcium it hyperstabilizes cells, leading to abdominal pain, altered mental state, and bone pain. Can lead to calcium kidney stones. Common causes of calcium disturbances include inappropriate PTH, or acid base disturbances.

How to MAKE Your Body MORE Alkaline, Less Acidic to PREVENT CANCER with NATURAL CURES? Why pH is Crucial to Health? Most of us never consider the acid/alkaline balance of our blood, but a proper pH is a crucial aspect to health. Many doctors stress the importance of pH because a balanced pH protects us from the inside out. Disease and disorder, they say, cannot take root in a body whose pH is in balance. It is an imbalance of acidity and alkalinity that allows unhealthy organisms to flourish, damages tissues and organs and compromises the immune system. So what is proper pH? What we call pH is short for the potential of hydrogen. It is a measure of the acidity or alkalinity of our body’s fluids and tissues. It is measured on a scale from 0 to 14. The more acidic a solution is, the lower its pH. The more alkaline, the higher the number is. A pH of 7 is perfectly neutral. The healthiest pH is one that is slightly alkaline. Optimally, we want a pH of 7.365. This number will fluctuate throughout the day, but the normal range is between 6 and 7.5. What affects pH? Normally, the kidneys maintain our electrolyte levels, those of calcium, magnesium, potassium and sodium. When we are exposed to acidic substances, these electrolytes are used to combat acidity. High degrees of acidity force our bodies to rob minerals from the bones, cells, organs and tissues. Cells end up lacking enough minerals to properly dispose of waste or oxygenate completely. Vitamin absorption is compromised by mineral loss. Toxins and pathogens accumulate in the body and the immune system becomes suppressed. What causes acidity in your body? Even though there are many sources of acidity and toxicity in our environments, the biggest contributor to unbalanced pH is our diet. Fruits and vegetables contain potassium, a natural buffer to acidity. The western diet contains little in the way of fresh and raw fruits and vegetables. Processed foods contain tons of sodium chloride–table salt–which constricts blood vessels and creates acidity. Eating too much animal protein causes sulfuric acid to build up in the blood as amino acids are broken down. All grains, whole or not, create acidity in the body. Americans ingest most of their plant food quota in the form of processed corn or wheat. Our problem is more a matter of not taking in enough alkaline-promoting foods rather than taking in too much acid. Calcium-rich dairy products cause some of the highest rates of osteoporosis. That’s because they create acidity in the body! When your blood stream becomes too acidic, it will steal calcium (a more alkaline substance) from the bones to try to balance out the pH level. So the best way to prevent osteoporosis is to eat lots of alkaline green leafy veggies! Acid-Forming Foods Cold cuts Corn flakes Eggs Lentils Oats Conventional Meats: beef, chicken and pork Milk Pasta Peanuts and walnuts Rice White bread Whole wheat Certain foods on this list, like eggs and walnuts, might be acidic in your body, but don’t let that scare you away from eating them. They contain a host of health benefits like antioxidants and omega-3 fatty acids. A healthy balance is what we are shooting for where pH is concerned–it is possible to become too alkaline. Alkaline Foods Fruits, mushrooms and vegetables (especially citrus, dates, raisins and spinach) promote an alkaline pH. Strangely enough, acidic fruits such as grapefruit and tomatoes don’t create acidity in the body. They do just the opposite and contribute to an alkaline environment. It sounds like a good idea, and it is true that tissues and fluids must maintain a certain pH level to function properly. However, your body has mechanisms to keep pH levels in check — regardless of what you eat. Let’s take a closer look at the alkaline diet and break it down. Much of the discussion surrounding the alkaline diet focuses on the significant changes in the human diet over the last 10,000 years. Until a recent point on our evolutionary timeline, humans mostly consumed fruit, vegetables, and small amounts of lean protein. Once the food industry came into existence, we began eating more refined grains, fattier sources of meat, and processed foods that are high in sodium, saturated fat, and refined sugars — all of which cause inflammation and contribute to serious, chronic diseases. The alkaline diet discourages such foods. If you’re researching the alkaline diet, you may have noticed some sources describe it using lofty words like “miraculous.” In reality, there are many variables, diet-related and otherwise, that affect your health. In many ways, the alkaline diet is excellent. It’s balanced and encourages the consumption of raw fruits and vegetables. It’s definitely a healthy plan that may reduce your risk of developing chronic, diet-related diseases. But, does it live up to all the claims? What Is the Alkaline Diet? Also called the acid-ash diet, the alkaline diet promotes foods alleged to influence acid-base homeostasis in the body. Specifically, whole, raw, organic fruits and vegetables. These foods are thought to produce a “net alkaline effect.” Advocates of the diet say that an alkaline environment within the body helps weight loss efforts, increases energy, and can boost the immune system. However, despite its popularity, the data has not substantiated such claims. In actuality, it’s difficult to say if the alkaline diet has any real effect on the body’s pH balance. How pH Affects Your Health To understand how the body’s internal pH levels affect your health, it helps to have an elementary understanding of what pH is. The pH scale runs from 0 (acidic) to 14 (basic or alkaline); 7 is neutral. Pure water has a pH of 7. Sulphuric acid has a pH of almost 0, and is both extremely acidic and highly corrosive. At the opposite end of the scale, lye has a pH of nearly 14. Lye is also highly corrosive, but that’s because it’s extremely alkaline. The acidity or alkalinity of a substance can be balanced with a buffer that resists changes in pH. Alternatively, you can add a substance with a pH on the opposite side of the scale to try and get closer to a pH of 7; i.e. adding a base to an acid, or vice versa. The human body needs to maintain a pH in a narrow range, between 7.35 and 7.45, to function properly. This pH range provides the ideal working environment for the body’s fluids and tissues. Your stomach is acidic, about 3.5 on the pH scale, and it must remain so in order to break down food. Blood is mostly neutral—a characteristic that enables it to transport various substances around the body without affecting or reacting to them. The wrong pH in a given system or organ can seriously affect your health. For instance, the inside of your stomach is lined with a thick layer of mucus that protects it from being digested along with your lunch. But, the small intestine isn’t as well protected. Instead, alkaline bile is released into the small intestine to buffer and balance the acidity of the gastric acid secreted from the stomach. Dr. Bohac is discovering and developing new varieties of sweet potatoes that I hope will expand the limited choices now available to most of us. These range from very sweet, moist ones to drier, lighter-fleshed varieties that make excellent stand-ins for white potatoes."You can survive on sweet potatoes during periods of starvation," says Dr. Bohac. "They're a far more complete food than rice." Under a grant from the McKnight Foundation in Minnesota, Dr. Bohac is now working with researchers in Uganda to develop improved sweet potato varieties with more beta-carotene and resistance to devastating pests. Simply Delicious Recipes In general, bake moist, orange-fleshed varieties at about 400 degrees F for 40 to 50 minutes (prick first with a fork). They need no added sugar. Drier types can be microwaved for 5 to 10 minutes on high, depending on their size; prick with a fork before cooking. Or, cut them up, and pan-fry them in a little olive oil. Mashed sweets. You can roast or boil peeled sweet potatoes until they're tender, then mash them as you would white potatoes. Season with chopped garlic that you've sauteed quickly in a bit of olive oil. Roasted sweets. I peel and cube sweet potatoes, toss them in olive oil, salt, and pepper, spread them in a baking pan (don't crowd!), and roast them at 450 degrees F. Stir every 10 minutes until they're nicely browned.
