Design Retrospective cohort study using an anonymised electronic health record case register. Setting South London and Maudsley National Health Service Trust, a large provider of inpatient and community mental healthcare in the UK. Participants 21 012 adults presenting to SLaM between 1 April 2006 and 31 March 2013 with unipolar depression. Exposure Prior antidepressant therapy recorded in electronic health records. Main outcome measure Time to subsequent diagnosis of mania or bipolar disorder from date of diagnosis of unipolar depression, censored at 31 March 2014. Methods Multivariable Cox regression analysis with age and gender as covariates. Results The overall incidence rate of mania/bipolar disorder was 10.9 per 1000 person-years. The peak incidence of mania/bipolar disorder incidence was seen in patients aged between 26 and 35 years. Prior antidepressant treatment was associated with an increased incidence of mania/bipolar disorder ranging from 13.1 to 19.1 per 1000 person-years. Multivariable analysis indicated a significant association with selective serotonin reuptake inhibitors and venlafaxine. Conclusions In people with unipolar depression, antidepressant treatment is associated with an increased risk of subsequent mania/bipolar disorder. These findings highlight the importance of considering risk factors for mania when treating people with depression.

Up to a fifth of patients on SSRI antidepressants do worse than with placebo. There is no way to tell who will do worse. It is interesting that no comparison was made with exercise and no credit was given to placebo in this big pharma sponsored study published in Archives of Psychiatry in December of 2011. Little mention was made of the side effects such as GI symptoms, fatigue, insomnia, suicidal thoughts and even suicide, impotence, seizures, osteoporosis, liver toxicity and much more. To us it seems far more reasonable to use placebo because the largest collection of clinical trials showed that these drugs are no more effective than placebo, but they have terrible side effects. I stopped taking Remeron for a couple of days because every time I took it I could feel it causing depression. Paradoxical effect they call it or poop out. I've been on it for 15 years so has it been with most of my medicine. Long term use. The couple days I had off of it were not that bad in terms of depression. I'm super super crabby so much I can hardly stand myself and my body feels like someone beat me up with a bat. My legs are even locking up. I'll take it just make this depression go away.

About six months ago my head started shaking. At the time, it was so mild, no one could notice it. It was just a light shaking sensation. But it has progressively become worse. I am in a constant state of shaking my head "no". This past week I was diagnosed with "cervical dystonic tremors", also refered to as dystonia. Major depressive episodes that occur in major depressive disorder can be triggered by stress but they're not always triggered by stress. Myth number three is that depression isn't dangerous or serious. Depression is quite serious. Even if you take away the risk of suicide and self-harm, depression still has a lot of consequences. It affects relationships, career, self-esteem, and how productive somebody can be. Myth number four is that you can't be depressed and anxious at the same time. Not only can somebody be depressed and anxious at the same time, it's actually remarkably common. Myth number five is that people with depression simply need to snap out of it. Someone can't just snap out of a major depressive episode or from depressive feelings. It simply has nothing to do with this disorder. Myth number six is that talking about depression will make the symptoms worse. We know that mental health counseling mental health talk therapy tends to be quite effective for major depressive disorder, so talking about it in that context is usually helpful. Myth number seven is that depression will go away by itself. Even though some depressive episodes may end without treatment, mental health treatment tends to reduce the duration of major depressive episodes. Treatment is the most effective way to reduce the symptoms of depression, not “toughing it out” or “waiting it out.”

To be called Major Depression, by DSM V criteria, you need to have at least five of these nine symptoms for over two weeks: S: Sad or irritable mood I: diminished interest or pleasure in things you usually would enjoy G: feelings of guilt or decreased self esteem, feelings of worthlessness or inadequacy E: Decreased energy or fatigue C: Decreased ability to concentrate or think clearly A: Changes in appetite; either increased or decreased P: Psychomotor retardation or activation S: Changes in sleep. Insomnia or hypersomnia. Especially mid-cycle insomnia S: Suicidal ideation or thoughts of death The World Health organization ranks Major Depression as the second most costly disorder worldwide, behind only Cardiovascular disease. Major Depression is twice as common in women. It’s thought that the reason for this is that men are less in touch with their emotions and cope with anger or alcoholism. As with all common emotional or mental health disorders, there are many screening tests for Major Depression On-line, so you can recognize it yourself. Feelings of guilt or decreased self esteem, feelings of inadequacy or worthlessness. Another symptom that is specific to Major Depression is feelings of Hopelessness, as if nothing is ever going to change for,the better. Guilt and decreased self esteem and feelings of hopelessness don’t occur when someone is Bereaved or sad following a loss. People are so often taught not to be in touch with their emotions or to be ‘strong, Suck it up!’ But being out of touch with your feelings is really just using Denial and the opposite of being strong. So the ‘brave’ or wise thing to do is acknowledge your emotional pain so that you can get it addressed and not have to carry around in our daily lives. Remember, when people suffer from untreated anxiety and depression, everyone in their lives suffer also. The way to treat Major Depression is the same as all Mental health disorders: First take away anything that is making it worse, such as alcohol and drugs of abuse. It’s very clear that alcoholism makes Depression much worse. The depression often gets tremendously better when people get their alcoholism addressed. Often, alcoholism is not the source of the Depression, but you have to turn off a broken pipe before you can clean up a mess. And really, anxiety and depression probably can’t be helped unless the alcohol or drug overuse is first addressed. The medications that are most effective for treating Major Depression are similar to the Meds we discussed for anxiety; the Serotonin medications or ‘antidepressants’. It doesn’t matter so much which antidepressant you chose; Zoloft, Prozac, Lexapro, Effexor...Anxiety and Depression are so common and such money makers that there are many effective choices. Even though there is a great need for new types of antidepressants that work by different mechanisms of action, the pharmaceutical industry is driven by profit only and so there have been no new types of antidepressants for decades. When one antidepressant like Celexa finally becomes generic, they just change it a little bit and call it Lexapro. Or when Effexor finally became generic, they just repackaged it and called it Pristiq. The important point is that there is not a shred of evidence that newer medications are any better than older generics, just extremely expensive. Even if your insurance company ‘covers it’, we all pay that exorbitant price for healthcare. As discussed in the previous videos, Anxiety is the mother of all symptoms that we suffer and Major depression and anxiety always occur together. I find people find much relief from their Depression when anxiety is also treated. Remember, antidepressants or Serotonin agents treat both anxiety and depression, but a medication such as low dose Clonazepam specifically for anxiety is very helpful. I am afraid this happens more often than we know. I could have very easily been placed in a psyche-ward or accidentally killed myself the first few days after taking that pill. Then, no one would be able to tell my story. What happened........ After suffering a chronic sleep disorder for decades, I couldn't get more than 4 hours of sleep a night so I went to the doctor for some medical help. September 23rd 2006: The doctor prescribed Lexapro (10mg). So I took this pill having been reassured by frequent television adverstisements that it was safe. My doctor also acted as if these pills were harmless. I took the pill and went to sleep about noon. 8 hours later I woke to the most awful, strange, powerful inner sensations. I was also filled with a feeling of dread or horror. I had never experienced anything like this before and I couldn't believe it. It felt as if my brain was dying...... I jumped out of bed and ran around my house.... I couldn't stop moving or the sensations would get worse. Sheer horror! The symptoms: Many are similar to Serotonin Syndrome. I've seen a lot of other videos on the topic of quitting the SSRI antidepressant Zoloft (sertraline) but not many on successfully getting through the withdrawals and coming through the other side safely. I hope this video is reassuring to anyone out there who is going through the same dilemma. If you have any question please feel free to ask/ or share your own experience. I'll explain what led me to do this and I hope it isn't too rambling. I have been taking Zoloft since 2011 and just recently have been told that manufacturing of the drug has ceased until Jan 2016. I don't know about the rest of the world but in Australia it has become like something out of Mad Max, with people trying to track down the last supplies. I tried the generic brand which I have been told is exactly the same. But after a month of this I began feeling more depressed/ sad than I ever have and began suffering from insomnia and blurred vision. The last straw for me was when I told my doctor of these changes and all I got in return was a cocky-bullshit answer that I must be imagining these changes. ''Yeah, well doc, I don't really want to imagine wanting to killing myself everyday, so politely go fuck yourself''. So I tried tappering off them from 100mg to 25mg (maybe to steep) and started noticing the withdrawals after a day. After a few days of this I stopped them completely and the dizziness became a constant thing. Week 2 brought with it the numbness and brain zaps. With week 3 came the insomnia/ aching joints. At about a month of stopping nearly all the signs have gone away. I may notice the dizziness if I move or look too quickly but you get used to doing things slow. So this video is just me telling you the withdrawal symptoms I experienced. It is not advised to just completely stop any antidepressant but I figured I was worse on the generic so I wanted to stop more than anything. I want to try and see what I am normal for a few months. The most important thing is being in the right headspace to do such a thing. Only try and go off them if you think you are ready. In my situation I have realised that most of the time if I feel anxious it is because of my surroundings and not what is going on in my head. Also avoid people who make you feel terrible. The world is made up of mainly idiots so it is more likely they have a problem and you don't. At least you have made the steps to help yourself. If I ever relapse into really bad depression/anxiety like I know that I can always go back on Zoloft. By then I think Pfizer would have mined the ''missing ingredients'' they need to make Zoloft from Mars.

Weeks after starting my newest round of medications (Seroquel & Nortryptaline) as a last ditch effort, I had my blood drawn for possible hormone imbalances and my Testosterone levels came back 200 ng/Dl and 150 ng/Dl. The average 25 year old male has 750 ng/Dl. With this discovery I for the first time had any type of possible explanation as to why I was not getting better and why I might be so so sick. The symptoms of such Low T are very similar to those of major depression. I started Testosterone replacement therapy soon after and have been checking in with the world and documenting my experience with treatment as well as giving my insight and perspective on various topics of mental health. I am blessed to say that I have slowly, over the last year and 10 months, been improving and becoming more stable which I never thought to be possible. My low T manifested itself in the form of Major depression, anxiety, and depersonalization/ derealization for over a year. Gaining some mental stability back is nothing short of a miracle as I was near death for what felt like forever. I do not consider myself to be totally healed yet but I am closer now then ever before and aim to use what I have been through to help or at least offer support to others in need I was able to successfully come off my Seroquel and Pamelor. I work out all the time as a part of my mental health recovery!!! Weight training and all kinds of cardio rule much of my free time!
