Pain killers in pets are crucial in many conditions to help them remain comfortable. In this video I run through all you need to know about the most common ant-inflammatories in dogs and cats. Find out how they work, what side effects they might cause and why as well as what we can do to keep them a safe as possible and reduce their side effects. The most common pain killers in pets, both dogs and cats, are the non-steroidal antiinflammatory drugs, also known as the non-steroidals or NSAIDs. They are used as pain killers, to reduce inflammation and swelling and also to reduce a high temperature. While there are many different options for pain killers in dogs and cats, none of the others them have all of these effects. The most common uses of non-steroidals in dogs and cats are the management of chronic pain conditions such as arthritis as well as for pain relief following injury or surgery. These drugs also have some anti-cancer effects, an area of their use where research is ongoing and recommendations changing. There are many different drugs within the non-steroidal anti-inflammatory class including carprofen, meloxicam, firoxocib and mavacoxib to name but a few. These are then sold under many trade names including Rimadyl, Metacam, Previcox and Trocoxil. Non-steroidal anti-inflammatories work by stopping the enzymes from working that make pro-inflammatory chemical know as prostaglandins. These prostaglandins cause swelling, an increase in temperature as well as amplifying the pain signal sent to the brain by the local nerves. The enzymes that are blocked are known as the COX enzymes, specifically COX 1 and COX 2. COX 1 is actually responsible for the function of normal, healthy body processes. These include maintaining the intestinal lining and ensuring adequate blood flow to the kidneys. COX 2 however is the enzyme that goes into action whenever damage takes place and results in the production of these inflammatory prostaglandins resulting in pain and inflammation. We want our drugs to block COX 2 and not COX 1. Most readily accessible human NSAIDs do not do this in dogs and cats and as a result side effects are really common when given to our pets. Our pet specific anti-inflammatories however are considered COX 2 preferential or COX 1 sparing the most common side effect is mild gastrointestinal upset resulting in diarrhea and vomiting as well as lethargy and inappetence. This appears to happen in 2-9% of patients, with reports varying slightly. If treatment is continued despite vomiting or diarrhea being present then there is the risk of intestinal ulceration developing which may progress to full perforation. Other side effects do happen although are considered rare. Dogs are more prone to liver damage which occurs in around 1-in-2000 patients and is generally reversible so long as it is detected early and the damage is not too severe. Cats on the other hand are more prone to kidney damage. While this can happen following a single injection, kidney damage is very rare when our NSAIDs are used appropriately. Any patient being given these drugs should be monitored at home for side-effects and if there are any concerns then they should be stopped and you vet consulted. When it comes to finding a dog pain killer that works, what about giving your dog gabapentin? Is it a true pain killer and replacement for the more commonly used non-steroidal anti-inflammatory pain killers like rimadyl? Or is it like tramadol which works well in people but not in dogs? If a pain signal is blocked from reaching the brain then the result will be a reduction in pain. Gabapentin acts on the nerves themselves unlike some other pain killers like the non-steroidal anti-inflammatories (NSAIDs). Neuropathic pain is essentially pain that is generated by the nerves themselves and results in 2 main conditions.  The first of these is known as hyperalgesia which is an increased sensitivity to pain. The second condition is allodynia and is where something that is not normally painful becomes painful. The good news is that this pain wind up is reversible, although it can take several months.  Our standard pain killers in dogs and cats, the NSAIDs, will help with this.  Gabapentin though can also be used to help your dog or cat become pain free faster, or where our non-steroidals are not enough by themselves. Gabapentin may well also play a useful role in helping keep our pets comfortable after surgery or injury but should probably not be used by itself. Gabapentin is a very safe drug with few side effects to be concerned about.  The most common side effects of gabapentin in dogs and cats are sedation and wobbliness.  Some animals may also develop vomiting. The biggest danger with giving gabapentin is using a human liquid formulation that contains xylitol. Pain killers are so important but are they safe? More specifically is metacam safe and just how can we go about reducing pain killer side effects in cats? We used to think cats weren't too affected by pain. Well of course they are and now that our cats are living longer, many are suffering from the effects of chronic pain due to arthritis as well as following surgery or suffering injuries. Meloxicam is the active ingredient, with Metacam being the most common trade name product used. It is a member of the non-steroidal anti-inflammatory drug group (NSAIDs). Metacam's use in cats is highly controversial in some parts of the world, notably the US, while being completely accepted by the majority of vets in other parts, notably Europe and Australasia. This has led to different licensing of the product. In other words the same drug has different dosing regimes depending on where it is being used. No-one will deny that there are risks with giving any non-steroidal pain killer and cats do appear to be more sensitive to these drugs. It's been shown clearly that giving the highest licensed dose for just 3 days can cause mild changes in the intestines and kidney with 9 days of this treatment dose causing intestinal ulceration and peritonitis. The fact of the matter is though that for long term treatment, the licenced dose is 6 times less than this single treatment dose. The additional benefit of meloxicam being available as a liquid is that it gives us the ability to gradually reduce the dose give to the lowest effective dose. But how do we know that this is safe? there have been several studies looking at this with a couple of studies in Australia that looked at safety in both otherwise healthy older cats as well as older cats with existing kidney disease which concluded that rather than be dangerous, long term meloxicam may actually delay the worsening of disease in cats with otherwise stable chronic kidney failure. NSAIDs offer our cats one of the best chances of a pain free life. Be that during the recovery from surgery, getting over a ligament or muscular injury or living with arthritis long term. Yes, there are alternative pain killers but none of them are proven to work as well in cats, especially long term and in most instances they themselves are actually not licensed for this use. Pain has many detrimental effects, it has a massive impact on a cats quality of life and should not be ignored. Of course we don't want to use any drug irresponsibly and there are a number of actions we can take to minimize the risk of any side-effects. - Blood testing - stop if vomiting or diarrhoea - Stop if your cat appears depressed, becomes unwell or stops eating or drinking - Do not give a higher dose than your vet has prescribed - Do not give at the same time as steroids or any other NSAID - Don't use human pain killers unless specifically advised to by your vet (and never ever give your cat paracetamol). - Do contact your vet promptly if you have any concerns - If your cat is fat then give a dose approriate to their ideal weight rather than their actual weight. - Use other management and treatment strategies for arthritis and other chronic pain conditions to reduce the dose of NSAIDs needed to keep your pet pain free. My Dog Fainting?

My dog has been having this behavior for over a month now. We went to the vet twice. Blood work is normal. This started shortly after taking Meloxicam for a slight limp. He has been off of the medication for a month or so. He burps sometimes and sometimes his stomach makes noises. If I lay with him on his bed he usually can relax and sleep. His stools have been a bit soft and he's been on chicken and rice for a few weeks. The stool was getting better and I tried to start weening him back onto his dog food (solid gold Hundenflocken) and his stools started to get soft again. The vet is stumped and just wants to start doing tons of tests. Any educated ideas out there?

As per your ***EXOTIC Rabbit Savvy*** Veterinarian's prescription for the amt you need for your bunny, this is how to measure out the 1.5mg/ml (DOG strength) Metacam for your rabbit. Remember that, according to Boehringer Ingelheim, Metacam is light sensitive and will lose it's efficacy when exposed to light. So give your dose to your bunny and put the remainder back into the bottle asap. Note there should be no air bubbles in your 1ml; otherwise your bunny won't get the full dose of the medicine he / she needs. So make sure all of the bubbles are out of the medicine in the 1ml syringe before giving it. Notice the decimal points on the 1ml...0.05 ml is very different than 0.50 ml (which would be half a ml) for example. You want to make sure the dose you have been prescribed is correct and that you are giving the correct amt. It's advised also to give this medication after a bit of food to protect the tummy from getting an ulcer. 1ml is equal to (the same as) 1cc if your vet measures in cc. If your vet prescribes "drops" of metacam for your bunny, there is 0.05mg of meloxicam per drop (this is stated in the metacam pamphlet that comes with the metacam bottle). Some vets prescribe CAT strength metacam and it is not the same concentration as dog strength. It is weaker than 1.5mg / ml and your dose will differ from what you would give of this Dog strength concentration.
