The report notes that for more than three decades, too much weight, too little exercise and unhealthy eating habits have been second only to tobacco as preventable causes of disease and death. Since the 1960s, the report says, tobacco use has declined by a third, but obesity rates have doubled. According to the report, 2 in 3 adults and 1 in 3 kids are overweight or obese, which places them at risk for not only heart disease and diabetes, but also cancer. After reviewing more than 7,000 studies, the report's researchers have identified six cancers associated with being overweight or obese: Esophageal adenocarcinoma, colon and rectal cancer, kidney cancer, pancreatic cancer, endometrial cancer and breast cancer among postmenopausal women. This review also finds convincing evidence of an association between lack of sufficient physical activity and increased risk of cancer for colon cancer. We must also consider the very small absolute risk for breast cancer in this population. These are women below the age of 50 years. In this population, there were about 1.3 extra cases of breast cancer per 10,000 women per year who used hormonal contraception. This amounts to about 1 extra case of breast cancer annually per 7700 users of hormonal contraception. Also important to consider are some of the counterbalancing benefits. Hormonal contraception has been linked to a reduced risk for some other cancers. For example, for ovarian cancer and endometrial cancer, meta-analyses suggest about a one-third reduction in risk and some studies show a reduction in colorectal cancer as well.[2] Overall, the meta-analyses do not suggest an increase in total cancer or total mortality in women who take hormonal contraceptives. Moreover, hormonal contraception is a very effective form of contraception that empowers women to take control of their reproduction. We also should take into account that pregnancy and childbirth themselves are not free of risk. For example, during pregnancy there is increased risk for thrombosis, particularly pulmonary embolism, and maternal mortality rates are appreciable. In developed countries, mortality rates average about 2 to 3 per 10,000 live births, and in low-income countries, rates are about 10 times higher.[3] Will this study change clinical practice? It is important for women to have this information and it may be important for shared decision-making, with women reviewing their options for contraceptives with their healthcare providers. But it is not likely to lead to major changes in clinical practice because of these very low absolute risks and the complex benefit-risk ratio. For older women, women who are in their 40s, taking contraception causes a higher baseline risk for breast cancer than that of younger women (in their 20s and 30s). Older women may also have a slightly higher risk for cardiovascular complications with these contraceptives. Of course, a decision about taking hormonal contraceptives needs to be made only after a careful discussion of benefits versus risks. Women should have this information, as well as information about other options that do not involve hormonal contraception, such as intrauterine devices without hormone release, tubal ligation, and vasectomy for their partner. It’s a risk for women that increase with age. Dr. Samith Sandadi, a gynecologic oncologist on the Lee Health medical staff, says about one in 100,000 women may be diagnosed with vaginal cancer. “What we mainly rely on is symptoms. The most common symptoms that usually effect older women, is vaginal bleeding.” Because vaginal cancer is one of the rarer forms of gynecologic cancers, doctors say unfortunately there’s no screening for it. But in some cases, doctors may be able to detect the cancer before symptoms begin. “If someone has had a history of cervical cancer or if someone has had a hysterectomy and at that time they had abnormal pap smears then yes, we can pick up some of these abnormal cells on a vaginal smear,” said Dr. Sandadi. Doctors say early detection is critical. There are two treatment options for vaginal cancer. If it’s caught early, patients may undergo surgery to remove the cancer. “However, if it’s a little more advanced and it’s a stage two then we probably rely on radiation therapy and chemotherapy,” said Dr. Sandadi. The most common cause for vaginal cancer is the HPV virus. “Recent statistics have shown that 80 percent of women have this virus but most women clear it. But in some women who are immune compromised or they have a persistent HPV virus that’s what usually causes vaginal cancer,” said Dr. Sandadi. Doctors say embarrassment and confusion can cause patients to ignore their symptoms. But it’s important if a woman experiences any irregular bleeding to make an appointment with her physician.
