Wrong dose, wrong insulin There are four basic types of insulin: Rapid and short-acting insulins are injected before a meal to cover the rise in blood sugar from the food you're about to eat; intermediate and long-acting insulins control blood sugar for up to 24 hours, covering periods of time when shorter-acting types have stopped working. People with type 1 diabetes often take a combination of shorter and longer-acting insulins and that's where mix-ups can happen. The fix: If this happens to you, eat enough carbohydrates to cover the extra insulin, keep monitoring your blood sugar, watch for signs of hypoglycemia (such as shaking, sweating or a headache) and call your doctor right away if you have any concerns. To prevent it from happening again, make sure you know which insulin is which and what the right dose is. 2. Exercising when insulin's peaking Hitting the gym or your local walking trail at the same time a dose of insulin is reaching peak effectiveness in your body could lead to low blood sugar. Exercise uses up blood sugar as well as glucose stored in the liver; as insulin "pulls" more blood sugar into cells, levels could drop dangerously. The goal is to inject the insulin into the subcutaneous tissue between the top layer of the skin, the dermis and the underlying muscle layer. The only concentration of insulin available in the United States is 100 units per milliliter. A milliliter is equal to a cubic centimeter. All insulin syringes are graduated to match this concentration. Insulin syringes are available in various volumes, for example: 3/10 cc, which would hold a maximum dose of 30 units, 1/2 cc to hold a maximum dose of 50 units and 1 cc to hold a maximum dose of 100 units. Some insulins are cloudy suspensions. To ensure uniform dispersion of the insulin in the cloudy suspension, roll the vial gently between your hands. Avoid vigorous shaking, which will produce air bubbles or foam and interfere with obtaining the accurate dose. Wipe off the top of the bottle with an alcohol swab. Discard the swab. Pick up the syringe and remove the needle cap. With the syringe held upright, pull the plunger back until the end of the plunger is at the mark of your dose,which in this example is 20 units. There is now air in the syringe. Check the insulin bottle to ensure you have the correct type of insulin. With the insulin bottle held firmly on a counter or tabletop, insert the needle through the rubber cap into the bottle. Push the plunger down so that the air goes from the syringe into the bottle. Turn the bottle upside down so that the air in it is at the top. With the tip of the needle kept in the liquid, pull the plunger back to your dose. If any air bubbles are in the syringe, push the plunger back in and draw up the dose again. Remove the syringe and needle from the bottle. Do not let the needle touch anything else until it touches your skin for the injection. Wipe the skin of the chosen injection site with an alcohol swab and let the skin dry. Pinch up the skin and put the needle fully through the skin at an 80 to 90 degree angle and push the plunger down completely. Discard the syringe and attached needle into a puncture-proof container and replace the container's screw cap. Instead of a puncture-proof container you can purchase a special "Sharps" container with a hinged lid to store your used syringes and needles. * When your storage or "Sharps" container is 3/4 full, dispose of it according to the policies of your local authorities. The recommended sites for insulin injections are shown. Change the place of each injection by moving a couple of inches from the previous site. By doing this, you can stay in one general area for several days.

The nurse correctly explains that it is: A. the time the insulin will be working in your bloodstream B. the time it takes for the insulin to reach the bloodstream C. the time the insulin is working hardest to lower your blood sugar D. none of the above Ans.: C. Peak hours of the insulin is described as the time the insulin is working its hardest to lower the blood sugar levels. I like this question a lot, since there are so many different types of insulin it is difficult to keep track of them. It is always really important though to keep in mind which insulin you are giving to your patient and also when that medication is going to be working the hardest. I have had several episodes working as a nurse, especially on night shift, where my patients blood sugar crashed in the middle of the night from lantus or intermediate acting insulin. Although my patient had received these medications hours before.... the effects had not reached their peak. Mix this with not eating a sufficient dinner and also missing their evening snack and your patients blood sugar can really go down. If my patient has a lot of insulin that they take on a normal basis. However, I notice that they are not eating that much - I usually communicate with them… they know their bodies better than anyone else --- especially if they have had diabetes and been taking insulin for a while. I will say to them: Your blood sugar is: 126, we have an order to give 10 units of humalog with your meal…. is this how much you usually eat? And if you were home, would you take this much insulin. A lot of times they will tell me that their endocrinologist suggest a smaller dose when they do not eat that much. So with this information I will page the doctor and see what they would like me to do. When it comes to insulin, you can never be too safe and giving too much insulin, which can induce a hypoglycemic event…. even coma can be extremely dangerous…. A recent study in the Journal of the American Medical Association said the price of insulin nearly tripled between 2002 and 2013, and three manufacturers -- Sanofi, Novo Nordisk and Eil Lilly -- have raised the list prices of their insulin in near unison, The New York Times reported. The price increases have triggered anger among patient groups and doctors, who say the rising cost of insulin appears to have little to do with higher production costs. "People who have to pay out of pocket for insulin are paying enormous prices, when they shouldn't be," Steve Berman, a lawyer for the patients, told The Times. The lawsuit alleges that the three companies increased the list prices on their insulin in order to win favor with pharmacy benefit managers, who team with drug makers and health insurers to determine how a drug will be covered on a list of approved drugs. While benefit managers are complicit, the lawsuit targets the drug makers because "they are playing the game, and they are the ones who publish the list price," Berman said. Representatives for the three drug companies could not immediately be reached for comment, The Times reported. This medication is always given with food so that it can help the cells absorb insulin, helping your body not have to work so hard to create it. And finally: Option F. Hypoxic States - In order to answer this question, you have to know what hypoxia means - the definition of hypoxia is when your body is not receiving enough oxygen. If your patient is not receiving enough oxygen, then they are most likely not in any state to eat; Remember back to the ABC’s which if you have not heard of this yet… then you will hear about this in nursing school: ABC’s Means Airway, Breathing and circulation…. when these are affected, these are the priorities and everything else can wait…. especially a metformine tablet.

The most important aspect of any drug is all drugs have what is called as the shelf life, which means their effectiveness is constant for a certain period of time, after which it will come down gradually. So what we usually see is the effect of insulin will be significantly reduced. Say for example if you take 5 units of insulin before its expiry, it will work for 5 units only. But of it is expired, you take 5 units it may be as effective as only one unit or even sometimes totally non effective also. The second aspect of any medicines is its preservatives that will be there. They have the ability to keep the insulin in a particular state that is in a liquid state for a longer period of time, beyond which the insulin may get precipitated. So your risk of developing some allergic reactions may go up and the insulin may totally not function at all. It is very important to understand this because the risk of taking an expired insulin is the sugars will usually go very high and in Type 1 diabetes where the insulin is a must, you are likely to get develop diabetic ketoacidosis, which is actually a very life threatening condition and needs hospital admission and ICU admission and sometimes even cause death as a complication. So never take expired insulin and when you buy, always look at the expiry of the insulin. Don’t buy any insulin that is likely to get expired in 15 days or 1 month because you need to understand how many days you will use insulin. So always preferably take insulin that is going to last for atleast 2 to 3 months whenever you see. So always look at the expiry date whenever you take any insulin and also for that matter any drug, always look at the expiry date.

5. Injecting with an insulin pen Before you inject, make sure there is enough insulin in the cartridge for your injection and check that the expiry date on the cartridge has not passed. Screw on the needle. Before each injection, make sure that your pen is giving insulin by dialing 1 or 2 units and, holding the pen upright, press the plunger: you should see a drop of insulin on the needle tip. After the plunger is pushed in, leave the needle in the skin for at least 10 seconds. This will allow the last drops of insulin to be completely injected. After you have finished, remove the tip and put it in a needle disposing bin. 6. How do I inject? If your nurse has asked you to inject using a skin fold, do so by taking a clean fold of skin between your thumb and index finger making sure that you only lift the skin and not the muscle beneath. Quickly push the needle straight into the skin fold as far as it will go and with your thumb on top of the plunger, smoothly inject all the insulin. You must continue to hold the skin fold throughout all the injection of the insulin. When finished, remove the needle slowly and only then, release the skin fold. 7. Where do I inject my insulin? The best areas where to inject are: - Stomach - Upper arms - Thighs - Buttocks Insulin is absorbed in different speeds depending on where you inject. So in order for your insulin to work the same way from one day to the next, always inject in the same area at the same time. Ask your diabetes specialist nurse which injection areas are best for you and write down the time of the day for each of them. 8. How and why do I rotate my injection sites? It is important that you don't keep injecting your insulin in the same place. Change sides using, for instance, your right thigh one day and your left thigh the following day and vice versa. Each time you inject in a site, make sure that you don't inject in the same place as the last time. Move the location of the injection by about a fingers width from the place you used the previous time. There are site rotation guides available to help you rotate your injections. Ask your nurse if you want to use them. If you always inject in the same place and/or reuse your needles more than once, your sites may become red, sore and lumpy. This is what we call lipodystrophy or lipo. Lipos can develop anywhere on your body where you inject. Insulin injected into a lipo may not work properly and it may affect your blood sugar level. This is why its very important to always rotate your injection sites and change your needle after each injection. 9. Will I always need insulin? In your case, you have what is called Type 1 diabetes. This means that your body completely stops making the insulin it needs to work properly and injecting insulin is the only way to replace what your body no longer makes. You may have heard about a person having diabetes and taking pills but it's not same diabetes. That's what we call Type 2 diabetes. 10. What makes your blood sugar level rise and fall? Looking after your diabetes isn't just about insulin injections. You should eat regular meals and avoid too many sugary foods and sweet drinks. Doing regular exercise several times a week will also help you to balance your blood sugar level. Controlling your diabetes is all about getting the balance right between insulin, food, and exercise! Imagine a submarine is your blood sugar level: When you eat or drink, your blood sugar level rises. When you inject insulin, your blood sugar level falls. If you exercise, your blood sugar level falls even faster. This is why balancing the amount of insulin, food and exercise are so important. Your blood sugar level should not be allowed to go too high or too low. By doing regular blood sugar level tests, your insulin dose can be adjusted to keep your blood sugar level as healthy as possible.
