Lung cancer, also known as lung carcinoma, is a malignant lung tumor characterized by uncontrolled cell growth in tissues of the lung. This growth can spread beyond the lung by the process of metastasis into nearby tissue or other parts of the body. Most cancers that start in the lung, known as primary lung cancers, are carcinomas. The two main types are small-cell lung carcinoma (SCLC) and non-small-cell lung carcinoma (NSCLC). The most common symptoms are coughing (including coughing up blood), weight loss, shortness of breath, and chest pains. The vast majority (85%) of cases of lung cancer are due to long-term tobacco smoking. About 10–15% of cases occur in people who have never smoked. These cases are often caused by a combination of genetic factors and exposure to radon gas, asbestos, second-hand smoke, or other forms of air pollution. Lung cancer may be seen on chest radiographs and computed tomography (CT) scans. The diagnosis is confirmed by biopsy which is usually performed by bronchoscopy or CT-guidance.

Biological markers or biomarkers are molecules that indicate normal or abnormal processes in your body. They are found in: - Blood - Urine - Stool - Other bodily fluids - Tissues 20+ biomarkers are currently used to help identify diseases such as: - Diabetes - Cardiovascular Disease - Cancer In cancer, biomarkers include substances produced by the cancer tissue or by other cells reacting to cancer in the body. They can be particularly helpful in: - Detecting and Diagnosing - Predicting responses to therapy - Tracking treatment results or cancer growth - Determining whether a cancer has returned following remission. There are 3 main tests: 1. Genetic – searches for abnormal changes and mutations, including extra, missing or incorrectly placed genes. 2. Philips and PathAI team up to improve breast cancer diagnosis using artificial intelligence technology in ‘big data’ pathology research. Royal Philips, a global leader in health technology, and PathAI, a company that develops artificial intelligence technology for pathology, are collaborating with the aim to develop solutions that improve the precision and accuracy of routine diagnosis of cancer and other diseases. The partnership aims to build deep learning applications in computational pathology enabling this form of artificial intelligence to be applied to massive pathology data sets to better inform diagnostic and treatment decisions. The initial focus of this effort is on developing applications to automatically detect and quantify cancerous lesions in breast cancer tissue. transcript: #ibelieve - Breast cancer is one of the largest medical problems faced today. One in eight women will be diagnosed with breast cancer at some point in their lifetime. Therefore it is an area where building new tools to more effectively diagnose the disease and cure the disease can have a major impact on a large proportion of the population. So the central mission of pathology has not change that much in the past few hundred years. It has always been to provide the most useful diagnosis for the patient. But what has changed tremendously is the amount of data at our disposal about the patient about the tissue sample including things like genomic data, transcriptome data, lots of different morphologic data types. And the goal of computational pathology is to enable pathologists to most effectively integrate these data to provide the most useful diagnosis for the patient. Over the past five years there have been huge advances in fields like artificial intelligence and high performance computing and one major aim of PathAI and Philips is to see how can we develop technologies integrating these new approaches to make significant impact on improving how we can diagnose diseases like breast cancer. In pathology, we hope that with the use of deep learning technology, we are able to make pathologists more accurate, efficient as well as consistent. The evolution of deep learning has been very staggering over the past few years. The performance has improved considerably such that the algorithms are even starting to challenge humans. We started with a large dataset that was already analyzed by pathologists. So pathologists got into that image and labeled this is cancer and this is normal. Then we extracted a lot of examples from these images and fed it into our deep learning systems. So they learned to identify cancer pretty good. But then we realized there was a number of areas where they are making mistakes. We took those regions and fed them back into the classifier to improve the performance of the system and that actually allowed the system to improve continuously, achieving an extremely high performance. They perform much better than any systems that people have seen previously in pathology. In terms of identifying where cancer is in a histopathology image. And the best systems did quite well by themselves and pathologists do quite well by themselves and make not than many errors. But the true exciting and potentially transformative thing was when you combine the results of the expert human pathologists with the results of the top performing systems you saw a significant reduction in the overall error rate, showing the best system is really a combination of the strengths of the pathologists with the strengths of the computational system. And that systems performs much better than either one alone. So the really exciting part is being able to integrate these systems into clinical workflows such that they can learn continuously from the pathologist. Enabling the pathologist to focus more on the complex tasks, while the machine handles the more rudimentary aspects. Partnerships are extremely important to building the best products for pathologists. Building effective tools in computational pathology requires expertise in clinical pathology and knowing the workflow of the clinic. And things like high performance computing and artificial intelligence. As well as the field of generally digital pathology and things like whole slide imaging. And to really execute most effectively in each of these different areas, partnerships really enable these things to happen. Therefore we are so thrilled to be working with Philips, who bring enormous expertise in many of these areas and we can really build great things together and will be much stronger than either one of us alone.

The immune system fights it by using white blood cells to attempt to destroy it. A team of scientists and engineers at The University of Texas at Austin has invented a powerful tool that rapidly and accurately identifies cancerous tissue during surgery, delivering results in about 10 seconds—more than 150 times as fast as existing technology. The MasSpec Pen is an innovative handheld instrument that gives surgeons precise diagnostic information about what tissue to cut or preserve, helping improve treatment and reduce the chances of cancer recurrence. The current state-of-the-art method for diagnosing cancers and determining the boundary between cancer and normal tissue during surgery, called Frozen Section Analysis, is slow and sometimes inaccurate. Each sample can take 30 minutes or more to prepare and interpret by a pathologist, which increases the risk to the patient of infection and negative effects of anesthesia. And for some types of cancers, frozen section interpretation can be difficult, yielding unreliable results in as many as 10 to 20 percent of cases. However, in tests on tissues removed from 253 human cancer patients, the MasSpec Pen took about 10 seconds to provide a diagnosis and was more than 96 percent accurate. The technology was also able to detect cancer in marginal regions between normal and cancer tissues that presented mixed cellular composition. The team expects to start testing this new technology during oncologic surgeries in 2018. Although maximizing cancer removal is critical to improve patient survival, removing too much healthy tissue can also have profound negative consequences for patients: For example, breast cancer patients could experience higher risk of painful side effects and nerve damage, in addition to aesthetic impacts. Thyroid cancer patients could lose speech ability or the ability to regulate the body’s calcium levels in ways that are important for muscle and nerve function. Physicians can operate the disposable handheld device easily. It requires simply holding the pen against the patient’s tissue, triggering the automated analysis using a foot pedal, and waiting a few seconds for a result. Meanwhile, the pen releases a drop of water onto the tissue, and small molecules migrate into the water. Sometimes tissue samples or biopsies are taken under Xray guidance especially if the tumor is located inside the body such as in lung or liver. So the question now arises, what is the importance of having cancer diagnosis? The answer to that question is that we need cancer diagnosis for many reasons such as confirmation of having cancer or not , locating the tumor itself, identifying the grade and markers of the tumor. The grade of cancer is essential as it determines the degree of aggressiveness of cancer and this has an implications on treatment as well as the prognosis of cancer. In a recent clinical research published on the journal Pathological Oncology Research in 2017, 121 patients with breast cancer were included in this study and it was found that tumour grade was one of the most important indicators for tumour aggressiveness and progression later after treatment. A handheld device can identify cancerous tissue in 10 seconds, according to scientists at the University of Texas. They say it could make surgery to remove a tumor quicker, safer and more precise. Tests, published in Science Translational Medicine, suggest the technology is accurate 96% of the time. The MasSpec Pen takes advantage of the unique metabolism of cancer cells. Their furious drive to grow and spread means their internal chemistry is very different to that of healthy tissue. How it works: The pen is touched on to a suspected cancer and releases a tiny droplet of water. Chemicals inside the living cells move into the droplet, which is then sucked back up the pen for analysis. The pen is plugged into a mass spectrometer - a piece of kit that can measure the mass of thousands of chemicals every second. It produces a chemical fingerprint that tells doctors whether they are looking at healthy tissue or cancer.

(Inside Science) -- An interview with Michael Giacomelli, a postdoctoral fellow from the Massachusetts Institute of Technology, on a new, faster cancer imaging technology. “The diagnosis of cancer and especially the evaluation of cancer that is removed from the body is 100-and-something-year-old technique that has advanced very little. “We’ve developed a technology based on ultrafast lasers where we scan specimens as they’re cut out of women undergoing surgery for breast cancer. “The advantage is we can generate images in seconds.” Why do current cancer-identifying techniques need improvement? “So, a typical histology requires overnight processing. So, patients are sent home basically after their surgery, and then after a processing delay, a physician looks at the tissue that was removed from the patient and determines whether or not they removed sufficient tissue. And if they did not, they actually bring the patient back. And in the case of breast cancer, they bring quite a lot of patients back and they re-operate on them if they didn’t cut enough tissue out. So, it’s a major problem actually, to try and make sure that an entire tumor is removed on the first go, if at all possible. “If you want to evaluate a piece of tissue during surgery, you have very limited options. And as a result, a lot of surgeries, and especially surgeries for breast cancer, are done almost entirely based on how the tissue looks to the surgeon’s eye and how it feels to their hand. There’s no microscopic imaging or advanced diagnostics that are readily available for the operating room. “The technology we’ve developed uses a technique called multiphoton microscopy, in which femtosecond laser pulses are focused deep in the tissue. What we’ve developed is a portable system. And it can be used to survey huge pieces of tissue in a matter of minutes.” What does the technology mean for breast cancer patients? “The entire reason that we have this problem with multiple surgeries is just that the slides take a day to generate. So, if you can make the slides in real time, or you can make an image that’s equivalent to the slides, you can simply look at the tissue as it is cut out of the patient and determine whether or not you got all of the cancer, or whether or not you’ve left some of the tumor in the patient.” Can it be used for other types of cancers? “There is a group at Memorial Sloan Kettering that is looking at skin cancer surgery using a related technique, and they are having a very high level of success. There are people looking at related techniques for things like prostate cancer as well.” Future Developments “In terms of technical developments, I think miniaturization is very important. The prototype instrument we’re running now is about 250 pounds and it looks about the size of a refrigerator. We’re working on versions now that are just a few pounds. “I’m also looking into ways that we can do in vivo imaging.
