In this stage, it’s not unusual to skip ovulation a few times a year. The missed ovulation means there is less progesterone produced, which leads to a condition called estrogen dominance. Steroids are a group of hormones that regulate body functions – and they are all derived from cholesterol. Or fat. That’s right, fat. Estrogen, which is usually the focus of HRT, is actually a generic term for three different forms of hormone called Estriol, Estrodiol and Estrone - Estrodiol being the most potent. Estrogen is called the female sex hormone because it regulates the menstrual cycle and causes thing to grow during puberty – Dr Raj likes estrogen because it creates curves and it also stimulates cell growth in preparation for pregnancy. Estrogen is a growth hormone. It has received a lot of bad press because people have been taking too much estrogen in the form of synthetic hormone for a long period of time. And any hormone you take in excess is going to cause problems. And a growth hormone like estrogen if you take too much of it. You are going to get excessive cell growth which is another term for what we call cancer. So its understandable, estrogen is not good or bad, but if you take too much estrogen its going to cause problems. Progesterone is the pro gestational hormone. It’s the hormone that keeps the uterine lining intact if you get pregnant. And its depleted when you are under stress. So the typical reaction is high stress, high cortisol levels when you are under stress and the depletion of the progesterone. Its pretty amazing what progestronine helps with. It helps protect from many of the negative effects of extra estrogen. It helps women sleep. It gives them better sex drive, better energy levels, makes your skin look better. It’s a wonderful hormone. If we compare them side-by-side, you’ll see that they counterbalance each other in many ways. There’s an ideal ratio between these two hormones. Estrogen given in too large of a dose can cause depression, headaches, it increases the risk of blood clotting and stroke. It causes the loss of many nutrients such as zink and copper. It interferes with oxygen levels. Again that in a person that is getting excessive estrogen which has basically been the history of menopausal treatment. Dr Raj wants to leave you with an important point to free yourself from chronic fatigue and pain and depression and overweight. Ovarian hormones do not spontaneously malfunction by themselves. The female hormones do not all of a sudden malfunction by themselves. They are responding to stress as a stress hormone called cortisol. High insulin levels increase the production of male hormones called androgens. High androgen levels lead to symptoms such as body hair growth, acne, irregular periods and weight gain. Because the weight gain is triggered by male hormones, it is typically in the abdomen. That is where men tend to carry weight. So, instead of having a pear shape, women with PCOS have more of an apple shape. You also need to keep a check on the food habits. Avoid dairy, chocolates and packaged food. PCOD or Polycystic Ovarian disease. 5 – 10 % in the reproductive age is suffering from thus problem. PCOD”S is because of hormonal imbalance. The ovaries normally secrete female hormones called estrogen and progesterone at a certain ratio and there is a small secretion of the male hormone called androgen. When there is a disturbance in this hormonal ratio, and where there is increase in the androgen, multiple follicles are formed in the surface of the ovary, that is why it is called as polycystic ovary. The reasons for this hormonal imbalance is not clear but there are some triggering factors, lifestyle, hormonal imbalance, or genetic. What are the symptoms of this polycystic ovary? There is irregular period or no periods art all, prolonged periods, scanty periods. It causes weight gain, acne. The male distribution of hair on the face, on the body, thinning of hair, when there is no ovulation in PCOD that causes infertility. When it comes to the diagnosis, the diagnosis is made by medical history, physical tests, and few lab tests for the hormonal levels. Last would be pelvic ultrasound scan here you can see multiple follicles in the polycystic ovaries. There is no treatment for polycystic ovaries. There are just curatives to cure the symptoms. Reduce your stress by doing yoga, exercise. Change your lifestyle. Avoid smoking or alcohol. Have a proper nutritional diet, a timely sleep is very important. Try to reduce your weight by exercises and dietary regime. Medical line of treatment will vary from person to person. If you are an unmarried girl, doctor will try to regularize your periods and reduce your Polycystic Ovarian Syndrome like acne and hair distribution. Alternative medical approaches uses plant based or phyto-hormones to balance out the system. And the science behind this all revolves around salivary tests. We can measure your hormone levels by looking at the saliva. The saliva is what is called a ‘dialisade’ of the blood. Meaning that certain molecules diffuse equally between the saliva and the blood and fortunately progesterone and estrogen are in that category. The ideal ratio of progesterone to estrogen is about a 22:1. Most women are way out of balance, having low progesterone. Many women are self-prescribing and taking progesterone in the form of creams. After reviewing thousands of lab tests I can tell you this strategy frequently backfires, in some women progesterone levels from the creams can build up so high that the progesterone starts to convert into estrogen and creates the very problem the woman was trying to avoid. So you can induce an estrogen-related problem, and potentially estrogen-related cancers, by taking too much progesterone. The most difficult patients I have ever worked with are women who have been on progesterone cream for too long. It’s even harder to manage than women who are on excessive estrogen. Another treatment I’ve seen in the alternative medical community to treat menopause is the use of soy products. Soy has natural phytoestrogens in it that can help but if you’re intolerant or allergic to soy then it will cause digestive or bloating symptoms that will outweigh the benefits. Progesterone can be a good treatment providing it is combined with sufficient testing, with adequate follow-up. Instead of relying on remedies that seem to alleviate symptoms not curing the underlying issues. Dr Raj wants you to know ovarian hormones and female hormones do not spontaneously malfunction by themselves. They are responding to stress as a stress hormone called cortisol. Every molecule of cortisol that you produce comes from progesterone. When you under stress guess what happens? Your cortisol goes up and your progesterone drops. Dr Raj can test your cortisol and fix it. He can test your female hormones and fix your whole hormonal system at the same time and eliminate all your symptoms. This will help eliminate depression and fatigue, night sweats, hot flashes, mood swings and it will improve your metabolism so you will able to start losing weight effectively. Pituitary gland can produce symptoms either hormone excess or decrement in the amount of hormones produced. It can also produce symptoms because of mechanical effects. If there is tumor in pituitary you can get headache and the tumor can also grow upwards ans suppress the opticazm where the optic nerves cross over. If there is a growth at this point then you can have bitemporal hemianopia so it becomes difficult to do tasks like driving and your visual feel may get contracted. So you will be able to see things in centre of you visual field not side, this is one important symptom of pituitary lesions. It can produce wide range of hormonal manifestations, very common pituitary disorders like prolactinoma. Among women it produces a symptom called emanoria that is either your cycles stop coming or become irregular and also they may be associated with headache and difficulty while conceiving which is a symptom that high prolactin can produce. Non functional pituitary tumors present with headache or detected incidentally on CT scan they do not produce much of symptoms. Postpartum pituitary necrosis which is a heavy bleeding during the time of delivery there is an injury to the pituitary gland, decreased hormone level is seen which causes lactational failure, irregular cycles or stopping of cycles. So these are the things we commonly see in adults. Congenital pituitary disorders are commonly present with poor growth, lack of sexual maturity. rarely we get cushing syndrome where there is excess cortisol production because of small tumor in pituitary and person becomes overweight, develops diabetes, excess facial hair growth. Acromegaly is also seen where there is an excess growth hormone hands and feet becomes large, jaw becomes more prominent and also again you get diabetes and high blood pressure. Hence these are the symptoms of pituitary gland disorder.

Water retention is a big deal with PCOS and hormone imbalance. It can make us feel fat, bloated, thick and unhealthy. Maybe you’ve gained 5 pounds in one day, your waist is much less defined, your rings no longer fit, and your shoes feel tighter. If you’re struggling with fluid retention you should know you’re not alone. Hi, I'm Robin Nielsen, the Chief Wellness Officer here at Insulite Health. I’m so glad you’re here. We’ve found that water retention, also known as edema or fluid retention is a very hot topic for women with PCOS and hormone imbalance, so we want you to understand what is happening and show you how you have all the power to reduce fluid retention in your body. And it can happen at any age. I can tell when I’m retaining water. Rings that were loose I can’t even put on my fingers. My face looks a little puffier and my shoes are tight. That’s because the extremities are the first to retain fluids due to less circulation in those areas…. When I was younger this happened for me all the time, and it’s a main reason I gained weight so easily. But as I learned why this happens and how to avoid it, I rarely retain water any more. You might have “cankels” – swollen ankles - where it’s hard to see the bones in your ankles. My mother and great grandmother had this symptom – and it’s so important to address because it’s a serious indication that something’s not right. Some other symptoms of fluid retention are: aching in the affected areas, feeling stiff in the joints, rapid weight gain over days or weeks, unexplained weight fluctuations, when you press the skin it may hold an indent for a few seconds. Fluid retention, bloating, and water retention are three terms for the same condition – excess fluids accumulating in your body’s cavities. Women with PCOS often report this troubling symptom and as we age it can become more and more of a problem. Imbalanced hormones, especially estrogen, might be affecting your body’s normalizing mechanisms, or it could be a side effect of birth control pills, which is one of the more common PCOS drugs. There can be many causes of fluid retention: insulin resistance, hot weather, standing for long periods of time, hormone fluctuations – particularly estrogen, the birth control pill, nutrient deficiencies, elevate cortisol (stress), chronic venous insufficiency where the weakened valves in the veins of the legs fail to efficiently return blood to the heart and certain medications. Insulin tells your kidneys to hang on to sodium and water which in turn causes fluid retention. So it’s critical to implement the PCOS 5-Element System to reduce fluid retention: 1) the right nutrients, it’s been found that many nutrients like magnesium, chromium, vanadium, carnitine, the B vitamins and more help to improve insulin sensitivity, reduce stress and support blood vessels better, 2) the right food – a diet high in refined, processed carbohydrates causes an spike in water retention, but a well-balanced whole food diet reduces insulin and it’s aging effects, 3) movement – using the right types of movement to improve insulin sensitivity and to balance hormones, 4) understand cravings so you’re not tempted to indulge in foods and toxic behaviors that don’t support you, 5) get some support! In fact, it’s thought to affect almost 7% of pre-menopausal women in the US. But there is surprisingly limited information on how to treat it naturally. That's the topic of this video. What Is The Best Diet for PCOS? PCOS (Polycystic ovary syndrome) is a condition characterised by hormonal imbalances in women. Specifically, an imbalance in male hormones (or androgens) produced by the ovaries. Unfortunately there is no known cure yet, and the cause is unknown. However, genetic predisposition coupled with inadequate diet is thought to be a major driver. STUDY: http://www.ncbi.nlm.nih.gov/pubmed/23420000 Research shows the most effective eating pattern for PCOS is one that promotes weight loss and reduces levels of the hormone insulin. This is because PCOS coupled with weight gain drives insulin resistance. That’s why PCOS dramatically increases risk of type 2 diabetes and other metabolic health conditions. Additionally, studies show high insulin levels appear to upset regulation of the sex hormones. This worsens PCOS in a viscous cycle. Whichever diet or eating pattern helps you to successfully lose weight and lower insulin will have the best possible outcomes. Assuming you have been unsuccessful following a low-fat diet (which can work), the best alternative is a low carb diet. A low carb diet is an eating pattern where carbs make up roughly 30% or less of your energy intake. By comparison, the average American diet is around 60% carbs. Reducing carbs tends to equal a higher protein intake, shown to keep you feeling full for longer and reduce total calories consumed through the day. That’s why low carb diets can work well for weight loss when not counting calories. A ketogenic diet, which is an extremely low carb diet, has also shown to be promising, but it's incredibly hard to stick to long-term, so mch so that I don't typically recommend it. Aside from overall eating patterns, there are some common food concerns of PCOS patients that should be clarified. One of the main issues is dairy consumption. Some observational studies have linked low-fat dairy intake with increased risk of PCOS, while others suggest full-fat dairy has a protective effect. It’s really unclear at this stage, but we cannot draw any firm conclusions from observational studies. If low-fat dairy is indeed bad for PCOS, and full fat dairy is not, it’s likely because low-fat dairy and other “diet” products tend to have significantly more sugar. In any case, dairy-free is certainly worth a try if other changes have not helped. With a wholesome diet, dairy is not essential. There have also been some concerns with soy and PCOS, but again there isn’t much evidence to go by. It seems moderate soy consumption is likely safe, but at the same time it seems an unnecessary risk to eat a lot or increase your intake. STUDY: http://www.ncbi.nlm.nih.gov/pubmed/21793705 Several studies indicate those with PCOS tend to have high levels of a compound in the blood called homocysteine. High levels are thought to be an independent risk factor for heart disease and stroke The most efficient way to reduce homocysteine is by increasing folate or folic acid intake, which are forms of vitamin b9. Folate comes from whole foods, while folic acid is the synthetic version. The top folate sources per 100-gram serve are beans and lentils, raw spinach, asparagus, and romaine (cos) lettuce. Lastly, you must Limit Junk Food and Added Sugars, which are a nightmare for treating PCOS. They’re typically high in calories and added sugars, which can raise insulin levels and disrupt other hormones. Know that treating PCOS also involves reducing chronic stress, increasing physical activity levels and getting adequate sleep. I felt exactly the same as when I started taking the birth control pills many years ago. it's all due to too much progesterone. If your progesterone is normal or normal/high, think very carefully before trying Vitex. It's very likely that it will just make things worse for you. Yet for women affected by this syndrome, its consequences are just as serious as it causes infertility, and if left untreated can also, over time, lead to diabetes and heart disease. Successful weight loss surgery in Mexico encompasses a number of health benefits that are byproducts of the weight loss process. Significant weight loss is known to lower high blood pressure and even eliminate the need for diabetes medicines, but it also improves infertility issues in obese women afflicted with PCOS. Gaining a tremendous amount of weight, particularly around the waist is a characteristic of PCOS. Women with PCOS have a difficult time losing that extra weight. Extra hair or its opposite, extreme thinning of hair, are other signs. Additional symptoms include acne or dark patches of skin, pelvic pain and sometimes depression. Researchers don’t know exactly why some women acquire PCOS, but high levels of insulin seek to be at the root of the illness. Being overweight also increases one’s chances of developing it. Genes also play a role, so if your mother or sister has PCOS, you are also more likely to develop it. Most women are diagnosed in their 20s or 30s, but even girls as young as 11 have been found to have it. Women afflicted with PCOS grow numerous small cysts on their ovaries. These cysts do not harm the ovaries but lead to hormone imbalances, as he body produces more of the male sex hormone androgen. This tendency can cause women to stop ovulating, have irregular periods, often less than nine per year, resulting in difficulty getting pregnant. In addition to the aforementioned symptoms, PCOS suffers tend to develop insulin resistance, causing blood sugar levels to go up and increase risk of developing diabetes. Symptoms tend to be mild at first but increase as time progresses. PCOS affects as many as 5 million women in the United States. To be diagnosed with the syndrome, women must have two of three symptoms, including a high level of androgen, infrequent and irregular periods and more than 12 cysts. Common treatments include birth control pills to regulate periods or other female hormones. The diabetes drug Metformin and Aldactone, a diuretic, are also used to treat PCOS. More than 60% of women with PCOS are classified as obese. Weight loss is touted as one of the best natural methods for improving fertility in PCOS patients especially when it occurs in conjunction with lifestyle changes such as a healthy diet and frequent exercise. A study published in 2015 the Journal of Clinical Endocrinology and Metabolism indicated that obese and overweight women who experienced significant weight loss had higher rates of conception whether or not they had PCOS.

The stress and the emotional changes that happen post marriage may be the cause of delayed, irregular periods, few months post marriage. Now if this is the only cause of this, then appropriate counseling and it would settle down gradually. Or there are the causes like polycystic ovary or weight gain, hormonal imbalance maybe the cause of regularly irregular period. So if it continues to be or if the patient is very obese, you have to rule out other causes of irregular periods and treat appropriately with or without hormone, weight reduction, counseling. These are the options. But just a single episode of irregular periods maybe just a change in the stress or the emotional behavior of the patient. Just get a withdrawal bleed to get off the period and once that is done, nothing else is required.

Primarily she exposes the risk of female sterilization causing hormonal imbalances (ovarian function being compromised) and the risk of female castration. This is a risk that women in 2016 still are not informed of at the time of their informed consent. Through the IRH, Dr. Hufnagel worked with California Senator Diane Watson (now Congresswoman Watson) (retired) and other dignitaries for the successful passage of the nation's first informed consent law, Senate Bill 835. Known as "Informed Consent to Hysterectomy", this law and subsequent laws makes it mandatory for physicians in the states of California, New York, and Texas to fully inform women on the complications of, and alternatives and options to, hysterectomy. Dr. Hufnagel’s whistle-blowing actions of advocating for women’s health rights, advocating for and creating informed consent laws, and exposing information about post tubal ligation syndrome (and exposing other abuses in women’s care) has caused her to be targeted by her peers and the medical community she exposes. The attacks upon her continue to this day and are ongoing. Books have been written and movies made that detail the abuses that occur to whistleblowers. Examples include "The Insider", "Silkwood" and “Erin Brockovich”. Whistleblowers are often harassed, threatened, victimized, and fired from their employment. The abuses that Dr. Hufnagel has faced for blowing the whistle on the American College of Obstetricians and Gynecologists and her obgyn peers for their routine practice of performing unnecessary hysterectomies, withholding information regarding side effects of tubal ligation/female sterilization, and other numerous abuses have been no different.

When Dr Raj organizes a treatment plan, he's attentive to the differences between individuals as there are lots of variation. In general, the most common types of women that Dr Raj works with are usually younger, 40 to 55 years of age, and are just starting to edge into the hormone problem situation, what is also labeled ‘perimenopause’. Let's unpack the two different groups of women that Dr Raj sees. Group A represents ‘alpha’ personalities. A woman in this group is likely to be a professional or very active, always on the go – focused on kids and career. She exercises a lot but can’t keep the weight off like she used to. She has trouble sleeping and may suffer from migraines. Inwardly she may feel emotionally desperate, intensely afraid of ‘melting down’ or never being able to get back on track. Her sex drive has deserted her and her hair is not far behind. These women may not be eating too much but they’re not eating right and their stress levels are way too high. Group B comes from the same age bracket but have very different feelings and history. These women are more like Sara, the female client Dr Raj spoke about in the last video. Physically exhausted. They’re depleted, have given up on life. If you’re in this group you’ve probably had food issues most of your life, maybe a food addiction and can’t find the motivation to tackle anything. These women are always gaining weight, always tired and always getting depressed. It’s a pretty consistent pattern. They’re usually starting to overeat or compulsively eat, on and off diets or they have a sweet tooth, or eat too many carbs, too much pasta – it’s finally starting to catch up with them and they’ve got a weight issue. This group of women usually respond really well to just the diet changes. And it’s not an easy thing to do because they’re usually addicted to the foods that they’re eating; they’ve been eating them for so long that it’s hard to stop; sugar addiction or compulsive overeating usually requires professional help from a nutritionist. The good news is that women in that age group respond quickly. In 3 to 6 months things start to turn around, assuming that they get their food under control, because usually at that age there’s something happening with food. But at that stage in our lives, our mid 40s to 50s, diet changes can do probably three-quarters of the job. It’s just getting a person to the point where they’re able to implement the diet changes. And usually it boils down to a really strong emotional attachment to certain kinds of foods. Dr Raj speaks more about food issues in a different video. Dr Raj believes if this one point sinks in, you can free yourself from chronic fatigue and pain and depression and being overweight: Ovarian hormones do not spontaneously malfunction by themselves. The female hormones do not all of a sudden malfunction by themselves. They are responding to stress as a stress hormone called cortisol. Every molecule of cortisol that you produce comes from progesterone. When you are under stress, your cortisol goes up and your progesterone drops. Dr Raj works with his clients in testing their cortisol and fixing it. He wants to test your female hormone and fix your whole hormonal system at the same time and make all your symptoms go away. That will help eliminate depression and fatigue. Night sweats, hot flashes, and mood swings and it will improve your metabolism so you will able to start losing weight.
