John M. Carethers, MD, PhD, John G. Searle Professor and chair of the Department of Internal Medicine at the University of Michigan, presents a talk about how inflammation and DNA mismatch repair influence the outcome of patients with colorectal cancer. Dr. Carethers' research interests include familial cancer and polyposis syndromes, mechanisms of tumor progression, tumor genetics, tumor markers, DNA mismatch repair, molecular pathology, TGF-beta superfamily signaling in cancer progression, and colorectal cancer disparities. This lecture was part of the University of Wisconsin-Madison Department of Medicine Grand Rounds series. The reasons are varied, but some groups get colorectal cancer more often than others. Ethnicity, race, and social status all play a part. 7. Family Medical History Most colon cancer occurs in people with no family history of the disease. But, colon cancer can run in the family. Whether you're at increased risk depends on which family member was diagnosed and at what age. 8. Gender You may have heard that men are more likely to get colorectal cancer than women. You may have heard it the other way around, too. Both statements can be true depending on the context. 9. Genetics Researchers estimate that about 25% of colon cancer cases have some sort of genetic link. Examples of the most common genetic causes of colon cancer include mutations leading to FAP (familial adenomatous polyposis) and HNPCC (hereditary non-polyposis colorectal cancer). 10. Inflammatory Bowel Disease (IBD) Inflammatory bowel disease, often characterized by conditions such as ulcerative colitis and Chron's disease, increases the risk of developing colorectal cancer. In general, the longer a person has had inflammatory bowel disease, the greater his or her chance of developing colorectal cancer. 11. Lack of Exercise There's no denying that exercise is good for you. The couch potato in me would like to see colon cancer research a study every once in a while declaring otherwise, but that just doesn't happen. Research has shown that a sedentary lifestyle contributes to colon cancer development. 12. Obesity Nobody likes to have the word obese thrown at them. Health Alert: Pescetarian diet lowers colon cancer risk
CINCINNATI (Liz Bonis) -- A new report said one diet stood out when it comes to fighting cancer. The diet stood out not because of what people take away but because of what people added to it. There's some thought people would all rather add to than take away for any health benefit. For example people would rather add red wine, to fight heart disease, to the burgers and fries rather than take away the fast food. So in this case, to fight colon cancer people might want to add a great catch and eat what's called a pescetarian diet. Seafood is the key ingredient in the pescetarian diet. It's really salmon, or other fish, with a vegetarian diet. So that means fruits and vegetables make up the bulk of the diet along with fish. Researchers said the combo was so effective for lowering colon cancer risk those who at that way had a bigger drop in colon cancer, than those who ate the vegan way. Vegan uses just plant proteins instead of fish such as beans and no animal products at all. Those who ate the pescetarian way dropped colon and rectal cancer risk 16 percent. It was likely that the seafood works against inflammation which has been linked to certain cancers. Inflammation is a silent marker in the blood that may play a role in several top risks such as cancer and heart disease. And that means people negate it by eating anti-inflammatory foods. But regular screening is also important. Testing can often find colon cancer early, when it's most treatable, or sometimes even prevent it altogether. What are the risk factors for colorectal cancer? A risk factor is anything that affects your chance of getting a disease such as cancer. Different cancers have different risk factors. For example, exposing skin to strong sunlight is a risk factor for skin cancer. Smoking is a risk factor for cancers of the lungs, larynx (voice box), mouth, throat, esophagus, kidneys, bladder, colon, and several other organs. But risk factors don't tell us everything. Having a risk factor, or even several risk factors, does not mean that you will get the disease. And some people who get the disease may not have any known risk factors. Even if a person with colorectal cancer has a risk factor, it is often very hard to know how much that risk factor may have contributed to the cancer. Researchers have found several risk factors that may increase a person's chance of developing colorectal polyps or colorectal cancer. Risk factors you cannot change Age Younger adults can develop colorectal cancer, but the chances increase markedly after age 50: About 9 out of 10 people diagnosed with colorectal cancer are at least 50 years old. Personal history of colorectal polyps or colorectal cancer If you have a history of adenomatous polyps (adenomas), you are at increased risk of developing colorectal cancer. This is especially true if the polyps are large or if there are many of them. If you have had colorectal cancer, even though it has been completely removed, you are more likely to develop new cancers in other areas of the colon and rectum. The chances of this happening are greater if you had your first colorectal cancer when you were younger. Personal history of inflammatory bowel disease Inflammatory bowel disease (IBD), which includes ulcerative colitis and Crohn's disease, is a condition in which the colon is inflamed over a long period of time. People who have had IBD for many years often develop dysplasia. Dysplasia is a term used to describe cells in the lining of the colon or rectum that look abnormal (but not like true cancer cells) when seen with a microscope. These cells can change into cancer over time. If you have IBD, your risk of developing colorectal cancer is increased, and you may need to start being screened for colorectal cancer at an earlier age and be screened on a more frequent basis (see the section, "Can colorectal polyps and cancer be found early?"). Inflammatory bowel disease is different from irritable bowel syndrome (IBS), which does not increase your risk for colorectal cancer. Family history of colorectal cancer Most colorectal cancers occur in people without a family history of colorectal cancer. Still, as many as 1 in 5 people who develop colorectal cancer have other family members who have been affected by this disease. People with a history of colorectal cancer or adenomatous polyps in one or more first-degree relatives (parents, siblings, or children) are at increased risk. The risk is about doubled in those with only one affected first-degree relative. It is even higher if that relative was diagnosed with cancer when they were younger than 45, or if more than one first-degree relative is affected. The reasons for the increased risk are not clear in all cases. Cancers can "run in the family" because of inherited genes, shared environmental factors, or some combination of these. If you have a family history of adenomatous polyps or colorectal cancer, you should talk with your doctor about the possible need to begin screening before age 50. If you have had adenomatous polyps or colorectal cancer, it's important to tell your close relatives so that they can pass along that information to their doctors and start screening at the right age.

The degree of risk depends upon the type of relative affected. • Personal History of Chronic Inflammatory Bowel Disease: • Age: the cells in a 70 year old body have had more time to accumulate the changes needed to form cancer cells but those in a child are much less likely to have acquired the required changes. More than 91% of patients are diagnosed with colorectal cancer over the age of 50 • Other reasons include • Excessive drinking of alcohol • Obesity • Physical Inactivity • Smoking • A diet very high in animal protein. • A diet very high in saturated fats. • A diet very low in dietary fiber. • A diet very high in calories. • Women who have had breast, ovary and uterus cancers. • Patients with ulcerative colitis. They examined data from more than 100,000 men and women for a 26-year period. Researchers found that both men and women who ate foods that were likely to cause inflammation, such as red meat, processed meat, refined grains and soda were more likely to develop colon cancer than those who ate a low inflammation diet. Those who ate “low-inflammation diets” had more leafy green vegetables and dark yellow vegetables such as carrots, squash and sweet potatoes. There is a word of caution since most cancer-causing polyps are a result of genetic changes that occur randomly in the cells lining the colon. They may not be preventable and a colonoscopy to find and remove them is critical to prevention. Since every colon cancer starts with a polyp, this study is saying that's a critical step all of us should do with this screening. It serves many important bodily functions, such as eliminating food and bodily waste and removing toxins from the intestines, lymph system, blood and more. The colon is also essential when it comes to proper digestion, which is key to our overall health. There are many health conditions related to colon health which we want to avoid, such as irritable bowel syndrome, celiac disease and colonic polyps. Fortunately, there are foods we can consume to increase the health of our colon and its proper functioning. Lets now discuss these foods for colon health. 1. First, fruits such as raspberries or blueberries are important for the overall health of our colon. One reason is because they are loaded with important fibre that can help flush our digestive system and promote bowel movement. Fibre is incredibly important for colon health. Raspberries, in just a one cup serving, contain about 8 grams of fibre. Along with this, they are also a great source of important vitamins, minerals and nutrients, as well as antioxidants and phytochemicals. Blueberries are especially well known for being an antioxidant powerhouse. 2. Dairy products such as yogurt and milk are also great for the overall health of our colon. This is due to the high amounts of vitamin D these products contain. Vitamin D may be able to help reduce the risk of colorectal cancer as some studies have indicated. Yogurt is especially helpful for the health of our colon since it contains healthy active bacterial cultures. These healthy bacteria help to maintain proper balance between good and bad bacteria in our bodies and support gastrointestinal health. If that wasn’t enough, dairy products also contain good amounts of calcium, which can also lower the risk of colon-related diseases and ailments. 3. If we want to improve the health of our colon, then salmon can be very beneficial for this goal. This is because salmon is a great source of healthy omega-3 fatty acids. Research suggests that when omega-3 fatty acids are consumed regularly, this may help to reduce inflammation through out the entire body, and perhaps more importantly, improve the overall function and health of colon cells. As well, salmon contains many different nutrients, as well as protein and antioxidants to support colon health. 4. Dark colored vegetables, such as broccoli and artichokes are also great for colon health for a number of different reasons. These vegetables are great sources of antioxidants and phytochemical as well as different vitamins, minerals and nutrients. Broccoli is a great source of vitamin C, a potent antioxidant. Important for overall colon health, these vegetables are packed healthy fibre. Artichokes are especially high in dietary fibre, with just one heart containing about 10 grams of fibre. These vegetables are more than just nutritious as they can help with digestion, promote bowel movements, help with the elimination of toxins in the body and may reduce the risk of colon cancer. 5. Lastly, for improved colon health, beans such as black and kidney beans are incredibly helpful in this regard. They are an excellent lean protein that can be consumed as an alternative to fatty red meats. They also contain excellent amounts of vitamins, minerals and nutrients, especially magnesium, iron and calcium. They are surprisingly high in antioxidants which can reduce inflammation through out the entire body as well as eliminate free radicals and reduce oxidative stress, benefiting our colon. These cells are specialized white blood cells that are capable of seeking out cells infected with viruses and killing them. They also help to destroy tumor cells. This means that ginger enhances your immune system boosting your ability to fight infection and disease. Another interesting effect of consuming ginger is that it warms the body and causes sweating. This may sound like an undesirable effect but it’s been shown that there is actually a germ fighting agent contained in sweat called dermcidin. Primarily because by changing our diet we can reduce risk dramatically.  Here are 7 proven steps you can take right now to promote a healthy anti-cancer intestinal environment and prevent colorectal cancer naturally: 1.Eat Less Red Meat.  Studies show that eating red meat “frequently” increases the incidence of colon cancer. Eating red meat daily, and especially more than one serving per day. Increased risk is associated with increased inflammation associated with chemicals released by digestion of red meat. These chemicals increase damage to and inhibit the repair of DNA in the cells lining your intestines. Whereas carcinoma invasion and metastasis rely on basement membrane (BM) disruption, experimental evidence is lacking regarding the potential contribution of epithelial cell/BM anchorage on inflammation onset and subsequent neoplastic transformation of inflammatory lesions. Herein, we analyse the role of the α6β4 integrin receptor found in hemidesmosomes that attach intestinal epithelial cells (IECs) to the laminin-containing BM. Conclusions We provide for the first time evidence that loss of IECs/BM interactions triggered by hemidesmosome disruption initiates the development of inflammatory lesions that progress into high-grade dysplasia and carcinoma. Colorectal neoplasia in our mouse models resemble that seen in patients with IBD, making them highly attractive for discovering more efficient therapies.

Robert Narayansingh and Robert Hurta show that cranberry extract and quercetin decreased the expression of COX 2. Furthermore, the researchers noticed a decrease in the expression of COX-2 in cells stimulated with a combination that promotes the growth of tumors. These findings are important because they demonstrate that F-kB is signaling pathways that suggest a possible link between inflammation and cancer and anti-inflammatory properties of oily cranberry extract or quercetin that can be used to modify these pathways — researchers said. Future studies in vivo models of colon cancer should be carried out in the near future — they conclude.
